1.Anatomy of Pulmonary Lobes and Fissure: A Study Utilizing HRCT.

PURPOSE: This study was performed to evaluate the features of pulmonary lobes and fissure in Korean and to compare with the previous results. MATERIALS AND METHODS: HRCT scans of 82 healthy Korean adults, performed with scan interval of 1.5/2.0 cm, were reviewed. RESULTS: Located mostly on inner aspect, incidences of incomplete oblique and horizontal fissure were 60% (Rt 48%, Lt 39%) and 51% respectively, lower than 70--80% and 60--75% of western studies. Superolateral oblique fissure, inferior accessory fissure, left horizontal fissure and other accessory fissure were shown in 1%, 13%, 7%, and 8% respectively. Lower lobe upper part faced laterally (Rt 91%, Lt 82%) while lower part faced medially (Rt 61%, Lt 79%). Right middle lobe ws highest posteromedially (49%) and was convex (97%). CONCLUSION: Features of pulmonary lobes and fissure in Korean were similar to the results of previous reports but with lower incidence of variation.

3.A STUDY THE SHADE CHANGE OF SEVERAL KINDS OF ALL CERAMIC CROWNS USED FOR METAL DOWEL CORE.

The purpose of this study is to compared and analyze the shade changes(deltaE)about In-Ceram(R),IPS-Empress(R),OPC(R) by using of the spectrophotometer arising from initial status and before and after cementation of the resin cement mounted on the metal core. We used a couple of statistics such as 'One-Way ANOVA'and Multiple Range Test. We could be able to verify significantly what is being discussed here up to 95%. The results drawn from our research are as follows: 1. At the time of our experiments regarding the initial shapes of all-ceramic and mounting status of all-ceramic on the metal crown(deltaE1), and a comparison of mounting of all ceramic on the metal crown with all-ceramic cemented on the metal core(deltaE2), at the time of shade change of initial shapes and after we cemented on the metal core. (1) no significant difference among all-ceramics was found. (2) no particular difference was found regarding the In-Ceram(R) (3) a significant difference between the deltaE1 and deltaE2 regarding the IPSS-Empress(R) was found (P<0.05). (4) a significant difference between the deltaE1 and deltaE2 regarding the OPC? was found (P<0.05). 2. When we compared the shade changes (deltaE) resulted from before and after the cementation on each of the parts involved of some all-ceramic . we could be able to find shape change increase from incisal third, middle third, and cervical third in that order in In-Cream(R)(spinell), IPS Empress(R) and OPC(R) all. In addition , we could be able to find a siginificant difference between cervical third and incisal third, middle third. (P<0.05) From what we have just seen, we might conclude that there is a significant shade change difference before and after the cementation with respect to IPS-Empress(R)and OPC(R). In addition, we could also be able to find more shade change difference at the cervical third rather than incisal third and middle third depending on the parts involved.

4.Krukenberg Tumor: Clinico-pathologic analysis of 36 cases.

A total of 36 typical Krukenberg tumors of the ovary was obtained from the pathology file of the Department of Pathology, College of Medicine, Seoul National University during a period of 17 years from January 1968 to December 1984. By definition, all were characterized by the presence of mucin containg signet ring cells within the cellular, nonneoplastic ovarian stroma. The clinico-pathologic characteristics of 36 Krukenberg tumors were as follow: The Krukenberg tumors accounted for 16.3% of all ovarian malignancies. The age of the patient at the time of diagnosis of the Krukenberg tumor ranged from 28 to 69 years with an average of 43 years. A primary carcinoma of stomach (31 cases) of colon (1 case) was found in 32 (88.9%) of 36 patients. The primary carcinomas was not detected in four cases, and autopsy was not performed in any case. In 22 cases the primary carcinomas had been diagnosed before ovarian tumors were found. The ovarian and the primary carcinomas were identified synchronously in 6 cases, while in 8 cases the primary carcinomas were not discovered until after the ovarian tumors had been treated. The gross diameter of the ovarian tumor ranged from 1.5cm to 28cm with an average of 10.3cm. The largest weighed 4,550gm. The Krukenberg tumors typically formed rounded or reniform, solid mass that were coarsely lobulated or bosselated. The cut surface was yellow white and associated frequently with nodular, myxoid or gelatinous area and cystic changes of various size. Both ovaries were involved in 29(80.6%) of the cases and one ovary in 7(19.4%). Krukenberg tumors classified into the three major types on the basis of the characteristic morphologic patterns of signet ring cells. The first type was classic Krukenberg tumor(28 cases) represented by predominent components of typical signet ring cells. The second type was tubular Krukenberg tumor(5 cases) characterized by tubular structures resembling a Sertoli-Leydig cell tumor. The third type was re tiform Krukenberg tumor (3 cases) characterized by an irregular network of elongated, often slitlike tubules and cysts, which resembled the rete testis.
5.Diffuse Panbronchiolitis: High-Resolution CT Findings and Correla:don with Pulmonary Function Test.

