Background/Aims: Pancreatitis is one of the major complications of ERCP. The purpose of this study is to evaluate the incidence rate and risk factors of ERCP- induced pancreatitis. Method: We analysed 99 consecutive patients(45 males and 44 females) undergoing ERCP from March to September, 1994. The level of serum amylase was measured and clinical assessment of pancreatitis was performed before, 24 hour, 48 hour, and 72 hour after ERCP. We defined clidical pancreatitis as combination of elevated amylase level over 2 days with abdomial pain and tenderness.(continue..)

We report a case of toxic shock syndrome in 17-year-old female patient with skin abscess. She had a skin abscess on left inner thigh about 6th days ago, but no treatment was done. And then she presented with the features of high fever, hypotension, generalized rash, myalgia and diarrhea. After the treatment with anti-shock measures, skin wound dressing and systemic antibiotics adminstration, she was completely recovered with generalized desquamation on 10th hospital day.

Authors experienced a case of human infection with Anisakis sp. in left tonsil. The case was 27 year Korean female who visited Department of Otolaryngology, Severance Hospital with the chief complaints of swollowing difficulty, and the sensation of a foreign body in the throat, on November 20th 1968. Physical examination of the oral cavity revealed that both palatin tonsil were chronically enlarged. A larval worm was found in crypt of the upper portion of the left palatin tonsil. Chief complaints of the patient was improved after removal of it by forceps. The greyish white living worm was fixed in lactophenol solution for morphological study. The worm measured 36 mm in length by 0.6 mm in width, 2.4 mm in esophageal length, 1.4 mm in ventricule, 0.1 mm in tail length, and 0.025 mm in caudal spine length, respectively and histological study was carried out from the serial section of the larva which was stained with hematoxylin eosin. Authors identified that the larva was Anisakis species.

The patient was a 34-year-old male who had the dorsum of his right foot burned with strong alkali. Subsequently, the wound changed to an ulcer 3 days after the accident, and it was resistant to conventional treatment. After seventeen days of a granulation tissue-forming period, we applied a suction blistered epidermal graft using an intrinsic heating system we had devised. Suction blistered epidermal grafting is a less painful method with minimal scarring that is applicable to various skin diseases and researches. However, currently used techniques require patients to be immobile for some time and it makes physicians and patients feel uneasy and uncomfortable. The time needed for the suction blister formation is dependent on many factors. Among them, the temperature and the suction pressure are the important factors that can be modified easily. So, we devised a suction device having the capability to heat itself and used a suction machine with a pressure meter. We applied this to the above patient. We present a case of a chemical burn wound treated with suction blistered epidermal grafting.

Eczema herpeticum is a herpes simplex virus infection with disseminated skin involvement superimposed on a pre-existing dermatosis. Dariers disease has been reported to be among the dermatoses susceptible to the sudden onset of a widespread vesicular eruption accompanied by high fever known as Kaposis varicelliform eruption. We report a case of eczema herpeticum in a 46-year-old woman associated with Dariers disease.

A 5 year old girl was visited with the chief complaint of abnormal genitalia from birth. Physical examination revealed enlarged clitoris, urogenital sinus formation, increased urinary 17-ketosteroid, Jailer's test positive and sex chromatin positive. Diagnosis was made congenital adrenogenital syndrome.

PURPOSE: During the night time decision making on patients suspicious of appendicitis is often difficult because diagnosis and timing for operation are frequently delayed. Therefore, we analyzed above cases and solution is suggested. METHODS: This retrospective study included 360 patients who underwent laparotomies for suspected appendicitis at Hanil Hospital during one year, from March 1998 to Feb. 1999. They were divided into two groups according to presenting time to physician (Day time: 6 a.m. to 6 p.m., Night time: 6 p.m. to 6 a.m.). Sex & age distribution, time of presentation to physician, duration of symptoms, symptoms & physical findings, white blood cell counts, interval from presentation to operation, hospital stay, and pathologic diagnosis were compared. RESULTS: There were no significant differences in sex & age distribution, duration of symptoms, symptoms & physical findings, white blood cell counts, pathologic diagnosis between the two groups. However, during the night time, the interval from presentation to operation was longer than that of the day time (9.15 hours versus 4.83 hours, p<0.001), the rate of delayed appendectomy during the night was 58.0%, the rate of negative laparotomy increased when appendectomy was delayed for more than 12 hours compared with less than 12 hours (28.1% vs 11.7%, p<0.01), and in the cases with perforated appendicitis, delayed appendectomy for more than 12 hours had longer hospital stay compared with less than 12 hours (12 days vs 9.44 days, p<0.01). Factors causing delayed appendectomy were related to the physician (42.5%), lack of anesthetic & nursing supports (19.5%), failure to structure the operation team (20.7%), and patient itself (17.3%). When white blood cell counts were rechecked in the next morning, levels above 10,000 cells/mm3 were highly associated with appendicitis in contrast to that below 10,000 cells/mm3 (91.7% vs 43.5%, p<0.002).
