1.Study on the (14)C-glucose metabolism by Clonorchis sinensis.

Radioactive (14)C-glucose(U) was given to Clonorchis sinensis in Tris buffer medium, in corder to trace the metabolic fate of the labelled carbon. The labelled carbon from glucose enters into intermediary metabolites and end products of anaeroblic glycolysis, Embden-Meyerhof pathway, and of aerobic Krebs cycle. These product were identified by one or two-dimensional paper chromatography in combination with autoradoigraphy. The labelled metabolites detected in this experiment corresponded to pyruvic acid, latic acid, malic acid, succinic acid and fumaric acid. Amino acids, such as alanine, aspartic acid, glutamic acid, valine, theronine, and serine, derived by the degradation of (14)C- glycose were also found. Labelled compounds behaving like alanine, aspartic acid and glutamic acid were observed in the chroma to gram of incubation medium. The preciptation which suggests a positive reaction for protein occured when absolute ethanol was added to the incubation medium.

2.Effect of Lidocaine and Ketamine Pretreatment on Vascular Pain Associated with Intravenous Propofol Injection.

BACKGROUND: Propofol has a high incidence of pain with intravenous injection, and different methods have been used to minimize the incidence and severity of this pain. In this study, we have compared the effect of lidocaine pretreatment with that of ketamine pretreatment on propofol injection pain. METHODS: Ninety healthy female patients scheduled for general anesthesia were randomly divided into three groups; saline group (n=30), lidocaine group (n=30) and ketamine group (n=30). Each patient received 2 ml of pretreatment solution (normal saline, 1% lidocaine, 0.5% ketamine) via 18G angiocatheter inserted in the antecubital fossa after applying an arm tourniquet inflated to 50 mmHg. The tourniquet was released 1 minute later, followed by intravenous injection of 2.5 mg/kg of propofol. The assessment of pain was made at the induction of anesthesia and in the recovery room, and the severity of pain was classified as none, mild, moderate, severe by one observer. RESULTS: The severity and incidence of pain diminished significantly in the lidocaine group and the ketamine group compared with the saline group at the induction of anesthesia (p<0.05) and there was no significant difference between the lidocaine group and the ketamine group. We had similar results in the recovery room and one patient from the saline group and the ketamine group had no recall regarding injection pain. CONCLUSION: Intravenous ketamine pretreatment is as effective as intravenous lidocaine pretreatment in alleviating the propofol injection pain.

3.The epidemiological studies on the filariasis in Korea II. Distribution and prevalence of malayian filariasis in southern Korea.

During 1964 to 1967, a survey of 30,534 persons for filariasis was made on all over the country with the following results: Among 24,816 draftees from all over the country in the army recruitment camp 155(0.63 %) were found infected with Brugia malayi. Cheju Do showed the highest microfilaria rate(3.5 %), North Kyongsang Do(1.4 %) the next and South Cholla Do(1.2 %) the third. Blood films from 2,308 inhabitants were examined and 407(17.6 %) showed microfilaria in Cheju Do. 30(3.1 %) out of 974 inhabitants in North Kyongsang Do were found to be infected. The microfilaria rates were 2.0 per cent for 400 inhabitants of Chindo island in South Cholla Do. However, no positive case of microfilaria was found in the inhabitants of Kokumdo among 1,820 persons examined in South Cholla Do and of Namhae island(among 165 persons examined) in South Kyongsang Do. The mean microfilarial density per 20 cu. mm of blood was 52.6 in the inhabitants of Cheju Do, 12.2 in North Kyongsang Do and 27.3 in Chindo island(South Cholla Do). Any distinct relationship between the incidence of positive cases of microfilariae and age or sex of the cases was not observed in Cheju Do. However, in Norh Kyongsang Do and South Cholla Do the microfilaria rate of the male group is higher than the one of the female group of the inhabitants. From the above survey results it turned out that filariasis was found throughout Southern Korea except Kyonggi Do and South Kyongsang Do. A total 30,534 persons examined, 601(2.0 %) were found to be infected by Brugia malayi. Therefore, it seems that there are some endemic foci of malayian filariasis in three main areas such as North Kyongsang Do, South Cholla Do and Cheju Do.

4.A Biomechanical Study of Graft Fixation in Posterior Cruciate Ligament Reconstruction.

