1.Two Cases of Malignant Histiocytosis.

Two cases of malignant histiocytosis, in which skin involvement was a prominent finding at initial clinical presentation, are reported. 1nteresting histopathologic findings, including Langerhans granule in one case and numerous eosinophils in another, were observed. In one case remission was achieved with BACOP chemotherapy (Bleomycin, Adriamycin, Cyclophosphamide, Vincristine, and Prednisone). In the other case, the patient died nine months after the onset of the disease without any treatment.

3.Corneal Sensation after Phacoemulsification Versus Planned Extracapsular Cataract Extraction.

In order to compare the corneal sensation after phacoemulsification and the planned extracapsular cataract extraction(p-ECCE), we studied 40 eyes of 31 patients prospetively. A Cochet-Bonnet esthesiometer was used to measure corneal sensitivity preoperatively and at one day, three day, one week, one month, and two months postoperatively, by the same surgeon using the same technique. There was no difference in corneal sensation between phacoemulsification and p-ECCE group preoperatively. The mean corneal sensitivity at 10:00(2:00) o`clock in phacoemulsificantion/p-ECCE was 17.3+/-7.0(18.2+/-6.7)/9.9+/-1.5(10.3+1.9), 22.1+/-6.9 (23.2+/-6.3)/14.1+/-2.3(14.3+/-2.3), 29.4+/-7.7(30.6+/-7.2)/17.7+2.2(18.7+/-2.2), 37.7+/-9.3(37.8+/-9.4)/26.3+/-7.9(27.7+/-8.4), 56.3+/-7.6(56.3+7.9)/59.2+/-3.8(59.4+/-2.7)mm after 1, 3, 7, 30, 60 days, respectively. Corneal sensation at the center and the 3, 6, and 9 o`clock positions was not changed in all eyes. Corenal sensitivity was significantly more recovered in phacoemulsification group than the p-ECCE group at postoperative onemonth(p<0.05). However, there were no statistically significant difference in recovery of corneal sensation between phacoemulsification group and p-ECCE group at postoperative two months. Conclusively corneal sensation returned to peroperative level at two months postoperatively in both groups.

4.A Case of Ethmoid Sinus Mucocele with Transient Intraocular Pressure Elevation.

The ethmoid mucocele, resulting in the proptosis, is known a rare benign lesion. There are a number of ophthalmic manifestations of cystic lesion arising from the ethmoid sinus, which has destroyed the lamina papyracea and invaded the orbit. It is demonstrated by simple orbit X-ray and orbit CT scan. We experienced a case of a ethmoid mucocele in a 36-year-old man. which was associated with frontalgia, proptosis and transient intraocular pressure elevation and was improved after intranasal ethmoidectomy.

5.Scientific Publication Productivity of Korean Plastic Surgeons: An Analysis of 1974-2000 SCI Papers.

The aims of this paper are to identify where the quality research activity has been and is carried out in Korea, and to examine weather the Korean plastic surgeons have sufficient capacity to place their official journal "The Journal of Korean Society of Plastic and Reconstructive Surgeons(KPRS)" to international databases. We investigated the publication productivity of all departments of plastic surgery in Korean medical colleges, hospitals, and clinics as measured by the number of papers in journals indexed in SCI(Science citation index). The 27 year period from 1974 to 2000 is covered. A total of 195 papers is published in SCI journals by the plastic surgeons in Korea. The first SCI paper was published in 1985; the number of publication has gradually increased. Since 1998, over 30 papers are annually published in SCI journals by Korean plastic surgeons. The production of SCI papers are concentrated in a few universities. About seventy percent of SCI journal papers is published by top seven medical schools; 25.6% by Yonsei university. In 1985, only one medical school was involved in the production of SCI journal papers. After 1997, however, more than 10 medical school and some surgeons in local clinics published SCI journal papers. The percent of SCI journal papers among all papers written by the Korean plastic surgeons increased recently: About 15% are published in SCI journals in 1999 and 2000 respectively. These data suggest that Korean plastic surgeons have enough capacity to place their official journal KPRS into international data bases.

