2.Tracheobronchial Aspergillosis An autopsy case report.

Tracheobronchial aspergillosis is an unusual form of invasive aspergillosis characterized by noninvasive or only superficially invasive tracheobronchitis with a propensity for dissemination. We report a two-year-old male who suddenly died of respiratory failure. Postmortem examination revealed a pseudomembrane covering the mucosa of larynx, trachea and bronchial tree of both lungs. This pseudomembrane was composed predominantly of Aspergillus hyphae. There was transmural necrotizing bronchitis with fungal invasion to the narrow zone of peribronchial tissue, and dissemination to the stomach and kidney. This form of pulmonary aspergillosis had not been reported in this country.

3.Intraosseous Ganglion of Femoral Head: A Case Report.

Intraosseous ganglion is an uncommon benign tumor thought to be caused by an intramedullary metaplastic event. We recently experienced a case of intraosseous ganglion that occurred on the femoral head of a 65-year old man who complained of intermittent hip pain. Complete evaluation including bone scan, CT, MRI was done. His pain disappeared after curettage and bone graft was done on the lesion.

5.No-incision pubovaginal suspension for stress urinary incontinence: 15 cases.

Fifteen stress urinary incontinence patients were treated using the modified endoscopic suspension of the bladder neck that eliminates all incisions during the period from January 1990 to December 1990. The average operation time was 26 minutes and the mean days of urethral Foley catheter removal were 3.9 days operation. Two point three days were required to restore the normal voiding pattern. Of 15 patients treated. 1 failure occurred, for an overall success rate of 93%. There had been no significant complications. Therefore, no-incision pubovaginal suspension was simple and effective means treating stress urinary incontinence.

6.A case of leiomyoma on urethrovaginal septum.

Leiomyoma is rare. Most of patients present with urinary tract infection, hematuria, dyspareunia, or complaint of mass. A case of leiomyoma on urethrovaginal septum in a 36 years old female was presented. The presenting symptom was a protruding mass at the anterior vaginal wall The mass was removed surgically and confirmed as a leiomyoma pathologically.

7.Systolic Time Intervals in Valvular Heart Disease.

Systolic time intervals were studied in a total of 83 patients with pure or predominant isolated valvular heart disease. They consisted of three groups of patients : namely, 38 cases of mitral stenosis, 25 cases of mitral regurgitation and 20 cases of aortic regurgitation. The mean of the electromechanical systole was within normal ranges in all three groups, and threre was no significant difference between the groups. The mean of the left ventriclar ejection time was also within normal limits in all groups, as was that of the electromechanical systole, but it was significantly shorter in patients with mitral regurgitation than in the others, and was significantly longer in patients with aortic regurgitation. The mean of the pre-ejection period and the ratio of the pre-ejection period to the left ventricular ejection time were within normal ranges in patients with aortic regurgitation, whereas both parameters were significantly increased in patients with mitral stenosis or mitral regurgitation, particularly in the latter.

9.Level of Emergency Medical care Required in Religious Mass Gathering.

STUDY OBJECTIVE: determine the level of medical care required for mass gatherings and describe the types of medical problems encountered in a religious mass gathered ceremony. DESIGN: Standard charts and a four-tiered triage system(minor, moderate, urgent, and emergent) were developed before the event. The triage system was applied to each chart retrospectively by a single emergency physician. SETTING: Medical staff(10 physicians,13 nurses,1 pharmacist, and 54 first-aid attendants) were based in 8 advanced life support (ALS) clinics. INTERVENTIONS: First-aid attendants referred patients to the clinics, where nurses conducted initial assessments and referred patients to physicians at the venue. Three ambulances were stationed at the venues. RESULT: 22 trauma patients were developed and 183 medical complaints were encountered. Only 7 urgent medical problems were encountered.

10.A Case of Emphysematous Pyelonephritis.

Emphysematous pyelonephritis is a rare complication of severe acute pyelonephritis and a suppurative infection of the renal parenchyma characterized by the production of gas by bacteria through fermentation. Emphysematous pyelonephritis occurs in patient with diabetes mellitus or urinary obstruction. We review the diagnosis and management of emphysematous pyelonephritis and report a case of emphysematous pyelonephritis developed in renal tuberculosis.