Injuries of the posterior cruciate ligament have almost universally been treated nonoperatively in the past due to indignity of this complex structure. Recent studies have shown new informations on the anantomical, histological, biomechanical characteristics of the Posterior cruciate ligament PCL), and thereby interest of reconstruction surgery is increasing. But intraarticular PCL reconstruction still remains one of the most challenging problems in knee surgery. Although, various types of graft and surgical techniques have been used for reconstruction, results of surgery has been variable. In this study, three different methods of graft fixation for PCL reconstruction are evaluated biomechanically to determine the effect on mechanical property of reconstructed PCL using 30 porcine knees. Grafts used in this study were bone-patellar tendon-bone fixed with interference screw and Achilles tendon tied with suture. Additionally, five original PCLs were also tested by Instron universal testing instrument to determine its mechanical proprety. Tensile test was performed at extended knee position. It was demonstrated that mechanical strength of bone-patellar tendon-bone graft is superior to that of Achilles tendon graft. All of the failures of bone-patellar tendon-bone graft and Achilles tendon graft occurred at tibial fixation site whereas original PCLs which failed at proximal midsubstance of ligament. In conclusion, tensile strength of Bone-patellar tendon-bone graft used for PCL reconstruction is higher than that of Achilles tendon but latter can be used in terms of as clinical meanings.

5.A Clinical Study of Lumbar Disc Sequestration

Sequestration of disc herniation is important cause of failure of chemonucleolysis. It is important to recongnize this particular variety of disc herniation before considering enzymatic discetomy. Disc herniation were classified into three-anatomical categories. type I: Subligamentous Herniation type II: Extra-ligamentous Herniation type III: Free-extraligamentous Herniation, Sequestratid disc. one-hundred and eight-six cases of herniatid intervertebral disc were evaluated in Department of Orthopedic Surgery, Wonju College of Medicine, Yonsei University and in Department of Orthopedic Surgery, Inchon Christian Hospital from March 1982 to March 1988. The results of study are as follows; l. Among 186 patients, type III sequestrated disc patients were 32 cases(17.2%). 2. The ratio between male and female was about 2:l. 3. The duration of symptoms in type II,III were longer than type I. 4. On examination, the physical changes were more common in type III than in other types. Positive well leg rasing test was prominant finding especially in type III. 5. The myelographic findings, in type III, were irregularity of dural sac at the level of vertebra body both on obligue and lateral view. 6. The C-T findings, in type III, were irregular protruded disc, or caudal or cephalsd migrated disc materials.

6.Expression of p53 and nm23 Proteins in Non-Small Cell Lung Cancer.

To elucidate the role of p53 and nm23 in the development, progression, and metastasis of non-small cell lung cancer, we studied 91 paraffin sections of the primary non-small-cell lung cancers and the 34 paraffin sections of their metastatic lymph nodes using the immunohistochemical method. The results are as follows: 1) The incidence of p53 protein expression was positively correlated with the staging of lung cancers (p<0.025). 2) The incidence of p53 protein expression was higher in the lung cancers with lymph node metastasis than in those without lymph node metastasis (p=0.009). 3) The incidence of nm23 protein expression was lower in the adenocacinomas than in the squamous cell carcinomas (p=0.032). 4) The incidence of nm23 protein expression was lower in the lung cancers with lymph node metastasis than in those without lymph node metastasis (p=0.026). The expression of nm23 protein between the primary lung cancers and corresponding metastatic lymph nodes showed positive correlation (Kendall's Tau-b correlation coefficient=0.47140, p=0.0068). 5) The expression of p53 was not correlated with the expression of nm23 protein (Kendall's Tau-b correlation coefficient=0.11387, p=0.2800). The above results suggest that an overexpression of p53 protein and a downregulation of nm23 protein are associated with tumor progression and metastasis in non-small-cell lung cancer.

7.Comparison of Lower Urinary Tract Symptoms and Urodynamic Study in BPH Patients with or without Prostatitis.

