2.Clinical Observation of Decrease of Prostatic Volume in Men with Voiding Symptoms by Means of TRUS .

PURPOSE: It has been generally known that prostatic volume is increased related to aging process and changing in hormonal environment, so lower urinary tract symptoms develops. But, our experience has revealed the decrease of prostatic volume in many cases with lower urinary tract symptoms despite aging. MATERIALS AND METHODS: The numbers of patient underwent TRUS were 301 cases. Among the patients who had visited urologic outpatient clinic from march, 1993 to march, 1996, transrectal ultrasonography of prostate was applied to patient with following conditions. 1) over 8 in IPSS with or without enlarged evidence from digital rectal examination 2) enlarged evidence from digital rectal examination 3) no symptomatic relief after 2weeks therapy with alpha blocker 4) when the patient wants more evaluation. RESULTS: Decrease of prostatic volume were shown in 66cases(22%) despite lower urinary tract symptoms. CONCLUSIONS: In our experience, many patients with lower urinary tract symptoms showed decrease of prostatic volume. So, we expect that many more cases of decrease of prostatic volume can be seen among asymptomatic cases with age. Because we can guess that prostatic volume can be influenced by racial differences and dietary habit, we also believe that further evaluation and investigation of this nature is needed.

3.Inflammatory Gastric Adenocarcinoma with Neutrophil Leukcytosis.

A 36-year-old man received a subtotal gastrectomy for Borrmann type 2 advanced carcinoma in the antrum of the posterior well. Preoperatively, peripheral white blood cell count was 16,400/mn3 with 80% of neutrophils. Microscopically, the tumor was of a poorly differentiated adeocarcinoma with intense eutrophilic infiltration which was totally confined within the neoplastic portion and in the metastatic focus in the regional lympho nodes. Repeated postoperative whits blood cell counts dropped down to the normal level. Neither infectious nor knowledge, this is the first case of gastric carcinoma with massive neutrophilic stromal reaction(inflammatory carcinoma) and leukocytosis in the literature in the korea.

4.Conservative Management of the Renal and Perirenal Abscesses.

PURPOSE: We critically evaluated the most appropriate management of renal and perirenal abscess and identified the set of patients who had benefited from conservative management. MATERIALS AND METHODS: Between May 1995 and June 1999, fifteen patients with renal (10 cases) and perirenal abscess (5 cases) were treated in our hospital. Among the fifteen patients, six patients were managed by antibiotics alone, the other nine patients were managed by percutaneous aspiration and antibiotics. Percutaneous abscess aspiration and drainage was done under ultrasound guidance and local anesthesia. RESULTS: On the urine culture, 4 cases had positive urine culture (27%); E. coli was in 3 cases, Klebsiella in 1 case respectively. On the blood culture, E. coli was in 2 cases. On the abscess culture (9 cases were performed), the organisms were identified in 8 cases (89%); E. coli was in 6 cases, S. aureus was in 1 case, S. epidermidis in 1 case respectively. CONCLUSIONS: We suggest that proper antibiotic therapy alone or combined with ultrasound guided percutaneous drainage of renal and perirenal abscesses is a choice of reasonable, safe and effective management in selected patients.

6.Elastofibroma.

Elastofibroma is a rare benign tumorous growth presenting as a slowly growing ill-defined mass of fibroblastic tissue occurring in elderly persons and arising mainly form the connective tissue between the lower portion of the scapula and the chest wall. Its pathogenesis is not well established but it may be the result of nonneoplastic reactive hyperplasia taking place with constitutional predisposition in the background. A case of elastofibroma occurring in the subscapular area of a 65-year-old female cook is presented. The mass, 6x5x3 cm in maximum dimensions, was poorly circumscribed, solid, hard, pale fleshy and pray-white fibrous tumor. Microscopically, it was composed of numerous small globular and linear elastic fibers embedded in collagenous matrix. To our knowledge, it is the first case of elastofibroma in Korea.

7.A Study on the Limb Lengths Following Femoral Shaft Fracture in Children

Growth acceleration following femoral shaft fracture occasionally results in a significant leg-length inequality with limp. The ability to predict subsequent overgrowth would enable the surgeon to compensate for growth acceleration by providing the appropriate overriding of the fragments before the time union. The purpose of this study was to establish principles which would aid in predicting over growth. Between 1972 May and 1983 September, 115 inward patients who were 16 years old or the younger were treated in the Department of Orthopaedic Surgery of Hanyang University Hospital. In these cases, we analysed the causes of fractures, associated injury and methods of treatment. And also, by use of mentgenogram, evaluated on site, shape and degree of overriding of fractures. And then, the degree of overgrowth of bone was compared with unaffected site and analized by Bell Tompson's split orthoroentgenogram. 1. The average tibial overgrowth after fracture of femoral shaft is 2.2 mm in length. 2. The average femoral overgrowth after fracture of femoral shaft is 9.4 mm in length.

8.Thymoma: A clinicopathologic analysis of 66 cases.

A total of 66 cases of thymoma(57 surgically resected cases and 9 incisional biopsy cases) were reviewed with an attempt to correlate pathomorphologic features and clinical presentations. Criteria of benign or invasive thymoma were primarily determined by operative clinical and pathologic findings. Of them, 21 cases were invasive thymomas. The mean age of patients at the time of surgery was 47 years and it occurred largely in the sixth decade. Myasthenia gravis was accompanied in 29 cases(43%). One patient died during folow-up period, and five of the remainder suffered from recurrence. Microscopically, mixed type was the most common one(33 cases), being followed by predominantly epithelial type(17 cases) and predominantly lymphocytic type(16 cases). Thirty four cases of thymomas were cortical type, 29 mixed type, and the remaining 3 medullary type. None of the histologic type were significantly correlated with tumor invasiveness, Myasthenia gravis was more frequently associated with mixed and cortical type, respectively.

9.A Case of Subcutaneous Dermoid Cyst.

Authors observed one case of subcutaneous dermoid cyst which did not seem to have been described in the Korean literature. A 11 year old female patient complained of a asymptomatic mass near the left sternoclavicular joint in January 1978. The mass was discovered at the age of 5 when that was pea-sized and had been slowly growing. In January 1978, the mass was palpable as a peanut-sized subcutaneous solitary round mass, and overlying skin was freely movable and slightly elevated. There was no trauma history and family history was negative except her elder sister had a, axillary accessory breast. Histopathologic studies revealed a keratinizing stratified squamous epithelium lining the cystic wall, and sebaceous glands, hair follicle and eccrine glands in the surrounding tissues, and keratinous materials in the cyst. There was no evidence of malignancy. Diagnosis was confirmed by clinical features and histopathological findings, and surgical excision of tumor mass was performed.

10.A Case of Atypical Fibroxanthoma.

We described a 65-year-old woman with a papule on the left temple area of } year duration. The lesion showed typical findings of atypical fibroxanthoma and was located on the lesion of solar keratosis, So sunlight may be the most important one among the various insults to atypical fibroxanthoma of the skin.
