2.Composite Carcinoma-Carcinoid Tumor of Stomach: Report of a case.

The concurrence of typical carcinoid admixed with adenocarcinoma of the gastrointestinal tract is a well known but unusual phenomenom that has been reported in the esophagus, stomach, gall bladder, colon and appendix. The amine precursor uptake and decarboxylation (APUD) concept included the idea that neuroendocrine cells of the gastrointestinal tract were dervied from the neural crest by a process of migration during embryogenesis. However the intimate admixture of cells which have morphologic and immunocytochemical properties of both carcinoid tumors and adenocarcinoma suggests that these tumors are derived from a multipotential stem cell showing bidirectional differentiation rather than from unrelated cell lines. The term composite carcinoma-carcinoid tumor is therefore an appropriate designation for these neoplasms. We report a case of composite carcinoma-carcinoid tumor of the stomach in a 38 year-old female with brief review of literature.

3.Harrington Distraction Rods without Fusion for Thoraco-Lumbar Fracture or Fracture-Dislocation

Management of patients with unstable fracture or fracture-dislocation of thoraco-lumbar spine continues to be controversial. One area of disagreement involves spinal fusion with internal fixation for prevention of later spinal instability. Intemal fixation of the spine without fusion has been advocated in the treatment of the unstable thoraco-lumbar fracture or dislocation. During the period of January 1981 to March 1984, thirty-two patients underwent fixation by Harrington distraction rod without fusion for the treatment of thoraco-lumbar fractures. Eight cases among them had second operation for the removal of the Harrington rods in 13.4months after Harrington rod fixation and were followed up clinically and radiolagically for 1 year at the Department of Orthopaedic Surgery, St. Mary's Hospital, Catholic Medical College and Center. The rationales for this procedure were to minimize the number of permanently stabilized segments and to provide more spinal mobility. We have expected that spontaneous fusion of vertebral body occur by the prolonged immobilization with Hamngton distraction rods. After the Harrington rods, the average conection rate of kyphotic angle was 41.8% and average correction rate of height of collapsed vertebral body or displacement of vertebral body was 42.3% at one year follow up. Instrumentations in eight patient were removed at 13.4 months after the initial surgery and the patients were followed up for one year. At the time of removal of instrumentation, unilateral partial facetectomy was done for histologic examination of the facet joint which is above the lower Harrington hook in four patients. The average correction rate of kyphotic angle was 37.5% and the average correction rate of height of collapsed or displaced vertebral body was 40.6% at one year after the removal of H-rods. The range of spine motions were incresed gradually. But histologic findings of the immobilized facet joint revealed fibrillation, fissures, thinning of the normal cartilagenous surface, decreased cartilagenous cellularity, and vascular tidemark invasion which are characteristics of osteoarthritis. Through this study, we obtained following conclusions: 1. Immediate Harrington distraction rodding was effective for the anatomic reduction of the collapsed vertebral body. And it provided immediate spinal stability, and spontaneous fusion of vertebral body. 2. Prolonged immobilization of the spine by Harrington rods is believed to have deleterious effect on articular cartilage of the immobilized facet joint as the degenerative changes of the cartilage of the facet joint was consistently found in this study. And it was regarded as a predisposing factors in the development of symptomatic arthritis of the spine. 3. If a fractured and collapsed vertebral body can be reduced by Harrington distraction rods, a spontaneous fusion of vertebral body occur. Harrington rodding without fusion could be one of the useful surgeries for management of the fractures and fracture-dislocation of the spine.

4.Blocking Action of Nicardipine on alpha1-Adrenoceptors in Circulatory System of Rabbit.

To investigate relationship between alpha1-acrenoceptors and nicardipine, an 1,4-dihydropyridine calcium antagonist, effects of nicardipine, on phenylephrinne(PE)-induced vasoconstriction in isolated arterial rings and pressor response of rabbits were observed. In normal physiological salt solution(NPSS), 35mM KCI produced persistent contractions of thoracic aorta and carotid artery and the contractions were dose-dependently inhoboted by cumulative admini-stration of nicardipine in the range of doses from 10(-10)M to 10(-4)M, IC50s of nicardipine in the thoracic aorta and carotid artery were 3.3x10(-7)M and 4.6x10(-7)M, respectively and there was no difference between both values. Constant contractions induced by 10(-5)M PE in both rings were inhibited by the same doses of nicardipine in a dose-dependent fashion. The IC50s were 2.8x10(-4)M in thoracic aorta and 2.9x10(-5)M in carotid artery respectively, and the former was about 10 times greater than the latter. In Ca2+ free PSS, KCI did not produce any contractionn. Though constant and reproducible in NPSS, PE-induced contraction was transient and not reproducible in Ca2+ free PSS. The contraction in both rings were weakened to about 70% of those in NPSS. Pretreatment with nicardipine in the range of doses from 10-8M to 10-5M hardly affected the PE-induced contraction and the largest dose 10(-4)M slightly inhibited the contraction. Intravenous injection of nicardipine 10 to 1000microg/kg decreased blood pressure and heart rate of rabbits in a dose-dependent manner. The % decrease of heart rate was much smaller in comparison with the %decrease of blood presseure. Pressor effect of 30microg/kg PE was dose-dependently inhibited after treatment with nicardipine 10 to 1000microg/kg and ID50 was 314microg/kg(6.1x10(-7)mole/kg). Above results suggest that nicardipine blocks extracellulr Ca2+ influx by membrane depolarization and in a part by alpha1-adrenceptors, then relaxes arterial smooth muscles in rabbits.