PURPOSE: Diffuse panbronchiolitis(DPB) is a chronic inflammatory airway disease of unknown causes mainly affecting the respiratory bronchioles and the more proximal bronchi. Findings on chest radiographs and high-resolution CT(HRCT) are well known and Akira classified HRCT findings of DPB into four types. Purpose of this study is to evaluate the relationship between findings of HRCT and PFT. METHODS AND MATERIALS: We retrospectively analyzed the chest radiographs and HRCT images of eleven patients with DPB and compared CT classification with pulmonary function test. RESULTS: Chest radiographs usually showed small nodular opacities throughout the both middle and lower lungs. The HRCT findings of DPB were centrilobularly distributed small nodular densities, branching linear densities contiguous with small round densities, dilated and thickened peripheral and central airways including bronchioles. More than one CT type by Akira's classification, usually two or three types, were found in nine patients. There was good correlation between CT types and FEV1%(p<0.05), CT types and FEV1/FVC (p<0.05) respectively. CONCLUSION: HRCT seems to be more useful for diagnosis and disease progression of DPB as compared with the chest radiographs or clinical stage such as pulmonary function test.

6.In Vivo reaction of the Highly Porous Glass Ceramics in the Rabbit Tibia: Radiological and Histological Analysis

To evaluate the possibility of the newly developed highly porous glass ceramics as a space-filler in the cavitary bone defect, we made the opening sized 1 × 0.5 cm on the medial aspect of the right proximal tibia of nine rabbits. We impacted the highly porous glass ceramics firmly to the medullary cavity of rabbit tibia through the opening. Each three were sacrificed at 4th, 8th, and 12th week and analyze in vivo reaction of the glass ceramics in rabbit tibia with radiological and histological methods. On radiological examination, radiolucent line was seen around impacted glass ceramics at 4th week, but this radiolucent line was obliterated gradually to 12th week. On histological examination, new bone formation with osteoblast was appeared at 4th week without foreign body reactions. At 8th week, newly formed bone infiltrated into the porous space between glass ceramics particles was noticed, and the surface of glass ceramics was tightly bound by newly formed bone with osteoblastic rim and mature bone, At 12th week, the amount of newly formed mature bone increased, though there was on evidence of resorption of glass ceramics particle. So, we suggest that the highly porous glass ceramics is one of the possible artificial bone graft substitutes, especially as a space-filler.

7.A Family of Hereditary Olivopontocerebellar Atrophy (Menzel type OPCA, OPCA III with retinal degeneration).

This is a case report of the familial olivo-ponto-cerebellar atrophy (Menzel type OPCA, OPCA III with retinal degeneration). The patient is a 37 year-old male with 5 years history of slowly progressive cerebellar ataxia, dysarthria, visual change, horizontal nystagmus and signs of pyramidal dysfunction. The CT brain scan shows significant atrophy of brain stem and cerebellum with dilatation of cisterns. His younger sister, also, has similar clinical manifestations and radiological abnormalities, but mild. By history, his mother who died at 47 years of her age looked like to have same kind of chronic disease.

8.A Case of Renal Cell Carcinoma in Horseshoe Kidney.

A 66-year-old man with incidentally detected mass in the left kidney visited our hospital. Intravenous pyelogram, ultrasonogram, computerized tomography suggested a renal cell carcinoma in the left side of the horseshoe kidney. A left radical nephrectomy with division of the isthmus was performed. The pathological diagnosis revealed renal cell carcinoma consisting of clear cell type without invasion of the capsule or renal pelvis.

9.Biliary Tract & Pancreas; Effectiveness of Endoscopic Ultrasonography in Detecting the Extrahepatic Choledocholithiasis.

BACKGROUND/AIMS: Ultrasonography is the easiest, fastest procedure for the diagnosis of choledocholithiasis, but the diagnostic failure are mostly because of the intrapancreatic level of this condition and the absence of bile duct dillatation. Endoscopic ultrasonography is a promising procedure for the diagnosis of extrahepatic cholestasis due to noninvasiveness and accuracy in the digestive gas interposition. The aim of this study was to prospectively compare the diagnostic accuracy of endoscopie ultrasonography with abdominal ultrasonography and endoscopic retrograde cholangiography in 66 patients with suspected choledocholithiasis. METHODS: All of the patients had abdominal ultrasonography, endoscopic ultrasonography ~and endoscopie retrograde cholangiography within 72 hours and final diagnosis was determined by endoseopic retrograde cholangiography with sphincterotomy. RESULTS: (l) Choledocholithiasis was confirmed in 45 patients: Thirteen patients had nondilated common bile ducts and 20 patients had stones with diamerer < 1 cm. (2) Endoscopic ultrasonography was more sensitive(95.5%) than abdominal ultrasonography (44.4%: P<0.0001) and similar to endoscopic retrograde cholangiography (97.8%). Specificity was same as 100% in 3 diagnostic tools. (3) Endoscopic ultrasonography was superior to abdommal ultrasonography for the diagnosis of choledocholithiasis with nondilated common bile duct(100% vs 15.3%, P<0.0001) and with less than 1cm sized small stone(100% vs 10.0%, P<0.0001). Endoscopic ultrasonography results did not depend on stone diameter or common bile duct dilatation. CONCLUSIONS: Endoscopic ultrasonography could be an accurate and safe diagnostic tool for the diagnosis of choledocholithiasis and could be promising especially in the cases with difficult cannulation of bile duct and gal1 stone pancreatitis.

10.Hemangiopericytoma of the lacrimal sac.

Hemangiopericytomas are soft tissue tumors composed of pericapillary pericytes. These tumors are rare in the lacrimal sac. To the author's knowledge, only four previous cases of these tumors occurring in the lacrimal sac have been reported. A primary hemangiopericytoma of the lacrimal sac in a 34-year-old female is reported. Total excision with the surrounding tissue appears to be the treatment of choice for this tumor.
