1.Clinical Investigation of Nail Changes in Psoriatic Patients.

This study was aimed to investigate the clinical manifestation of nail changes in psoriasis and was based on analysie of 205 p oriatics visited the department of dermatology, Seoul National University Hospital from November, 1984 to June, 1985. The results were summerized as follows: Nail changes were noted in 78(38. 0%) out of 205 patients in the finger nails. Toe nails were involved in 26(12 7%) cut of 205 patients. 2 Nail changes in male were noted in 49 (46, 7%) out of 105 patients and in female, 29(29%) out of 100 patients. 3. The peak incidence of nail changes according, to age group was over 40 years (45. 1%.) and the lowest incidence, under 9 years(26. 7%). 4, Pitting(57. 7%) was the commonest change ii the finger nails and subungal keratosis(50. 0%) and discoloration(46 2%) in the toe nails. 5 Incidence of nail changes according to psoriatic type was 23. 7% in guttate type, 42. 6% in nummular type, 53 3% in annular type. 6 The patients with the more severe degrees of psoriasis had a higher incidence of nail involvement. Mean duration of psoriatics with nail change and without nail change was 6. 3 years and 6. 5 years, respectively, 8 Among 65 patients with psoriatic nails, onychomycosis was noted in 9 patients(13.8%).

3.A Comparative Study of Anthralin Therapy in Psoriasis.

Anthralin is highly efficacious in the treatment of psoriasia, but it has two major drawbacks: staining of the skin and clothing and irritation of the skin. There have been several studies that the effect of the anthralin with short contact therapy(SCT) is nearly the same as that with the simple application(SA). This study was planned to compare the effect and side effects of SCT with those of SA of anthralin in 27 psoriatic patients visited between November 1982 and June 1983 at the Department of Dermatology, Seoul National University Hospital. The results could be summerized as follows: 1. For 4 weeks of treatment, the longer the duration of treatment, the more similar the effect between the SCT and the SA. 2. After 2 weeks of treatment, the effect of the SA was better than that of the SCT in 5 cases, the same in 18 cases and worse in 1 case. 3. For 4 weeks of treatment, all side effects(irritation, ataining and pruritus) were more frequent in the SA than in the SCT. And irritation was observed earlier than staining. 4. After 2 weeks of treatment, the side effects of the SA were more than those of the SCT in 12 cases and the same in 12 cases. With all our results, we have concluded that the patients who cannot tolerate the SA could be tried with the SCT.

4.A Study for Skin Protection Effect of Sunscreens: I . Comparison of Effectiveness by the Concentration.

The purpose of this study is to evaluate the effectiveness of these para-amino- benzoate, benzophenone and cinnamate sunscreens in the same base(cream) and the effect of concentration on sun protection with three different concentrations(2 5%, 5. 0% and 7. 5%.). Twenty-one normal healthy Korean-men, majority(2. 5%) of whom have skin type iv or v, were applied to their untanned back with above prepared nine different sunscreens and calculated individual sun protection factor (SPF) of these different sunscreens. 5.Generalized Actinic Granuloma annulare with Impaired Glucose Tolerance Test.

We speculate that this is case of an intermediate state between granuloma annulare and actinic granuloma. This intermediate state would be a variant of granuloma annulare related to actinic damage. Therefore this case can be termed actinic granuloma annulare with generalized distribution.

7.Emollient-Phototherapy for Psoriasis.

The present study was undertaken to evaluate the efficacy of white petrolatum-UVB therapy on patients with psoriasis. Nineteen patients with chronic plaque type psoriasis selected in the Department of Dermatology, Seoul National University Hospital were treated with white petrolatum followed by exposure to UVB. Clinical response to therapy was graded as follows: Grade 1: Minimal improvement, Grade 2: Definite improvement Grade 3 Considerable improvement, Grade 4: Clearing. We campared the clinical response according to involved site: trunk and extremity. A responsive case was defined as a case with G4 in trunk and/or extremities. The results can be summarized as follows: 1. Fourteen of the 19 patients(73. 7%) were classified as being responsive cases. 2. Among the responsive cases, the 11 cases of trunk lesion all showed G4 clea- ring. The extremities lesion cases showed G4 clearing in 9 out of 14 cases, the other 5 cases showing GR. This showed that the trunk lesions were more responsive to the treatment than the extremities lesions. 3. In the cases of trunk lesions, the mean number of therapy was 6. 3 for G2 response, 11. 2 for GR and 16. 7 for GR. In the cases of extremities lesions, the mean number of the therapy was 8. 6 for G2, 16. 1 for GR and 19. 2 for Gl. 4. Pruritus was a complaint in 13 patients (68. 4%) during therapy. Among them moderate to severe pruritus was noticed in 6 cases (51. 6%). From the above results the emollient phototherapy seems to be an effective therapy in the treatment of psoriasis.

8.Binding of B Cell - Derived Autocrine Growth Factor to Hemoglobin.

Normal human B cells produce autocrine growth factor in response to Staphylococcus aureus Cowan I strain (SAC). However, the functional role and molecular nature of the B cell derived-B cell growth factor (B-BCGF) are largely unknown. We have tried to investigate the nature of B-BCGF using mAb for several years. We report here that B- BCGF is capable of binding to hemoglobin (Hb). The concentrated culture supernatant from tonsillar B cells stimulated with SAC for 24 h was loaded into the fast protein liquid chromatography and ion-exchange chromatography. The peak with BCGF activity was shown to have a M.W. of 16-18 Kda in polyacrylamide gel electrophoresis followed by silver stain. Amino acid sequence of the fraction was found to identical to human hemoglobin (Hb) by more than 85%. However, Hb itself had no BCGF activity. The presence of Hb in culture supernatant was due to the contamination of SRBC during B cell purification. SRSC were completely removed from B cells by percoll-gradient centrifugation and B cells were stimulated with SAC and exogenous Hb was added to the cultures. The Hb fraction from FPLC again showed a BCGF activity. These data strongly suggested that BCGF binds to Hb. We confirmed this in dot blot as well as Western blot. The M.W of Hb-binding BCGF was 20 Kda. This information may provide a rapid progress in research of BCGF.

9.A Case of Atropie Blanche Accompanied by Cryoglobulinemia.

We report a case of atrophie blanche which was accompanied by cryoglobulinemia. The patient, 17-year-old female, have had recurrent painful ulcerations and ivory-white atrophic scars with telangiectases surrounded by hyperpigmentation. On histological examination, vessel wall thickening and hyalinization of the intima, partial vascular occlusion, vascular proliferation and mild perivascular chronic inflammatory cells infiltration and hemorrhage in the upper dermis were noted. The patient had been treated with dipyridarnol(Persantin) and acetylsalicylic acid hut the lesions had been waxed and waned.
