4.Screening Rates of Major Cancers after a Cancer Diagnosis in Adults in Korea.

BACKGROUND: With the number of cancer survivors increasing, follow-up care to deal with problems related to their original cancer is needed. One of these is screening for a second primary cancer as cancer survivors are one of the high-risk groups for cancer occurrence. The purpose of this study was to assess the screening rates of major cancers in patients with a history of cancer in Korea. METHODS: Our data were from the 4th Korea National Health and Nutrition Examination Survey. We narrowed the data to include 11,169 adults aged 40 and 80 years. Participants were grouped according to their history of cancer and the time since cancer diagnosis into no cancer group (no cancer diagnosis), cancer survivor group (cancer diagnosis > or =5 years ago), and cancer follow-up group (cancer diagnosis <5 years ago). We estimated the screening acceptance rates of major cancers according to the cancer history and assessed the relationships between them. RESULTS: The cancer screening rates of stomach, breast, cervix, and colon were 42.4+/-4.9%, 45.5+/-5.6%, 42.1+/-6.4%, and 24.1+/-3.8% for the cancer survivor group and 45.6+/-5.2%, 61.9+/-6.2%, 48.8+/-7.0%, and 20.8+/-4.3% for the cancer follow-up group. A history of cancer diagnosis was not related to the acceptance rate of stomach and cervical cancer screening. Breast cancer screening (odd ratio [OR], 1.783; 95% confidence interval [CI], 1.044-3.048) increased only in the cancer follow-up group. At 5 years after a cancer diagnosis, only the screening rate for colon cancer (OR, 1.701; 95% CI, 1.119-2.588) persistently increased compared to individuals without a history of cancer. CONCLUSIONS: The screening for breast cancer was the only screening examination whose rate increased in the cancer follow-up group, with the significance disappearing in the cancer survivor group. Our results demonstrate that the screening rate for secondary cancers is below optimal in cancer patients in Korea.

5.Acute Myocarditis Complicated by Permanent Complete AV Block Associated with Salmonella Group B Gastroenteritis.

Acute infectious myocarditis in children can be caused by many pathogens, including bacteria, viruses, fungi and protozoa. Patients may be asymptomatic or may have 'flu-like symptoms' to life-theratening arrhythmias or sudden death. We herein report a case of acute myocarditis complicated by complets and permanent AV block, associated with Salmonella group B gastroenteritis in a previously healthy 5-year-old boy. Presenting symptoms were dyspnea, cyanosis, vomiting, and diarrhea. Electrocardiography on admission showed very slow ventricular escape rhythm at 37 beats/minute. He was treated with antibiotics, inotropic agents, ventilatory support, and transvenous temporary pacemaker. Ten days later, permanent dual chamber pacemaker was implanted. All three consecutive stool cultures done after admission grew Salmonella group B. His ventricular function recovered rapidly and completely and he became fully active 2 months after the onset. However, his conduction system was totally destroyed and at last follw up evaluation 2 years after the onset, he is still totally dependent on the pacemaker without any escape rhythm.

6.Body Lateropulsion as an Isolated or Predominant Symptom of a Pontine Infarction .

BACKGROUND AND OBJECTIVES: Body lateropulsion with falling to one side is a well-known clinical feature of stroke in the posterior circulation. Body lateropulsion as an isolated or predominant manifestation of a pontine stroke has not previously been reported. To elucidate the possible mechanisms of patients presenting with body lateropulsion as an isolated or predominant symptom of isolated pontine infarction. MATERIALS AND METHOD: Between May 2004 and February 2006, out of 134 admitted patients with an isolated pontine stroke we identified 8 consecutive patients (6.0%) in the Keimyung University Stroke Registry who had body lateropulsion as the main presenting symptom. RESULTS: All lesions were localized to the paramedian tegmentum just ventral to the 4th ventricle. All except 1showed a uniform pattern of body lateropulsion, in which the direction of falling was away from the side of infarct. In 2 patients, body lateropulsion was the sole clinical manifestation, whereas the other patients had other neurological signs. All but 1 had contraversive tilting of the subjective visual vertical (SVV). In all cases, the direction of SVV tilt corresponded to the direction of body lateropulsion. The mean net tilt angle was 6.1 CONCLUSION: Based on the known anatomy of ascending vestibular pathways, the SVV tilting, and MRI findings, body lateropulsion probably results from damage to the graviceptive pathway ascending through paramedian pontine tegmentum.

7.CYP3A5*3 Polymorphism and Its Clinical Implications and Pharmacokinetic Role.