7.Tissue Plasminogen Activator in the Treatment of Fibrinous Membrane after Cataract Surgery.

We injected tPA into the anterior chamber of 43 pesudophakic eyes with moderate to severe fibrinous membrane which had developed following cataract surgery from January, 1990 to August, 1990 at St. Mary's hospital. The results were as follows: 1. Intraocular tPA injection was resulted in complete fibrinolysis within 1 hour in 38 of 43 eyes and patial fibrinolysis in 5 of 43. 2. No corneal endothelial cell loss was found, and tPA did not appear to cause a significant rise in intraocular pressure. 3. Complications of tPA injection included mild, transient, periocular pain(3 eyes), anterior chamber fibrin debris(2 eyes) and anterior chamber hemorrhage(1 eye). The results indicated that tPA is dramatically effective for the clearance of fibrinous membrane after cataract surgery.

8.Adenoid Cystic Carcinoma in the Orbit.

The adenoid cystic carcinoma in the orbit is rare, but is the most common in the malignancy of the orbit. The clinical signs are proptosis with displacement of the globe downward and inward, limitation of ocular movements and diplopia. The diplopia and ocular pain is due to the tumor metastasis to the extraocular mescle and nerve sheath. The authors have experienced a case of adenoid cystic carcinoma in the right orbit in a 72-year-old man.

9.Usefulness of Fat-Suppressed Turbo SE MR Imaging for the Evaluation of Meniscal Tears.

PURPOSE: Suppression of the relatively high signal intensity of fat leads to more efficient use of thedynamic range for display of tissue contrast. In order to evaluate meniscal tears, we compared a fat-suppressedturbo spin-echo(FSTSE) sequence with turbo SE(TSE). MATERIALS AND METHODS: One hundred and seven knees in 103consecutive patients referred for MR study of the knee were imaged using both FSTSE and TSE sequence. The turbo SEsequence provided proton density-weighted and T2-weighted images (dual echo technique) with an effectiveecho-train length of five. For fat-suppression, a frequency-selective chemical presaturation pulse was applied.Forty-two knees (84 menisci) were studied arthroscopically and the findings were taken as the reference standard.FSTSE and TSE images were reviewed retrospectively by two radiologists. Next, for each patient, the quality ofFSTSE and TSE images was compared;the former were scored by each reviewer as either superior to, equal to, orinferior to TSE images. RESULTS: Among the 214 menisci evaluated, the results of FSTSE and TSE imaging were verysimilar (kappa index 0.87). Twenty four tears were found during arthroscopy in 84 menisci. FSTSE imaging was moresensitive than TSE (96% versus 83%), though specificity was equal(98%). Among the 107 cases, FSTSE images wererated by both observers as superior to TSE images for overall quality and visualization of the meniscus itself.CONCLUSION: For the evaluation of meniscal tears, FSTSE sequences were more sensitive than those obtained withTSE, and their image quality was superior. For the study of meniscus tears among a large population, FSTSE istherefore more useful than TSE.

10.A Clinical Study of the Biconvex Posterior Chamber Lens Implantation in Cataract Surgery.

One hundred and twenty-three cases of planned ECCE with biconvex posterior chamber lens implantation were performed in St. Mary's Hospital from April 1988 to February 1990, were followed up for more than 6 months. We also analyzed 123 cases of planned ECCE with convex-plano posterior chamber lens implantation during the same period. The results were as follows: 1. The difference of visual acuity between the two groups was not statistically significant. 2. Of the 123 eyes with biconvex lens, 106 eyes (86%) were within +/- 0.50 spherical equivalent diopter compared to 55 of 123 eyes (44.7%) with convex-plano lens. This difference is statistically signficant (p<.001). 3. Eight out of the 123 eyes with biconvex lens (6.5%) developed after-cataract and two of these eight eyes were of the central type: 36 out of the 123 eyes (29.3%) with convex-plano lens developed after-cataract and 31 of the these 36 eyes were of the central type. This difference is statistically significant(p<.001).
