During 4 years (31/12/1997 - 31/12/2001), the Viet Tiep Hospital has operated 105 cases with postoperative adhesive ileus, 56 males and 49 females, the youngest is 13 years old, the oldest is 83 years old, with a mean age of 42.4. Clinical - X ray signs: stomachache 100%, loss fart 100%, vomit 97.1%, 100% X quang of stomach had steam level, brilliant intestine 33.3%. The therapeutic results: mean period of hospitalization was 11.3 days, mortality 0%, the patients having the time before operation of 15 hours - 24 hours are 50.5%, of > 24 hours are 40,9%

Gallstone ileus is a well-known complication of cholelithiasis, but is relatively rare. Most ectopic gallstones are located in the small bowel; they are rarely found in the stomach and duodenum. We describe the imaging findings of a case of intragastric gallstone, as well as a case in which duodenal obstruction was caused by a large gallstone (Bouveret's syndrome).

PURPOSE: The aim of this study was to determine the incidence and duration of postoperative pneumoperitoneum on plain radiographs and to identify the radiologic findings associated with anastomotic leakage. METHODS: A retrospective analysis was conducted on plain radiographs of 384 patients who underwent intra-abdominal anastomoses between March 2005 and December 2008. RESULTS: Of the 384 patients, 93 patients (24.2%) had postoperative pneumoperitoneums. Of the 93 patients, 86 patients (92.5%) had physiologic pneumoperitoneums and 7 patients (7.5%) had pneumoperitoneums associated with anastomotic leakage. The initial air height was significantly greater in the leakage group than the physiologic air group (12.16 +/- 7.65 mm vs. 7.71 +/- 5.08 mm, P = 0.04). The area under the receiver operating characteristic curve of the initial height of free air for anastomotic leakage was 0.69 (95% confidence interval, 0.59 to 0.78). The best cut-off point was 11.7 mm. The height of the pneumoperitoneum increased with time in the leakage group. Ileus was significantly more prevalent in the leakage group than the physiologic air group (P < 0.01). CONCLUSION: Postoperative pneumoperitoneum is a common phenomenon after abdominal surgery. An initial air height >11.7 mm, increasing air height over time, and the presence of ileus on plain radiographs suggest a high likelihood of anastomotic leakage.

PURPOSE: Gum chewing activates chephalic-vagal reflex as in food consumption and increases the release of gastrointestinal hormones which are related with gut motility. The objective of this study was to assess whether it is effective in shortening the time of hospitalization and postoperative ileus. METHODS: Twenty patients who received open abdominal surgery for colon cancer in Gachon University, Gil Hospital were collected. They were further categorized to gum-chewing group (n=10, mean age=52.0 years, range 37 to 70) and control group (n=10, mean age=59.7 years, range 35 to 75) randomly. The patients in the gum-chewing group chewed gum three times a day from the first postoperative AM until the day they began oral intake. The time of gas out was recorded in each group. RESULTS: The mean time of gas out were 2.35 days (SD 1.2) in gum-chewing group and 2.87 days (SD 1.2) in control group (P=0.41). The mean postoperative hospital days were 10.5 days in gum-chewing group and 13.0 days in control group (P=0.23). CONCLUSION: There were no statistically significant results for shortnening of postoperative ileus and hospital day in this study.

The occurrence of an adenocarcinoma arising from a rectal diverticulum that causes mechanical ileus is very rare. Recently, we diagnosed a case of a mucinous adenocarcinoma in a rectal diverticulum after an emergent abdominal perineal resection and permanent colostomy by laparotomy. Here, we present a case report and a review of the literature.
