2.Carpal Tunnel Syndrome among Packing Workers in A Rayon Manufacturing Factory.

The carpal tunnel syndromes is one of the most common peripheral nerve entrapment syndromes. The typical symptoms are pain, numbness and paresthesia in the median nerve territory of the hand. Recently, it is widely recognized that occupational factor is regarded as the important cause of the carpal tunnel syndrome. Clinical study is performed in the 42 female workers who is repetitively working at packing department in a rayon manufacturing factory from November 1991 till March 1992. The study included a questionnaire, physical examinations, and the neurophysiological test. The summary of the results obtained was as follows: 1. Among 42 packing workers, 9 workers(21.4%) were diagnosed as carpal tunnel syndrome by electromyography. The affected side was bilateral in 4 workers(9.5%), right in 4 workers(9.5%), and left in one worker(2.4%). 2. Among 42 subjects, 28 workers(66.7%) complained the clinical symptoms related to carpal tunnel syndrome, 11 workers(26.2%) showed positive Phalen sign, and 7 workers(16.7%) showed positive Tinel sign. 3. Researchers regard electromyographic finding as the gold standard for diagnosis of carpal tunnel syndrome. The sensitivity and specificity of the clinical symptoms to diagnose the carpal tunnel syndrome were 0.89, 0.39 respectively. If the carpal tunnel syndrome is diagnosed by the combination of the positive findings of the symptoms and the physical examinations, either Tinel of Phalen sign, the sensitivity and specificity were 0.67 and 0.76 respectively. Considering above results, though this small number of worker is not adequate for epidemiologic conclusions, carpal tunnel syndrome seems to be an important occupational disorder among packing workers in a rayon manufacturing factory.

4.Effects of Dexamethasone on Endothelin-1(ET-1) Production by Keratinocytes.

Epidermal keratinocytes are important sources of a wide variety of cytokines that include the endothelin-1 (ET-1). Glucocorticoids have been shown to inhibit the production of several cytokines. However, their effect on ET-1 synthesis by keratinocytes is still unknown. It has been reported that ultraviolet B (UVB) irradiation stimulates both the synthesis and release of ET-1 and it was observed that ET-1 secretion by HaCat cells increased with increasing UVB exposure. In this study, the effects of glucocorticoid on ET-1 production were evaluated using cultured HaCat keratinocytes. The results showed that dexamethasone suppressed basal re-lease of ET-1. In addition, it strongly inhibited the UVB-mediated augmentation of ET-1 production. Furthermore, lincomycin slightly enhanced the inhibitory effect of dexamethasone on ET-1 synthesis.

PURPOSE: To evaluate the radiologic characteristics of axillary lymph nodes which are found on mediolateral oblique(MLO) view of mammography and to assess their clinical significance. MATERIALS AND METHODS: We retrospectively evaluated 119 cases axillary lymph nodes on MLO view of 410 cases of mammography in 205 patients regarding the size, bilaterality, number, grade of density, presence of central lucency, calcification of lymph nodes and presence of associated parenchymal pathologic lesions in breast. RESULTS: Axillary lymph nodes on MLO view were demonstrated in 119 (29%) among 410 cases. Axillary lymph nodes(112 cases) in cases without breast parenchymal lesions or with benign breast lesions showed; unilaterality in 68 cases(61%), smaller than 1.5cm in 100 cases(89%), lower density than breast parenchyma in 99 cases(88%), presence of central lucency in 99 cases(88%) and calcification in 3 cases(2.7%). Axillary lymph nodes(7 cases)in cases with breast cancer showed small size of less than 1.5cm in 4 cases(57%), absence of central lucency in 7 cases(100%) and iso-to higher density than breast parenchyma in 7 c&ses(100%). CONCLUSION: Axillary lymph nodes noted on MLO view of mammography, more than 1.5cm in diameter, absence of central lucency and iso to higher density than breast parenchyma could be suggested as clinically significant.

7.The Results of Eight Years' Clinical Experience with Intravesical Mitomycin C Plus Hyperthermia for the Treatment of Superficial Bladder Tumors.

PURPOSE: A non-randomized study was performed to assess the efficacy of Mitomycin C (MMC) plus hyperthermia as a primary therapy after an initial transurethral resection (TUR) for treating superficial bladder tumors or as a salvage modality for recurrent T1 tumors after adequate BCG therapy. MATERIALS AND METHODS: A total of 31 patients from Nov. 1992 to Aug. 2001 with follow-up of 11 to 94 months (mean 27.4) were enrolled in this study. Of the 31 patients, 27 underwent MMC plus hyperthermia as the primary agent after TURB and among those 27 patients 7 had a tumor recurrence and were allocated to receive BCG. Of the 31 patients, 4 were given BCG initially, and at the time of first or second recurrence, MMC plus hyperthermia was attempted. RESULTS: Of the 27 patients who received MMC plus hyperthermia, 20 (74%) responded to the initial therapy without a local recurrence and the mean follow-up was 26.4 months with a range of 12 to 94 months. Seven patients had a tumor recurrence and the median interval to recurrence was 21.6 months. Of the 7 patients who had a tumor recurrence, the histology (stage or grade) had changed at the time of the recurrence in 4 (57%). Four patients who received BCG as an initial therapy had a tumor recurrence and the mean interval to recurrence was 14.5 months. These patients were treated with MMC plus hyperthermia and 3 those patients remain free of recurrence with a follow-up of 11 to 71 months. One of those patients underwent a cystectomy and urinary diversion. CONCLUSIONS: MMC plus hyperthermia is a viable option for appropriately selected patients who developed recurrent superficial bladder tumors after adequate BCG therapy.

9.Electrophysiological Studies in a Case of Friedreich's Ataxia.

Observing a 16 years old male of classical Friedreich's ataxia with progressive ataxia, dysarthria, areflexia, loss of vibration and proprioception, Babinski sign, pes cavus, scoliosis, hyperglycemia, optic atrophy and hearing deficits, the nerve conduction studies and several evoked potential studies are performed. The sensory nerve potentials are markedly decreased in amplitude and the average technique has to be used to get the potentials, worse distally. The motor nerve functions, however, are normal. There is no consistent waves recorded on BAEP even with maximal stimulation bilaterally. VEP reveals prolonged latencies bilaterally but with normal amplitudes. On median nerve SEP stimulated at wrist, the amplitudes of potentials at Erb's point look decreased and no definite evoked potentials are recored at contralateral scalp. In contrast, the cervical evoked potentials are normal in amplitudes and latencies.
