1.Clinical Observation on Vesicoureteral Reflex Following Renal Tuberculosis.

Clinical observation on vesicoureteral reflux following renal tuberculosis were obtained 46 cases of renal tuberculosis seen at N. P. H. during the period from April 1971 to April 1975. 1) Demonstrable vesicoureteral reflux were 27 cases with cystogram, voiding cystogram, retrograde cystogram and I. V. P. 2) Retrograde cystogram showed a highest peak in occurrence of vesicoureteral reflux. 3) The incidence of bladder Tb. and vesicoureteral reflux secondary to renal Tb. among these patients, in 50% of the cases occurred from marked contracted bladder and 2% from the bladder having normal capacity. In the presence of renal Tb. and vesicoureteral reflux mentioned above, the most frequent observable change was in marked contracted bladder, and it showed that their incidence were increased by severity of extent of bladder contraction.

2.Primary Atypical Carcinoid Tumor of Liver: A case report.

Primary hepatic carcinoid tumors are extremely rare although the liver is a frequent site of metastases from intestinal carcinoids. Recently we investigated a case of primary hepatic atypical carcinoid in a 47-year-old man who had infested with Clonorchis sinensis for 20 years. The resected right lobe of the liver was almost completely occupied by a huge tumor, measuring 20 x 19 x 12 cm. The cut surfaces of the mass were solid, soft and pale yellow, accompanied by several small satellite nodules, measuring up to 1.5 cm in diameter. Microscopically, the tumor consisted of polygonal to columnar cells with eosinophilic granular cytoplasm forming Lym-numerous small acini and large trabeculae. Their nuclei were round to polygonal with coarse stone chromatin, had obscure to small nucleoli and frequent mitoses. There were multiple necrotic foci of varing sizes. The surrounding dilated bile ducts contained several degenerating worms on in of Clonorchis sinensis. The tumor cells were argyrophil-positive but argentaffin-negative. Immunohistochemically, the tumor cells were positive for cytokeratin, chromogranin and somatostatin but were negative for CEA, AFP, insulin, glucagon, ACTH, growth hormone and volve-prolactin. Ultrastructually, the tumor cells contained variable-sized numerous electron dense of neurosecretory granules.

3.Arthrolysis and Quadriceps Release in Stiff Knee: Report of Five Cases

Many procedures of quadricepsplasty were reported by Thompson (1944), Judet (1959), van Nes (1962), Nicoll (1963) and Hesketh (1963) in confining the indication of them for the strictly extra-articular origin of their causes. During the period of January 1979 to August 1979, 5 cases of stiff knee joints were surgically treated by means of arthrolysis and quadriceps release. The result were gratifying. The average gain of flexion was 119 degrees, and extension lag was absent except a case which had compression fracture of femoral condyle.

Hepatobiliary scintigraphy using Tc-labelled radiopharmaceuticals is employed primarily for the diagnosis ofacute cholecystitis and for demonstration of biliary tract patency. We conducted a retrospective study of 55patients with hepatobiliary disease from Jun. '84 to Sep. '84 at Taegu Catholic Hospital and tried to determinethe etiology and fine the possible differential points by analyzing the scintigraphic findings. The followingresults were obtained: 1. Tree-like photon defect on hepatic parenchyme was suggested characteristic of completeobstructive biliary diseases, but could be seen in either benign or malignant etiology. 2. The grade of hepatocyteclearance was not useful in detemining whether the cause of obstructive biliary disease was benign or malignant inthis study. 3. Hepatocyte clearance was more severely impaired in hepatocellular disease than in obstructivebiliary disease. 4. The photon defect in porta hapatis with complete biliary obstruction was suggestedcharacteristic of common bile duct cancer. 5. The meniscus appearance at obstructed site of common bile duct waspathognomonic sign of choledocholithiasis. 6. When the gallbladder was not visualized, the differential diagnosisbetween acute and chronic cholecystitis was possible without delayed image by observing the transist time tobowel. The delayed transit to bowel was a favorable of chronci cholecystitis rather than of acute cholecystitis.7. Acute pancreatitis could be easily differentiated from partial biliary obstruction by clinical and laboratoryexamination, but the finding of abrupt narrowing of pancreatic common bile duct with sligt proximal dilatation oncholescintigraphy was also a key point in acute pancreatitis. 8. The segmental dilatation of intrahepatic duct wasthought meaningful sign of clonorchiasis.

Deep vein thrombosis and pulmonary embolism are serious and fatal complications in orthopedic surgery. Most cases of symptomatic pulmonary embolism in knee surgery have been reported after total knee arthroplasty, but rarely after patella fracture. We report on a case of symptomatic pulmonary embolism after surgical treatment of a patella fracture in a 42-year-old female patient.

10.A Case of Bladder Foreign Body with Hydroureter and Stones.

Most foreign bodies found in the bladder are inserted through the urethra. Abnormal psyche senility, inebriation, and eroticism are the most frequent causes for such induction. The following is a report of a male who slipped Nylon silk into the urethra during masturbation and lost it in the bladder while attempting to remove it.
