1.A Biomechanical Comparison between Taylor’s Spatial Frame and Ilizarov External Fixator

Taylor’s spatial frame (TSF) and Ilizarov external fixators
(IEF) are two circular external fixator commonly used to
address complex deformity and fractures. There is currently
no data available comparing the biomechanical properties
of these two external fixators. This study looks into the
mechanical characteristics of each system. TSF rings with
6 oblique struts, 4 tube connectors, 4 threaded rods, and
6 threaded rods were compared to a standard IEF rings
with 4 threaded rods. Compression and torsional loading
was performed to the frame as well as construct with
Polyvinylchloride tubes. TSF rings with 4 tube connectors
had the highest stiffness (3288 N/mm) while TSF rings
with 6 struts was the least stiff. The situation was reversed
for torsion where TSF rings with 6 oblique struts had the
highest torsional stiffness (82.01 Nm/Degree) and frame
Ilizarov rings with 4 threaded rods the least. Standard TSF
construct of two ring with 6 oblique struts have better
torsional stiffness and lower axial stiffness compared to
the standard IEF.

7.Treatment of Chronic Cavitary Osteomyelitis with Fragmentary Partial Bone Transport: A Report of New Operative Technique.

PURPOSE: To report the challenging problems in the treatment of patients with chronic cavitary osteomyelitis that is refractory to conventional treatment methods, and to illustrate the author's newly designed fragmentary partial bone transport technique using the modification of the Ilizarov method. MATERIALS AND METHODS: Three patients with chronic cavitary osteomyelitis with a large cavitary bone defect of the tibia were treated with the author's newly designed biological fragmentary partial bone transport technique. RESULTS: After an average of 78 days of fragmentary partial bone transport, the cavitary bone defects were completely filled with the transported bone fragment, and the draining sinus disappeared without any evidence of active infection. The Ilizarov frame was removed on an average of 271.3 days after surgery. The mean total length of the distraction was 8.0 cm and the mean healing index was 33.9 days/cm. CONCLUSION: A fragmentary partial bone transport technique using the Ilizarov apparatus can solve the problem of chronic cavitary osteomyelitis of the tibia by filling the bone defect with new living bone without sacrificing the bony continuity and effectively eradicating the infection. Therefore, this new technique is an effective treatment for chronic cavitary osteomyelitis of the tibia.

8.A Comparison of using Interlocking IM Nail versus Plate Fixation in Humeral shaft fractures

The tibial pilon fracture has been described as difficult fracture to manage. We have reviewed 23 cases of tibial pilon fractures from Mar. 1987 to Feb. 1993 at our hospital. 1. The fractures were classified into five types according to the system of Ovadia and Beals and the methods of treatment were divided into two groups; 9 cases were treated with Ilizarov device(Group I). 6 cases out of Group I were type 3, 4 and 5. Other methods were performed in 14 cases(Group II). 8 cases out of Group II were type 3, 4, and 5. 2. In type 3, 4 and 5 fractures, there were 86 per cent good and fair radiographic results in Group I and 63 per cent good and fair results in Group II. 3. Satisfactory results were obtained by the treatment of Ilizarov method especially in type 3, 4 and 5 fractures. The advantages of Ilizarov device were its primary reduction with ligamentotaxis, easy open reduction due to proximal and distal stabilization, minimal soft tissue injury and minimal internal fixation.

9.From Wolff law, Ilizarov technology to natural reconstruction theory.

Wolff law was an adaptable principle of bone, Tension-Stress Principle was equal to Distraction Osteogenesis or Distraction Tissue Regeneration, The Natural Reconstruction theory was a new orthopedic perspective proposed by Prof. QIN after deformity correction using Ilizarov technology. The thought about their relationship originated from a social phenomena, that the crowds and the confusion about export choice in Beijing's subway. Ilizarov technology and Wolff law were one concept related to Mechanics, and the former is completely in line with the latter. In other words, Ilizarov technology is an extension of Wolff law, is a repeated process of micro-trauma and continuous repair of bone trabecular initiated by moden engineering, just trabecular formed along the tension-stress direction. With adjustment of mechanical force,doctor can control the process of fracture healing and bone remolding to a certain extent. Natural Reconstruction theory enlarged the defined range of Wolff law obviously. Not only guided orthopedics clinical and basic research,but also related to the dialectical thinking of the doctor-patient relationship in sociology. There was an inevitable connection among Wolff law, Ilizarov technology and Natural Reconstruction theory. The history of discovery and understanding was a continuous process of thinking,practice and integration.
