5.Endovascular Management for Chronic Steno-occlusion of Iliac and Femoral Arteries.

The iliac and femoral arterial segments are the most common sites of atherosclerotic steno-occlusive lesions. As a less invasive treatment, endovascular therapy for these arterial segments has been widely accepted as a treatment of choice in selected patients. The long-term patency rate after endovascular treatment using angioplasty and/or stent for the iliac artery lesions was similar to those of surgical bypass. However, the results of femoral artery are still challenging. In this article, the indications, techniques and results of endovascular therapy for iliofemoral steno-occlusive lesions will be reviewed.

6.Endovascular Treatment of Isolated Common Iliac Artery Aneurysms With Short Necks Using Bifurcated Stent-Grafts.

Elective surgical repair has traditionally been considered to be the treatment of choice for the exclusion of isolated iliac artery aneurysms (IAAs). Recently, endovascular repair has evolved as an alternative to surgical repair, especially in patients at high surgical risk. However, in the absence of sufficient proximal necks, iliac artery aneurysms are not suitable for direct deployment of a tubular-shaped endograft. Here we report two cases of IAAs with short proximal necks that were excluded using an endovascular bifurcated stent-graft. The bifurcated stent-graft was successfully deployed with complete exclusion of the aneurysm. In neither case was there evidence of procedural failures. There were no signs of significant complications. We conclude that endovascular repair of IAAs with short proximal necks is feasible and efficient using an endovascular bifurcated stent-graft.

7.Isolated Bilateral Internal Iliac Aneurysms; Surgical or Endovascular Repair.

This report describes the open surgical or endovascular treatment of two patients with bilateral internal iliac artery aneurysms (IIAA). To prevent pelvic ischemia, complete aneurysmal resection and pelvic revascularization with an interposition graft was performed in the open surgical case. Endovascular repair including coil embolization of IIAA and stent-graft placement in the common iliac artery was done successfully in the other case. The contralateral IIAA was left undisturbed and followed for 36 months with no change in size. Prevention of pelvic ischemia is important in the simultaneous treatment of bilateral IIAA.

In the treatment of aortoiliac disease, two methods could be considered. One is percutaneous transluminal angioplasty, and the other is bypass graft. However sometimes two methods could be combined in high risk patients with a lengthy unilateral occlusion of one iliac artery and less extensive obstruction of the contralateral one, since higher risk aortofemoral bypass surgery may be obviated by femoro-femoral bypass graft after percutaneous treatment of the less diseased iliac artery. We report two cases of arortoiliac disease treated with Strecker stent followed by femoro-femoral bypass graft.

Leiomyomas most frequently occur in the genitourinary and gastrointestinal system. This report describes the clinical and imaging findings of a leiomyoma originating in the prevesical space, a rare location. The mass abutted the fundus of the uterus, but after the correct identification of feeding vessels from the right iliac artery, prevesical leiomyoma was diagnosed radiologically.
