1.Two Cases of Solitary Type Keratoacanthoma.

The solitary type of Koratoacanthoma is a common, rapidly growing, benign tumor with a natural history of spontaneous involution which was first described by Hutchinson in 1889. It was well known that this Keratoacanthoma is close resemblence to squamous cell carcinoma clinicallt & histopathology. The etiology of Kertoacanthoma is ucertain but it have been associated with many exogenous factors, including sun exposure, occupational exposure to heat and trauma, and tar and with autoimmune etiology and viral infection. We experienced 2 cases of soitary type of Keratoacantoma with typical clinical and histopathologic findings which presented on the pubic area and forehead but had no predisposing factors of Keratoacanthoma. The one was 66-year-old woman who had been a history of proritic, rapidly growing tumor on Mons Pubis since about 6 months ago. The another one was 40 year-old male who had been a history of asymromatic, growing tomor on forehead since about 3 months ago. In the H-E stained sections, there were horn filled invagination, dyskeratotic cell, horn pearl in the epidermis and an inflammatory infiltration in dermis. Diagnosis was cinfirmedby the clinical and histopatholigical pictures. The former was treated by electrocautery after simple excision and the latter by curretage and electrodesiccation.

2.A Study on the Effect of Topical Nonsteroidal Anti - inflammatory Drugs And Cortisosteroids on Ultraviolet Light - Induced Erythema.

lt is generally beIieved that several chemical mediators such as histamine, kinine, prustaglandin E and lysosomal enzyme are related to the pathogenesis of sunburn, but the exact mechenism of erythema is unknown. Recently, prostaglandins appear to be important mediators of ultraviolet erythema (UV-erythema) and several investigators has reported that the nonsteroidal antiinflammatory druga and corticosteroids supressed UV-erythema by inhibition of prostaglandin synthesis. This study was undertaken to evaluate the effect of topical nonsteroidaI antiinflammatary drugs and corticosteroids on UV-erythema induced by a artificial sunlamp. Nonsteroidal anti-inflammatory drugs (5% Bufexamac cream, 5% Ibuprofen cream, 5% Indomethacin cream) and corticosteroids(0.05% Fluocinonide cream, 0.5% Fluocortolone oint., 1%, Hydrocortisone cream) were applied immediately after 3~4 minimal erythema dose(MED) irradiation on back skin of 40 healthy volunteers and the delayed UV-erythema response was evaluated at 2, 4, 6, 8, 12, 24, 48 and 72 hours after irradiation, respectively. The resulta were as follows; 1) Minimal erythema dose(MED) was noted 4min. (62.5%), 3min. (25.0%), 5min. (22.5%). The averaged MED was 3. 63min. 2, MED has to be increased on site, applied nonsteroid and steroid agents topically, comparing with control site. 3) Blanching effect was detected in all cases of nonsteroid and steroid agents applicated topically, but in control site. 4) Average score in blanching effect was highest in 5% Indomethacin cream among nonsteroid agents(average score-2. 47) and 1% Hydrocortisone cream (average scoro -2. 63) among steroid agents.

3.A Case Report of 3 Clinical Types of Cutaneous Tuberculosis Simultaneously Appeared on One Person.

Tuberculosis of the skin may present clinically as plaques, ulcers, verrucous, lesions, nodules, tumors, vegetatives reactions, and cicartricial infiltration. The reaction of the host to the presence of Mycobacterium tuberculosis also varies with the extent and the degree of invasion, the virulence of the bacilli, and nonspecific factors such as age and generaI state of health. The incidence of cutaneous tuberculosis has tendency to decline in the world because of thc availability of effective antiTB drugs, elimination of infected milk herbs, and an elevation of living standards, but in Korea, cutaneous tuberculosis remained to be a significant medical problem. We experienced 21 year-old male who had a case of 3 clinical types including Lupus vulgaris, Scrofuloderma, Papulonecrotic tuberculid. Diagnosis of 3 clinical types vere established by clinical history and signs, histo pathology, tuberculin test and effective of treatment vith INH, streptomycin and literature were reviewed.