5.Anthralin - UVB Phototherapy for Psoriasis.

Our study was performed to evaluate the efficacy of anthralin-UVB therapy on patients with psoriasis. We studied twenty two patients suffering from plaque-type psoriasis of varying degrees of severity. The results can be summarized as follows. l. 2p patients(90.9%) out of the total 22 patients showed grade 4 improvement. The rernaining 2 patients failed to respond to treatment, 2. Arnong the improved 2Q patients, 2 patients who were treated more than 3p times for grade 4 improvement were excluded, so cleared patients were 18(81.8%). 3. Arnong the 18 cleared patients, the mean nurnber of therapy wasg. 6 for grade 3 and 12. 5 for grade 4 in the cases of trunk lesiona, and 13. 2 for grade 3 and 17. 8 for grade 4 in the cases of extrernities lesions. With above results, anth-ralin-UVB therapy was rnore effective in lesions of the trunk than in those of extremi.ties. 4. Notable side effects were pruritus in 6 patients(27.3pg), staining in 4 patients (18,2%), and 1ITltatioB in 2 patients(9.1%)

6.Comparative Study of Low - Strength Anthralin Therapy in Psoriasis.

We performed anthralin comparative study(0.01% vs 0.1%) to assess the effectiveness of low-strengh anthralin therapy in 34 psoriatic patients and the resu1ts can be summarized as follows. 1. In the case of 13 patients using Burdick UVB larnp, in 1 patient the effect of 0.1% anthralin was superior to that of 0.01% anthralin from the start to the end of treatment, in 5 patients(38.5%) initially the effect of 0.1% anthralin was superior but in time became equal to that of 0.01% anthralin, and in 7 patients (53.8%) the effect of either side was same throughout the courae of the treatment. In the case of 21 patients using Waldmann UVB cabinet, there were 2(9.5 %), 3(14.3%) and 16 patients(76.2%) in the order named above. 2. The side effect of 0.1% anthralin was more severe than or at least the same as that of 0.01% anthralin in every patient who complained about side effects. So low-strengh anthalin-UVB phototherapy was thought to be the effective and alternative method of treatment particularly for the purpose of lessening the side effects.

7.Simple Cyst Occurred in an Accessory Ovary.

The accessory ovary can be defined as an extraovarian tissue that is located near the normal ovaries and is connected to the broad ligament, infundibulopelvic ligament or utero-ovarian ligament. It has very rarely been reported. The majority was found during abdominopelvic surgeries for any other purposes, because they were usually small and less than 1 cm in diameter adn gave no particular symptoms related simply to their presence. We reported a case of accessory ovary in which developed a simple cyst of a largest diameter of 12 cm, and discussed the significance of the accessory ovary in clinical and pathological aspects.

9.Treatment of Psoriasis by Modified Ingram Regimen.

Our study was performed to evaluate the efficacy of anthralin-UVB therapy for psoriasis by simplifying the conventional Ingram regimen, 34 patients(21 admitted, and 13 OPD patients) were included in our study and the resulti can be summarized as follows. 1. There were no cases of treatment failure. 16 patients(76. 2%) out of the 2l admitted patients and 7 patients(53.8%) out of the 13 OPD patients showed clearing. 2. In the case of the admitted patients, the mean numbers of therapy(days) of the improved patients reaching grade 4 were 12. 0(18. 1 days) in the trunk lesions and 15. 5 (22. 0 days) in the extremity lesions' and those of the cleared patients reaching grade 4 were 11. 2(16. 0 days) in the trunk lesions, and 13. 4(lg. 5 days) in the extremity lesions, but the difference was not significant statistically(pp o.ps). The difference between the therapeutic effects of the admitted and the OPD patients was significant statistically(p<0. 05). 3. There were some notable side effects such as staining, pruritus, and erythema, but there were no cases of therapy termination.

10.A Study on UVI - induced DNA Synthesis in Mouse Skin in Vivo Studied by Autoradiography.

The status of DNA synthesis and the effect of UV radiation on the DNA synthesis were studied in mouse skin by microautoradiography. Mice exposed to 100mJ/cm of UVB were injected intradermally with tritiated thymidine, 5 minutes, 2, 6 and 24 hours after irradiation and biopsies were processed for light microscopic autoradiography, A total of 25 ICR female albino haired mice were used as subjects. We compared heavily labeled cells(>10 grains/nucleus) and sparsely labeled cells (3-10 grains/nucleus) in UVL irradiated skin with nonirradiated control skin. 1. Within 5 minutes after UVL exposure an apparent depression in the number of heavily labeled cells occurred. This reduction was statistically significant and remained so for at least six hours post,irradiation. By 24 hours after UVL exposure, the reduction was recovered to nearly control level. 2. By five minutes after UVL exposure, sparsely labeled cells were observed in basal cell layer and differentiated cell layer. The nurriber of sparsely labeled cells appeared highest at five minutes after UVL exposure and then tended to fall to nearly control level at 24 hours. This aberrant type of TdR H incorporation is thought to represent unscheduled DNA synthesis to repair pyrimidine dimers formed in UVL injured DNA molecules,