PURPOSE: Benign prostatic hyperplasia (BPH) and prostatitis may present similar clinical manifestations, although the severity of their symptoms is different. The differences between BPH patients with prostatitis and those without prostatitis were evaluated to help select appropriate therapeutic modalities and predict the prognosis. MATERIALS AND METHODS: 50 patients with BPH who were not associated with a neurogenic bladder, urethral stricture, diabetes, urinary tract infection or a previous prostate operation were studied. Of the 50 patients, 27 patients were free of prostatitis, and 23 patients had prostatitis. The mean ages of the two groups were 65.9+/-7.3 and 60+/-8.3 years, respectively. The evaluation was performed using the International Prostate Symptom Score (IPSS), uroflowmetry, the residual urine volume and urodynamic studies. Statistical analysis was performed using the Mann-Whitney test and the chi-square scales. RESULTS: According to the IPSS, the straining score was 4.3+/-1.0/3.2+/-1.8 (p<0.05) and the urgency score was 2.4+/- 1.9/3.8+/-1.1 (p<0.05) in BPH patients without or with prostatitis. In the L-PURR, grade 3 and higher, which indicated moderate to severe obstruction, was found in 10 patients with BPH alone (37.1%) and 6 patients with BPH associated with prostatitis (26.1%) (p<0.05). Detrusor instability was identified in 22% (6/27) of patients with BPH alone and 56.5% (13/23) of those with prostatitis (p<0.05). CONCLUSIONS: BPH patients associated with prostatitis had considerably more irritative symptoms, non-obstructive patterns and detrusor instability than those without prostatitis. These findings suggest that the relief of an obstruction by means of surgery alone should be reconsidered when treating BPH with prostatitis.

8.Multiple Fibromuscular Dysplasia of Arteries with Aneurysm Formation: A case report.

Multiple involvement of arterial fibromuscular dysplasia is unusal and such a case is quite limited in the literature. We present a case of multiple arterial fibromuscular dysplasia with aneurysm formation in a 38-year-old non-hypertensive man. Angiographic study showed multiple aneurysmal dilatations of the right renal, superior mesenteric, celiac axis, and left colic arteries. The resected right kidney demonstrated an aneurysm of the main renal artery and extensive infarction in the midportion of the renal parenchyma. The segmentally resected superior mesenteric artery disclosed a similar nature of aneurysm occluded by recent thrombi. Microscopically, all the resected arterial walls showed both intimal and medial fibroplasia together with periadventitial fibrosis resulting in marked narrowing of the lumen and subsequent thrombus formation.
9.Expression of tyrosine kinase A in the cerebral cortex of postnatal developing rat.

Tyrosine kinase A (TrkA)is an essential component of the high affinity nerve growth factor (NGF) receptor necessary to the mediate the biological effects of the neurotrophins, NGF. This study examined the distribution of TrkA-immunoreactivity (IR)cells in the postnatal rat cerebral cortex and the changes that occur in postnatal development as a result of the expression of this protein. TrkA-IR was detected at postnatal day (PD)3, PD6, PD9 and PD15. Base upon their somatodendritic morphology, the most commonly labeled cell type was the pyramidal neurons. At PD3 and PD6, layer I, II, III and V was immunopositive for TrkA, at PD9, not only at layer I, II, III, and V but also at layer VI. At PD15, the TrkA-positive cells were distributed in all layers. These TrkA-positive cells were not detected at PD0. In contrast, there was significant increase in the percentage of cells exhibiting TrkA-IR with development and the highest level was detected at PD15. These results suggest that the cerebral cortex expresses TrkA strongly during the postnatal period. Moreover, the postnatal development-related increase in the expression of TrkA-cells shows that NGF may have a trophic effect on these cerebral cortex neurons from the postnatal period.

10.Comparison of Wakefulness during Cesarean Section after Anesthetic Induction with Thiopental Sodium or Propofol.

BACKGROUND: Experience of wakefulness and pain perception during general anesthesia can be distressful to patients. For cesarean section, a light plane of general anesthesia is chosen for fetal safety and rapid recovery; there is an increased incidence of maternal wakefulness. Propofol may be the choice if smooth induction and rapid maternal recovery are desired. We compared propofol with thiopental sodium as an induction agent of anesthesia in cesarean section, noting in particular the patients wakefulness during operation. METHODS: Twenty six patients who underwent cesarean section received either thiopental sodium 4 mg/kg (n=13) or propofol 2.5 mg/kg (n=13) as an induction agent. To assess intraoperative wakefulness, a tourniquet was applied before the administration of succinylcholine for maintaining motor function in one arm. Wakefulness during anesthesia could be assessed by asking the patient to move her hand. RESULTS: Although the changes in blood pressure and heart rate were similar in both groups, the propofol group had a less increasing systolic blood pressure from the period immediately and 1 minute after intubation (P<0.05). The patients administered with propofol showed significantly higher incidences of "followed commands" and "made reaching movements" (P<0.05). The incidence of dreams was higher in the propofol group than thiopental sodium group. CONCLUSIONS: Propofol was similar to thiopental sodium in hemodynamic effects on mother, but incidence of intraoperative wakefulness was significantly increased in the propofol groups.