4.A Case fo Disseminated Malignant Melanoma Arising From Pigmented Nevus.

Malignant melanoma is a tumor arising within the melanocyte system of the skin, eyes and juxtacutaneous mucous membrane. Tbrough the lymphatic channel developed early metastasis and regional lymphade- nopzthy developed. Malignant melanoma may arise on apparently normal skin, in a junction nevua, rareh in a compound nevus and extremeIy rarely in an intradermaI nevus and 20% of total incidence of malignant melanoma are preceded by a nevocellular nevus. A 41 year-old male patient who had multiple, pea to walnut sized, black colored. nodules developing on entire body visited our Dermto. dept. About one year ago, solitary black colored nodule arose from preexisting pigmented nevus which had been from 20 yrs ago on Rt. ant. axillary area and then nodule remcved by wide exeision and regional lymphadenectomy at a Hosp. 2 months after surgical operation, multiple pea to walnut sized, black colored nodules has developed on entire body. There was no lymphadenopathy on entire body. In H-E & Fontana-Masson stain, numerous bizzarre large cells with hyperchromatic variable sized melanin pigments are seen in the dermis and subcutaneous tissues Mitotic figutes are present. Diagnosis was confirmed by the clinical and histological feature. The patient was treated with BCG waccination(4*10^6-8*10^6 live organisms weekly) by altemately intralesional inj, on the lesion and subcuraneous inj. on both shoulder during 6wks and has been in follow-up.

5.Clinical Analysis of Nontraumatic Prehospital Cardiac Arrest for Two Years.

BACKGROUND: Care for prehospital cardiac arrest is one of the major concerns of emergency medical services. But, in Korea, prehospital emergency medical service systems are not yet well established. We tried to offer one of the fundamental data for development of these systems. METHODS: After application of exclusion criteria, 183 patients who transferred to emergency center of our hospital after cardiac arrest in consecutive 24 months from Jan,1,1994 to Dec,31,1995 were included in this study. Retrospective review of the hospital charts of these patients was done. For statistical analysis, we divided patients to some categories. t-test or chi-square analysis was used. RESULTS: 24 patients of the 183 patients were secondary visitors(cardiac arrest was occurred during transfer from other hospitals), 159 patients were primary visitors. In the primary visitor group, only one third was ambulance visitors, and there is no statistical differences between arrest time of ambulance visitors and non-ambulance visitors(35+/-27 vs 37+/-24 min, p=NS). No organized bystander CPR was done. After arrival, 131 patients received CPR and 87 patients(66.4%) were not responded, 31 patients(23.1%) experienced transient ROSC, 13 patients(10.0%) survived until discharge, and only 2 patients(1.5%) were returned to their lives. CONCLUSION: We failed to find significant statistical survival differences between ambulance visitors and non-ambulance visitors, between presumed cardiac etiology group and non-cardiac etiology group. Survival rate was high in witnessed arrest group than unwitnessed arrest group(14.5% vs 2.1%, p=0.015).

6.Two Cases of Paget's Disease.

Pagets disease is charaterized by a unilateral sharply defined eczema with or without an underlying ductal adenocarcinoma of the female nipple which was described by James Paget on 1874. We experienced 2 cases of Pagets disease confined to the nipple & areola with or without a palpable mass. The one was 44-year-old female, who had been suffered from pea. sized, eczematous lesion and crust formation on the Lt. nipple and areola with a painful palpable mass in the Lt, breast and nipple retraction since about 3years ago(Fig. 1). The another one was 52-year-old female, who had been suffered frorn palm sized, same lesions of the former on the Lt. breast with accompanying itching sensation and without a palpable mass and nipple retraction since about 7years ago(Fig. 5). Histopathologically, there were large, round, clear staining cells with Iarge nuclei in the epidersi and an inflammatory infiltration in the dermis. Diagnosis was confirmed by the clinical and histological pictures. The former was treated by simple mastectomy and the latter by radical mastectorny.

7.Statistical Evaluation of the Disabled Patients Consulted to the Department of Dermatology During Their Admission.

