1.Clinical Observation of Decrease of Prostatic Volume in Men with Voiding Symptoms by Means of TRUS .

PURPOSE: It has been generally known that prostatic volume is increased related to aging process and changing in hormonal environment, so lower urinary tract symptoms develops. But, our experience has revealed the decrease of prostatic volume in many cases with lower urinary tract symptoms despite aging. MATERIALS AND METHODS: The numbers of patient underwent TRUS were 301 cases. Among the patients who had visited urologic outpatient clinic from march, 1993 to march, 1996, transrectal ultrasonography of prostate was applied to patient with following conditions. 1) over 8 in IPSS with or without enlarged evidence from digital rectal examination 2) enlarged evidence from digital rectal examination 3) no symptomatic relief after 2weeks therapy with alpha blocker 4) when the patient wants more evaluation. RESULTS: Decrease of prostatic volume were shown in 66cases(22%) despite lower urinary tract symptoms. CONCLUSIONS: In our experience, many patients with lower urinary tract symptoms showed decrease of prostatic volume. So, we expect that many more cases of decrease of prostatic volume can be seen among asymptomatic cases with age. Because we can guess that prostatic volume can be influenced by racial differences and dietary habit, we also believe that further evaluation and investigation of this nature is needed.

2.Cystandenoma and Primary Cystadenocarcinoma of the Liver.

Primary cystic neoplasia of the liver is rare. We report a cystadenoma with mesenchymal stroma (CMS) and a cystadenocarcinoma, and make a review of literature with particular reference to their histogenesis. The CMS has many similarities to the ovarian mucinous cystadenoma; occurring almost exlusively in female, being lined by mucus-secreting epithelial cells, and containing dense ovarian-like stroma. These features suggest that CMS may arise from the ectopic ovarian tissue within the liver. Cystadenocarcinoma may have its origin in CMS or cystadenoma without mesenchymal stroma of CMS. Cholangiocarcinoma arising from the congenital hepatic cysts can be differentiated only when it contains benign epithelia.

3.A Clinical Observation on Total Hip Arthroplasty

Clinical observation was performed on the 84 hips of 72 patients which were replaced by total hip, who were treated at Department of Orthopedic Surgery, College of Medicine, Busan National University, from March 1978 to March 1981. The follow-up period of each case varied from 1 to 4 years. The results obtained were as follows: 1. The peak age incidence ranged from 51 to 60 years, which comprised 37.1% (23 patients) of total cases. 2. The causes of the hip diseases were; 25 cases of post-traumatic avscular necrosis of the head, which comprised highest incidence, 19 osteoarthritis, 14 primary avscular necrosis of the head, 4 ankylosing spondylitis, and 3 rheumatoirl arthritis. 3. The most commonly used prosthesis was Muller type in 64 cases (88.9%); Trapezoidal-28 in 3 cases; Ceramic-metal type in 4 cases; and modified Muller type by Japanese in 1 case. 4. Anterolateral approach was used in all cases except for the 3 cases in whom anterior approach was made. In most cases, greater trochanter osteotomy was not performed except for the 9 cases in whom operative field were poorly exposed without osteotomy. 5. Several complications developed; perforation of femur shaft and acetabulum during the operation in 1 case each, Post-operative deep seated infection in 2 cases, thrombophlebitis in 1 case, wire breakage in 2 cases, stem failure in 1 case, and femoral shaft fracture in 1 case. 6. Postoperative functional evaluation by the Merle d'Aubigne and Postel method was done; pain relief in 92.4%, mobility recovery in 71.6% and functional inprovement in 70.1% belonged to grade 5 and 6. Relief of pain is the most prominent effect of the total hip rep1acement surgery in this series.

4.Post Traumatic Radioulnar Cross Union: A Case Report

The purpose of this paper is to report the rare complicated radioulnar cross union following open reduction for fractures of the forearm bones. The case was 62 years old male who complained of swelling and pain on forearm by the car accident. Roentgenogram revealed forearm bones fracture. Atpost operative 4 months, roentgenogram noted new bone crossing between radius and ulna. Excision of new bane was performed with extraction of nail and inserted chips of muscle. Post operative course was satisfactory, showing 50 degree of range of motion (20 degree supination and 30 degree pronation) from fixed pronation position.

5.Heterotopic Enchondral Ossification in Metastatic Colonic Adenocarcinoma: A case report .

Calcification and ossification of colon cancer is frequently encountered, especially in the mucinous carcinoma. However, cartilage formation or enchondral ossification has rarely been described in human colon cancer. This report describes a case of a 59-year-old man with retroperitoneal metastasis of mucinous adenocarcinoma of colon, which showed a widespread heterotopic ossification through membranous or enchondral ossification. The ossification appeared in apposition to tumor cell nests and in the organized mucin pool. In our knowledge, this is the first case showing enchondral ossification in gastrointestinal carcinoma in Korea.

6.Histological Grade of Prostatic Adenocarcinoma.

The authors attempted to choose what has the best reproducibility and predictability for prognosis of the prostatic adenocarcinoma among four most widely used gradings methods; the Gleason's Mostofi's, Bocking and MD Anderson hospital systems. According to these gradings systems, each of two pathologists made histologic gradings of 40 consecutive prostatic adenocarcinomas which had been diagnosed with the surgically resected specimens. Correlation between the histological grades and the clinical stages was studied and a comparison was made among each system. For the comparison, the Gleason's and MDAH systems were revised as 3 grades and adjusted to the other gradings systems. In this study, MDAH grading system yielded the highest reproducibility as represented by 90% agreement, as compared with the other systems which showed 82.5~87.5% agreement. By the Gleason's, Mostofi's and Bocking's systems, 46.2%, 23.1% and 46.2% of grade 3 tumors respectively fell under the clinical stage A. On the contrary, there were no cases of grade 3 in stage A and no cases of grade 1 in stage D, by MDAH gradings system. These results suggest that MDAH gradings system is superior to the other systems in reproducibility and for predicting the biological behavior.

8.Mechanisms of HPV-16-induced Transformation in Human Epithelial Cells in Culture.

Human epithelial cell line immortalized by Ad12-SV40 hybrid virus was transfected with plasmid containing HPV-16 via calcium-phosphate method. Subsequently, 8 clonal cell lines were obtained after G418 selection. Among these clonal cells, clonal cell-4(C-4) and clonal cell-6(G-6) showed increases of tumorigenic cellular properties such as saturation density, soft agar colony formation and cell aggregation. Morphological alteration such as appearane of foci was observed on these two clones after passage 6 and 7(50 to 55 days after transfection). When clonal cells and control cells were treated with MNNG(0.01ug/ml), both C-4 and C-6 showed increases of tumorigenic cellular properties and the level of increase was much more elevated as compared to clonal cells prior to MNNG treatment. Appearance of foci formation was observed in C-4 and C-6 after passage-6. After passage-8, control cells and all clonal cells showed morphological alterations. It seems that treatment of cells containing HPV-16 DNA with MNNG increases tumorigenic properties of the cells and expedite morphological transformations. (continue)

9.Coexistence of Bullous Pemphigoid and Psoriasis: A Case Report and Review of the Literature.

There have been several reports in the literature of coexistent psoriasis and bullous pemphigoid. In most cases, the bullous pemphigoid lesions have been considered to be induced by antipsoriatic treatments. We describe a patient with psoriasis in whom bullous pemphigoid developed during psoriasis treatment, but the exact etiologic factor was not identified. Recently, some reports have suggested that an immunologic or biochemical association between the two diseases plays a role in the pathogenesis.
