1.Comparison of the Sebum Excretion Rate and Follicular Density in Young Women With and Without Acne.

BACKGROUND: Acne, one af the commonest dermatological disorders, is a disease of the pilosebaceous unit, and the primarily involved site is the face, where this structure exists in maximal density. Among the many etiologieal factors of acne, changes in the kinetics of sebum secretion in acne patients have been described, but there is no report to compare follicular density and the sebum excretion rate in different facial regions between normal and acne patients. OBJECTIVES: The purpose of this study was to compare the sebum output and follicular density in different regions of the face in women with and without acne and to evaluate the differences between the two groups. METHODS: We studied 10 normal and 14 acneic women aged 19-27. Follicular density was determined by light microscopy counting pilosebaceous units on cyanoacrylate follicular biopsy specimens. The sebum excretion rate was calculated by an image analyzer with a sebum print on Sebutape. RESULTS: 1. Follicular density was not significantly different between the normal and acne group. The number of follicles decreased from the central to the lateral aspect of the face with the highest value being on the nose tip and the lowest on the lateral forehead. 2. The total sebum excretion rate and the number of actively secreting follicles showed different patterns in the two groups. There was a decreased value in the acne group in some central regions of face. In addition, central to lateral declining pattems, shown in the normd group, were not apparent in the acne group. 3. The follicular sebum excretion rate showed large variations in both groups, without apparent central to lateral declining patterns. The confluence of adjacent follicles seemed to produce falsely low or high values compared with previous studies. CONCLUSION: Sebum production is influenced both by the number of active follicles and their individual capacity to excrete sebum, and the total sebum excretion rate was lower than normal in low grade acne in this study. Obstruction of the outflow of sebum and regression of sebaceous glands due to comnlones may account for it.

4.A Clinical Study of Trochanteric Fracture of the Femur

It is generally accepted that trochanteric fractures are best treated by internal fixation as it provide satisfactory reduction and fixation of the fractures allowing early mobilization of the patient thus avoiding general and local complication following this sort of trauma. Fourty-seven cases of trochanteric fracture of the femur were treated at the Department of Orthopedic Surgery, Paik Hospital, Seoul, Inje Medical College from June 1974 to June 1982 and clinical analysis was made with particular emphasis on the comparision between compression hip screw fixation and Jewett nail technique. The result of the analysis are as follows: l. 63% of the male patient belongs to active age group in contrary to common belief in age incidence and the common causes of the injury was traffic accidents and fall from height. 2. By the Tronzo classification of the intertrochanteric fracture, the most common type is type III and type V was predomonant in the traffic accidents group and 87% of all cases belongs to unstable category. 3. Of the seventeen associated injuries, visceral injuries, fracture of the lower extremities, fractures of the upper extremities, fractures of the pelvis were observed in order of frequency. 4. More general complications were observed among the conservatively treated group and local complications occurred mainly in the group treated with Jewett naiL 5. The compression hip screw method seemed to be superior to Jewett nail owi#ng to shorter operation time, less blood loss and less complications. 6. Ender nail appears to be a promising method of treatment in the elderly patient because it had shorter operation time, negligible blood loss, minimal stress to the patient and early mobilization and weight bearing.

5.Congenital Quadricuspid Semilunar Valve : A case report.

A 17-year-old male patient was referred with symptoms of dyspnea. Multi-detector computerized tomography (MDCT) and echocardiography evaluation revealed quadricuspid aortic and pulmonary valves, an atrial septal defect (ASD), and pulmonary stenosis. We closed the ASD using a bovine patch and performed a commissurotomy of the pulmonary valve. Quadricuspid semilunar valves are very rare congenital abnormalities that are reported to occur nine times more frequently in the pulmonic valve than in the aortic valve. According to the Hurwitz and Roberts classification, the aortic valve was type A, and the pulmonic valve was type B. The aortic valve had normal function, but the pulmonic valve was stenotic and had abnormal function.

7.Erectile Response to Intracavernous Injection of Prostaglandin E1 or Papaverine in Impotence Patients with Diabetes Mellitus.

PURPOSE: To assess the erectile response to intracavernous injection of prostaglandin E1 (PGE 1) or papaverine in the impotence patients with diabetes mellitus (DM), and to determine the factors which would predict good response to either drug. MATERIALS AND METHODS: We injected papaverine (10~60 mg) or PGE1 (10~20 mg) into cavernosal tissues of 305 impotence patients with DM (mean age; 56.1 years). Erectile response was classified into poor, moderate, or good, and was comparatively analyzed according to kinds of vasoactive agents, patients` age, duration of DM and impotence, and treatment modality of DM. RESULTS: The frequency of good response to PGE1 was significantly higher than to papaverine (61.5% vs. 42.8%; p<0.05). However, no association between erectile response and patients` age, duration of impotence and DM, and treatment modality of DM was noted. CONCLUSIONS: Prostaglandin E1 seems to be more effective substance than papaverine for intracavernous injection therapy in impotence patients with DM.

8.Expression of Involucrin and Filaggrin in Various Skin Disorders: Immunohistochemical Study.

BACKGROUND: The cornified cell envelope (CE) is a unique structure found in the terminal differentiation of the skin. The precursor proteins of the CE are composed of many candidate structural proteins, among which invalucrin and filaggrin are important ones to participate in the complicated process of forming, the complex structure. OBJECTIVE: The purpose of this study was to evaluate the usefulness of expression of involucrin and filaggrin as markers of terminal differentiation in various skin disorders including tumors. METHODS: Immunohistochemical studies were performed in the formalin-fixed, paraffin-embedded skin samples of non-tumors (41 cases) and tumors (43 cases).

BACKGROUNDS: With the recent elucidation of genetic basis of Rh blood group, it is now available the molecular genotyping methods for Rh blood typing. These can be used when serological typing is difficult. This study was conducted to investigate the usefulness of Rh genotyping method for Koreans. METHODS: Genotyping for Rh C/c and E/e was performed in peripheral blood DNA samples from 34 blood donors by polymerase chain reaction using sequence-specific primers (PCR-SSP). The PCR determined genotypes were compared with serologically determined phenotypes. RESULTS: The Rh C/c and E/e genotyping results of 34 blood donors were full concordance with the results of their serologic phenotyping. CONCLUSIONS: Rh genotyping method on the basis of Rh genetic model can be applied to Koreans. This genotyping method would be useful tool in prenatal Rh typing of fetus at risk of hemolytic disease and when serotyping is not available for example massive transfusion.
