1.A Stduy on the Standardization of the Description of Orthopedic Diseases for Electroic Data Processing System (EDPS)

It is now well-known that the electronic data processing system(EDPS: computer) is very useful for indexing, sorting, verification, and so on. And a few orthopedics in Korea are processing informations with computers for their medical records respeetively. But since there are no established principles on the description of orthopedic diseases, a computer programmer unfamiliar with medical terms is poor in electronic processing for medical records. After processing with an IBM computer for several years, it has come to our attention that we have a need for standardization of the description of orthopedic diseases for EDPS. Based largely on the International Classification of Diseases(ICD-9th), we propose the following: 1. Diseases should be written in English, and whether it be capital letter or small letter does not matter. 2. Description of the disease should be in order of title, region of the body, cause or present status, and side of the body. The sequence may be changed without problem, but only the noun form should be used. It is also recommended that the key words in articles be defined. 3. Abbreviated forms should be avoided. If inevitable, the abbreviation should be used consistently from the first to the 1ast. 4. Connection with other types of medical records such as medical insurance system by registering the code number of the International Classification of Disesaes at the end of the description or on another line.

2.A Study of the SOURCIL in Idiopathic Avascular Necrosis of the Femoral Head

A carefull inspection of an AP X-ray of a normal hip reveals in the weight-bearing surface of the ilium a curved area of dense bone, called SOURCIL by Pauwels (1963). The configuration of this dense area is semilunar and represents subchnodral bony eburnation due to a respose by the articular portion of the ilium to the stress provoked by the compressive force acting on it. However, in many cases of idiopathic avascular necrosis of the femoral head, changes in SOURCIL may be seen. So we reviewed our 49 patients (unilateral 16, bilateral 33) since 1983 with comparison to normal hips (100 persons). The results are as follows; 1. SOURCIL is not visible at birth and by walking, it is developed with age and has the adult configuration at the age of 18. It is thought the configuration and sloping angle of SOURCIL is not changed with age after skeletal growth is completed. 2. Although the position of the X-ray tube may be changed, the shape and angle of SOURCIL is not changed significantly. 3. The average SOURCIL angle is 2.1° in normal hip group and 3.5° for the avascular necrosis group but, there is no meaningful statistical differences. It seems to be meaningful that the mean SOURCIL angle is 10.3° for patients under the age of 35 in avascular necrosis groups (9 persons). 4. If a high SOURCIL angle (about 10° or more) is present, the hemiarthroplasty such as bipolar endoprosthesis should be avoided because there is biomechanically far more wearing of the acetabular roof after that kind of operation. If inevitable, deepening the acetabulum by reaming is recommended for prevention of superolateral migration of prosthesis after operation. 5. With close observation of SOURCIL before and after operation, one can easily point out what is able to predict the prognosis on the biomechanical basis.

5.Spectral Analysis of REM Sleep EEG in Narcolepsy and REM Sleep Behavior Disorder.

INTRODUCTION: It has been proposed that narcolepsy and REM sleep behavior disorder (RBD) have overlapped symptom profile and pathophysiology. This study was aimed at measuring and comparing changes in EEG frequency band of REM sleep in narcolepsy and RBD, applying EEG spectral analysis method. METHODS: Nine patients diagnosed as narcolepsy and the same number of RBD patients were studied. Spectral analysis of the REM sleep EEG was performed in each patient on 9 epochs selected evenly from the first, second, and third REM periods. Then, we compared frequency band percentages of REM sleep EEG in narcolepsy and RBD. RESULTS: Narcolepsy patients had significantly higher delta frequency ratio than RBD ones (p=0.00). In alpha and beta2 frequency bands, RBD patients showed higher percentage than narcolepsy ones. Slow wave sleep was more prevalent in narcolepsy patients. But, no difference of REM sleep percentage was found between the two groups (p=0.93). CONCLUSION: Higher delta frequency ratio in REM sleep of narcolepsy patients than RBD ones reflects that sleep-promoting mechanism is more dominant in narcolepsy than in RBD.

7.One - staged Saucerization and Cancellous Chip Allograft for Treatment of Chronic Localized Osteomyelitis.

Treatment of chronic localized osteomyelitis is the same as the other osteomyelitis, that is, curettage and debridement of all dead tissue which is called saucerization. After saucerization there leaves empty cavity and autogenous bone graft has been used for filling the cavity in lower extremity hecause of weight bearing and avoiding fracture. We treated seven cases of chronic localized osteomyelitis with one-staged saucerization and cancellous chip allograft and retrospective analysis was done. All healed without complication and no recurrence was ohserved. Most orthopaedic surgeons are not willing to use allograft on the site of osteomyelitis because of the fear of recurrence. But. we think that as far as complete removal of infected and dead tissue, allograft could he a good solution in terms of rapid remodelling and early weight hearing. Another advantages of allograft are to get a large amount of bone and no damage of iliac apophysis. In summary, one-staged saucerization and cancellous chip allograft would he very useful treatment for chronic localized osteomyelitis.