The cytochrome P450 (CYP) 3A subfamily is estimated to participate in the biotransformation of 50% of the currently prescribed drugs. Four members of the CYP3A subfamily have been identified in humans: CYP3A4, CYP3A5, CYP3A7, and CYP3A43. Initial data suggested that CYP3A5 accounts for only a small proportion of the total hepatic CYP3A in about 20% of samples, but it was later revealed that CYP3A5 represents more than 50% of the total CYP3A amount in some individuals. Several genetic variants have been described for the CYP3A5 gene, of which the CYP3A5*3 allele (gA6986G), the most common form and leading to the loss of CYP3A5 activity, has been extensively investigated in the aspect of pharmacokinetics and disease risk. This review summarized the molecular characteristics of the CYP3A5 gene, and discusses the association of the CYP3A5*3 polymorphism with disease risks such as cancer and hypertension, along with its role in the pharmacokinetics of CYP3A substrates.

9.A Study of Symptom of Health and Fatigue-regulation Behavior in Middle-aged Women.

The purpose of this study were to identify a relationship between symptom of health and fatigue-regulation behavior in middle-aged women. The subjects were 241 women living in Seoul and near the city, ranging in ages of 30-59(mean=42.1 years) being interviewed during the month of Oct. to Dec., 2000. The following questionnaires were utilized in this study: a self reported symptom of health and a fatigue-regulation behavior scale originated from Kwon(1997). The analysis for the data was done by SAS program, t-test, ANOVA, and Pearson correlation. The results of this study were as follows 1. Total mean scores of health status were shown 29.1, physical symptom 17.9 and psychological symptom 11.2. According to symptom of body system were higher in muscular-skeletal sympom 5.5, neurologic symptom 3.5, and digestive symptom 2.7. 2. The mean scores of symptom of health were significantly different in duraion of marriage, status of menstruation, perceived fatigue, perceived cause of fatigue, perceived help of family, hours of sleep. Physical symptom was significantly different in years of educational, status of menstruation, perceived fatigue, perceived cause of fatigue, perceived help of family, hours of sleep. Psychological symptom was significantly different in parity, whether or not having past illness, perceived fatigue. 3. The neurologic symptom was significantly different in years of education, status of menstruation, perceived fatigue, time to rest The respiratory symptom was significantly different in years of education, status of menstruation and hours of physical exercise. The muscular-skeletal symptom was significantly different duration of marriage, status of menstruation, BMI. perceived fatigue, perceived cause of fatigue, perceived help of family. The cardiovascular symptom was significantly different in whether or not having religion, number of children, duration of marriage, number of pregnancy & abortion, parity, status of menstruation, BMI. The digestive symptom was significantly different in whether or not having religion and the number of children. The urologic symptom was significantly different in whether or not having job, type of family, status of menstruation, BMI, perceived fatigue, perceived cause of fatigue, perceived help of family. The score of fatigue was significantly different in number of pregnancy. 4. There were significantly positive relationships in the area of symptom on health, especially fatigue was significantly positive relationships in score of symptom of health, physical symptom and psychological symptom. 5. They choose fatigue-regulation behavior physical rest, method of diversion, management of stress, enough sleep, and psychological rest in order. In the near future, it is required that further studies investigate socio-environmental factors related to symptoms of health and develop programs motivating fatigue-regulation behaviors actively.

10.The expression and tyrosine phosphorylation of E-cadherin, beta-, gamma-catenin and EGFR after treatment of EGF and TGF-alpha in Cervical Cancer Cell Lines.

OBJECTIVES: Cadherin/catenin adhesion complex is fundamentally involved in epithelial cancer invasion and metastasis. E-cadherin and EGFR colocalize on the basolateral membrane of epithelial cell and EGF down-regulate E-cadherin expression. In the invasion and metastasis of cancer, E-cadherin expression is decreased and growth factors receptor is overexpressed. The present study was aimed to find the role of E-cadherin, beta-and gamma-catenin, growth factors and its receptors in cervical cancer cell lines. METHODS: The cervical cancer cell cultures were treated with different time duration of EGF 30 ng/ml and TGF-a 10 ng/ml(0, 10 min, 20 min, 30 min, 1 hr, 2 hr, 4 hr, 8 hr, 24 hr). The change in cancer cell morphology and the changes in E-cadherin, beta- and gamma-catenin, EGFR and activated EGFR expression were studied with a western blot analysis and an immunoprecipitation. RESULTS: Through a western blot analysis, E-cadherin 120 kDa band and EGFR 170 kDa band were expressed in CaSki, HT-3 and ME-180 cell line, which showed epithelial contact growth. 1n these 3 cell lines, expression of E-cadherin did not decrease with time dependent manner. after the treatment of EGF and TGF- alpha. The expression of EGFR decreased and activated EGFR expression increased in 30 minutes to 1 hour but decreased subsequently. When the cells treated with EGF, there were no change in beta-and gamma-catenin expression with there dependent manner. The tyrosine phosphorylation of beta-and gamma-catenin increased in 30 minutes to 1 hour but decreased subsequently with activated EGFR. CONCLUSION: This study showed that an activated EGFR which has involved with tyrosine phosphorylation of beta- and gamma-catenin influenced by growth factors rather than expression of E-cadherin, has a role in the invasion and metastasis of the cervical cancer.
