The iliac and femoral arterial segments are the most common sites of atherosclerotic steno-occlusive lesions. As a less invasive treatment, endovascular therapy for these arterial segments has been widely accepted as a treatment of choice in selected patients. The long-term patency rate after endovascular treatment using angioplasty and/or stent for the iliac artery lesions was similar to those of surgical bypass. However, the results of femoral artery are still challenging. In this article, the indications, techniques and results of endovascular therapy for iliofemoral steno-occlusive lesions will be reviewed.

Elective surgical repair has traditionally been considered to be the treatment of choice for the exclusion of isolated iliac artery aneurysms (IAAs). Recently, endovascular repair has evolved as an alternative to surgical repair, especially in patients at high surgical risk. However, in the absence of sufficient proximal necks, iliac artery aneurysms are not suitable for direct deployment of a tubular-shaped endograft. Here we report two cases of IAAs with short proximal necks that were excluded using an endovascular bifurcated stent-graft. The bifurcated stent-graft was successfully deployed with complete exclusion of the aneurysm. In neither case was there evidence of procedural failures. There were no signs of significant complications. We conclude that endovascular repair of IAAs with short proximal necks is feasible and efficient using an endovascular bifurcated stent-graft.

This report describes the open surgical or endovascular treatment of two patients with bilateral internal iliac artery aneurysms (IIAA). To prevent pelvic ischemia, complete aneurysmal resection and pelvic revascularization with an interposition graft was performed in the open surgical case. Endovascular repair including coil embolization of IIAA and stent-graft placement in the common iliac artery was done successfully in the other case. The contralateral IIAA was left undisturbed and followed for 36 months with no change in size. Prevention of pelvic ischemia is important in the simultaneous treatment of bilateral IIAA.

Malperfusion syndrome is a complication of acute descending aortic dissection (DAD) and it is associated with a poor clinical outcome. Surgical treatment for it has a high rate of mortality. Thoracic endovascular aortic repair (TEVAR) for DAD with malperfusion syndrome has resulted in good clinical outcomes. However, when both common iliac arteries are compromised by a false lumen, it is impossible to conduct TEVAR because there is no accessible artery. We successfully treated a case of DAD with malperfusion syndrome in which both common iliac arteries were compromised by placing stents in both arteries using the left brachial approach.

The benefits of the use of temporary clipping for intracerebral aneurysm surgery were proved through many experimental and clinical studies. There are two techniques of temporary clipping which are interrupted and continuous clipping. In the study of cerebral perfusion, interrupted clipping reduced ischemic damage to the brain. However, the comparison of histological changes in the arterial wall between them is not reported yet. Temporary clipping on the iliac artery of 80 rats was performed using Yasargil temporary mini clip. The specimens were divided into two groups; Group I (intermittently repeated clipping for 5 minutes was done 3 times on the same site with resting interval for 5 minutes: total clipping time was 15 minutes) and Group II (continuous clipping for 15 minutes). Under the light microscope, the histological findings were examined in the specimens, which were obtained at each time-interval after clipping (0 hr, 3 hrs, 6 hrs, 12 hrs, 3 days, and 3 months). The histological changes of the arterial wall of rat by two techniques for temporary clipping were observed. Although there is no significant difference between two temporary clippings, there is a trend of milder and more delayed arterial change in intermittently-repeated temporary clipping.

Mycotic aneurysm is a disease requiring immediate treatment because of the high risk of rupture. A difficult surgical approach, especially in the case of occurrence on the iliac artery, involving endovascular embolization and extra-anatomic bypass grafting, is known to be a suitable treatment. We performed extra-anatomic bypass grafting after endovascular embolization successfully in two patients. The postoperative computed tomography of both patients showed complete exclusion of the mycotic aneurysm.