A statistical evaluation was made on 248 patients who were consulted to the Department of Dermatology during the disabled patients admission in Korea Veterans Hospital from July 1, 1980 to June 30, 1982. The resuIts were as followings: l. Among the 248 disabled patients, the most common patients, who constituted 68. 8% of the total, were amputee of lower extremity (25.2%), patient of posttraumatic osteoarthritis (23. 2/) and paraplegia (20. 4%). 2. In age distribution, the most frequent age groups were 50 to 59 years, 42, 4, and the lowest age groups were 60 to 69 years, 4. 0g. 3. The 6 most common dermatoses were Eczema (36. 8%), Fungal infection (33. 6%), Drug eruption (8. 0%), Acne vulgaris (6. 4%), Urticaria and Scabies (3. 2% each). 4. In seasonal distribution, the highest incidence (32. %) occurred on winter, and the lowest incidence (14. 0%) on Spring. Eczema was prevalent during Autumn and Winter season and Fungal infection during Summer season. -countinue-

8.Two Cases of Bullous Pemphigoid Showing the Characteristics of Herpes Gestationis Autoantibodies.

Bullous pemphigoid(BP) and herpes gestationis(HG) are subepidermal bullous diseases which show clinical and immunological similarities. Both diseases show immune deposits along the basement membrane zone and their autoantibodies bind a common antigenic site within the non-collagenous stretch of the 180 kDa BPAG2 ectodomain. Besides its association with pregnancy, HG has some characteristic features that distinguish it from BP. The serum of patients with HG often contains an IgG that avidly fixes complement, and showes IgG1 subclass predominance. We report here two cases of non-pregnant young women presenting clinical and histological features of bullous pemphigoid or herpes gestationis. The immunopathology, IgG subtyping and immunoblotting studies showed that the autoantibodies in the patients were the characteristic ones of herpes gestationis. The patients might be a subtype of BP that have characteristics of autoantibodies of HG patients.

9.A clinical study on patients with porencephaly.

Porencephaly is relatively rare condition defined by an defect or a defect or cavity in the cerebrum owing to a developmental malformation or to a destructive lesion. Fory-five porencephaly patients diagnosed by Brain CT were clinically analyzed and the following results were obtained. 1) By the age group presenting initial symptoms, the peak incidence was from 1 month to below 3 years old. 2) In initial symptoms, seizure, spastic weakness, headache were showed in order of frequency. But 7 cases (15.5%) were asymptomatic. 3) The latency of diagnosis after presenting initial symptoms from the symptom onset time to 10 years. 4) The subsequent symptoms were as follows: spastic weakness, speech disturbance, gait disturbance, mental retardation, sensory loss and seizure showed independently or combined. 5) As etiologic factor, 21 cases (46.7%) were congenital, 16 cases (35.5%) were post-traumatic or post-operative and 8 cases (17.8%) were perinatal. 6) The prognosis was seen various from mild to severe. Out of 45 cases, 29 cases (64.4%) were no complications. But the prognosis in patients with post-traumatic or postoperative etiological factors was poor. With the advent of brain CT and the resultant capability of detecting structural defect and cerebral lesions responsible for epilepsy or focal neurologic signs, porencephaly was seen to be readily recongizable by CT examination. Since porencephaly is a significant contributor to the spectrum of CNS lesion and benign condition, ist recognition is important in determining prognosis and therapy.

10.Metastatic carcinoma of the thyroid to the skull: report of 2 cases

Two cases of skull metastasis from thyroid carcinoma are presented. The one case shows multiple involvement inthe skull base with sphenoid sinus, frontal bone, and rib. The other case is solitary extensive involvement to theright parietal bone. The follicular caroinoma of the thyroid shows a strong tendency toward vascualr invasion and dissemination through blood stream to the lungs, bones, liver, brain, and other distant sites. The sternum, thevetebrae, and the pelvis are the most common sites in bone metastasis of thyroid carcinoma, followed by the femur,shoulder girdle, skull, and rib. Metastatic caroinoma to the skull base, including sphenoid sinus s extremelyrare. Authors experienced recently 2 cases of metastatic carcinoma of the thyroid to the skull and report withreview of literature.
