3.The Study of High Frequence Hearing Loss in Pilots.

BACKGROUND: Hearing loss is associated with environmental noises. Not only the strength of the noise but also the exposure time of that seems important. We investigated to find the factors associated with hearing loss in pilots. METHOD: For the anlysis of hearing loss in pilots in the air force, we performed an audiometiric test on 563 pilots from 2000 to 2001 and analysed the results. REUSLT: The type of aircraft or regiment is not associated with hearing loss. The age, tenure and total flight time are the factors associated with hearing loss, especially in the high frequency zone. The flight time in the recent six months is shown not to be associated with hearing loss which means an exposure to noise in a longer term is important. In this study, the hearing threshold increased at 6,000 Hz regardless of age. CONCLUSION: The reason may be due to the noise environment of pilots. Therefore an additional study about pilots' normal limits of hearing threshold at 6,000 Hz and clinical meanings of that is necessary.

Menetrier's disease is a rare cause of thickened gastric wall. The differential diagnosis of diffuse gastricwall thickening such as lymphoma and adenocarcinoma should be expanded to include Menetrier's disease. The authorsexperienced a case of Menetrier's disease accompanied deep vein thrombosis, which was proved by operation andpathology. We carried out double cntrast upper G-I series and computed tomography , and report with review ofliteratures.

6.Mechanical Circulatory Supports in the Treatment of Fulminant Myocarditis.

BACKGROUND AND OBJECTIVES: Although fulminant myocarditis (FM) is known as a fatal disease, once patients have recovered, with aggressive pharmacologic therapy and/or mechanical circulatory supports (MCS), including intra-aortic pump, temporary ventricular assist device and percutaneous cardiopulmonary support, they may return to normal life with an excellent long-term prognosis. Elevated C-reactive protein, Creatinine phosphokinase and cytokine concentrations, decreased left ventricular ejection fraction and intraventricular conduction disturbances on admission may predict the progress of acute myocarditis to fulminant course. Early MCS helps save life and prevent multi-organ failures in patients with FM. The type of MCS may not affect the outcome of the clinical course when its complications are managed properly. Since other managements with immunoglobulin, antiviral agent or monoclonal antibody remain to be confirmed, aggressive hemodynamic support with MCS is the best management for patients with FM, who once recovered from the acute phase can return to normal life.

7.Cardiac Rupture of the Junction of the Right Atrium and Superior Vena Cava in Blunt Thoracic Trauma.

Cardiac rupture following blunt thoracic trauma is rarely encountered, since it commonly causes death at the scene. With advances in critical care, blunt cardiac rupture has been successfully treated with well-organized team approach including an emergency physician, anesthesiologist, and cardiac surgeon. We encountered a patient with blunt cardiac rupture of the junction of the superior vena cava and right atrium that extended 7 cm to the right ventricular junction. The patient was successfully resuscitated after a closed thoracostomy and pericardiocentesis with fluid loading. Cardiac injury was repaired via mid-sternotomy without cardiopulmonary bypass. The patient recovered without complications and was discharged on the 7th day after surgery.

8.A Serial Rapist - Male Identification and Crime Scene Analysis in the Cases of a Serial Rapist.

The study presents the application of STR polymorphisms to male identification in the cases of a serial rapist in Korea. Since October 2000 a rapist from Cheongju city (a town located in central district) committed at least 36 rapes. DNA profiles obtained from semen stains, buccal cells and tongue tissue left at the scenes of crime and vaginal swabs of victims gave information that one and the same man had committed all the rapes. The Y-chromosome haplotype (12 loci) obtained was used for the identification process of rapist in mixed genotypes including victim's alleles. The crime scene data were referred from investigating police authorities described the relationship of victim to the offender, victim's age, time and place of sexual assault and rape locations. The behavioural analysis of crime scenes showed more similarities than differences for related crimes. These components were usually assessed in terms of proximity in time and space. Certain crime scene aspects of a serial rapist help identify components of offender spatial behaviour. As a result of this work, future research on the geography of rape and behavioural aspects of rapist will be directed towards those aspects of offences, which have been identified as relevant to the distance between an offender's base, and the site where he approached his victim.

9.Quantitation of the Peripheral Blood T Cell and T Subsets Patients.

The study was performed to measure and compare the peripheral blood T cell and T subsets in normal controls and psoriatic patients. Thirty-two normal controls and fift:en psoriatic patients were subjected to the study and the percentages and the rati vs of peripheral blood T cell and T subsets were measured. The results were as follows: 1. Mean percentages of peripheral blood lymphocytes reactive with OKT3 monoclonal antibody in psoriatic patients were 72. 8+-8. 2%, They decreased significantl) as compared with these in control group(76, 6- i-4. 7%). Mc an percentages of peripheral blood lymphocytes reactive with OKT4 monoclonal antibody in psoriatic patients were 47. 3+6, 7p;. They increased as compared with these in control group(46. 5+-3. 9p;), but the increase was insignificant. 3. Mean percentages of peripheral blood lymphocytes reactive with OKT8 monoclonal antibody in psoriatic patients were 27. 2+5. 5g, They decreased significantly as compared with these in control group(30, 6- l-4. 3%) 4. Mean ratios of lymphocytes reactive with OKT4 monoclonal antibody to these reactive with OKT8 monoclonal antibody in psoriatic patients were 1.8+- 0. 48 They increased significantly as compared with these in control group(1. 6+ 0.34).