8.Treatment of Congenital Dislocation of the Hip with the Pavlik Harness

The congenital dislocation. of the hip is a common congenital disease in the orthopedic field. It is well known that the early diagnosis and effective treatment is very important for the patient to provide a favorable function in the whole life. The Pavlik harness made an important contribution to the treatment of children before standing age. The authors report a clinical experience of 34 patients(35 hips) with congenital dislocation of hip who were treated with Pavlik harness at the Department of Orthopedic Surgery of Chonnam University Hospital. There were total 230 patients(237 hips) diagnosed as congenital dislocation of hip for 10 years from 1978 to 1987. Fifty-one patients(52 hips) of them were treated with Pavlik harness. Only 34 patients(35 hips) were included in this study excluding 17 patients(17 hips) because of inappropriate records or follow-up. The follow-up period ranged from 1 year to 6 years with an average of 2 years and 5 months. The results are as follows ; 1. The most patients were girls, comprising 33 girls and 1 boy. 2. The age of children was 124.6days in average ranging from 13 to 201 days. 3. Of the 35 hips treated with Pavlik harness, satisfactory reduction was obtsined in 30 hips(85.7%). For 5 failed cases, the authors performed closed reduction under the general anesthesia in 3 cases and open reduction in 2 cases. 4. The causes of reduction failure were impossible reduction in 3 cases, nonconcentric reduction in 1 case and repeated redislocation in 1 case. 5. The acetabular and metaphyseal-edge angle improved from the average 33 and 5.8 degrees at the first visit to the average 19.9 and 30.6 degree at the final follow-up (20.7 and 29.2 degrees in normal side). 6. When compared with the value of the 30 hips which were reduced successfully, the initial acetabular angle of 5 failed cases was similar but the metaphyseal-edge angle of them was very low. 7. There was no reduction failure in 27 hips which had metaphyseal-edge angle above 10 degrees. But 5 reduction failures were observed in 8 hips with below −11 degrees. 8. An avascular necrosis was observed in only one patient. In conclusion, the Pavlik harness proved to be a successful means in treating patients with congenital dislocation of hip under 7 months of age with particularly above −10 degrees in metaphyseal-edge angle.

9.Functional Evaluation of Tumor Prosthetic Arthroplasty for the Malignant Bone Tumor around the Knee: Interim Report

With the recent development of tumor prosthesis, limb salvage technique has attributed to the preservation of the limb function in patients with malignant bone tumors around the knee without substantial difference of local recurrence and the survival rate. From Feb., 1991 to Sep., 1992, 9 patients were treated with limb salvage operation with total knee arthroplasty due to the malignant bone tumor around the knee at Dept. of Orthopedic Surgery, Kyungpook National University Hospital. We applied the above procedure mainly to young adult whose tumor was expected to be excised wide marginally without damaging major neurovascular structures. We evaluated the functional status of all 9 patients(4 osteosarcoma, 2 chodrosarcoma, 2 malignant fibrohistiocytoma, 1 malignant giant cell tumor) to estimate the efficacy of limb salvage operation with tumor prosthesis total knee arthroplasty. Mean follow up period was 10 months. Primary tumor site was 4 in distal femur, 5 in proximal tibia and stage was 4 IIa, 5 IIb. Range of the motion of knee is maximum 0°

10.Analysis of Local Recurrence of Giant Cell Tumor.

PURPOSE: This study was aimed to evaluate the recurrence rate of a giant cell tumor (GCT) of the bone. MATERIALS AND METHODS: The medical records of fifty four patients who were diagnosed with a giant cell tumor of the bone between March 1980 and December 2008 were analysed retrospectively. Among 54 patients, 27 were men, remaining 27 were women with the mean age of 33.1 years (range, 13-67 years). The mean duration of follow-up was 67.1 months. RESULTS: Twenty-one patients (38.9%) had a local recurrence. The mean time to recurrence was 21.5 months (range, 2-59 months). The local recurrence rate of the upper extremities was higher than that of lower extremities. According to Campanacci classification, patients with a grade I diseae had lower recurrence rate than those with grade II or III disease. There was no significant differences in the recurrence rates based on cryotherapy, the filling of bone cement or bone grafts and surgical margin. CONCLUSION: To prevent local recurrence of GCT of bone, curettage of the tumor and elimination of the remaining cells are more important than adjuvant therapy.
