1.Papillary Eccrine Adenoma.

We report a case of papillary eccrine adenoma occurring in a 39-year-old male who had a walnut sized, firm nodule on the right knee without subjective symptoms. Histopathologic exammination revealed a well circumscribed, non-capsulated dermal nodule consisting of multiple ducts with various sizes. These ducts were composed of double layered epithelial cells in most areas, but in some areas severe intraluminal papillation snd pseudostratification into the lumen were seen and the lumina were filled with a homogenous eosinophilic substance and partially with calcium deposition. Immunohistochemical studies with S-100 antigen and csrcinoembryonic sntigen (CEA) were also performed. Positivity for these sntigens strongly supports the hypothesis that this unusual neoplasm differentiates toward the eccrine secretory coil.

3.Spectrum Analysis of Rat EEG during Infusion of Thiopental and Ketamine.

The dose-ralated dffects of intravenous infusion of thiopental and ketamine on the rat EEG were evaluated quantitatively by spectrum analysis of EEG recorded from the rat scalp. The anesthetics were infused into jugular vein at various rates ranging from 0 to 8ug/min/g body weight, and then bipolar EEG was recorded from the rat scalp and tis spectrum were calculated by powere wpectrum analysis. the density of each bands(delta 1-3.25, theta 3.5-7.75, alpha 8-12.15, beta 1.13-17.75, beta 2.18-20.75, and beta 3.21-31.75Hz) and total density were derived from the spectrums. In visual inspection of conventional EEG, low doses of thiopental increased the amplitudes of spinles. but higher doses decreased the amplitube gradually to electrical silence with increase of infusion rates. During infusion of higher doses of ketamine, two types of EEG were identified by the spectral patterns:The one was the cases in which increases of the power density over all frequency ranges were observed, and the other was those in which marked increases of density in specific frequency were observed. In thiopental infusion, the densities of all bands were increased to peak at 1~2ug/min/g and therafter were decreased with higher rates of infusion. In ketamine infusion, the densities were increased when the infusion rate was increased. These results suggest that, by the changing patterns of the band densities dervied from spectrum analysis of EEG, not only the effect on EEG of thiopental of tetamine can be quantified but also their differences of mechanisms of action on brain be reflected.

4.Craniosynostosis : Updates in Radiologic Diagnosis.

The purpose of this article is to review imaging findings and to discuss the optimal imaging methods for craniosynostosis. The discussion of imaging findings are focused on ultrasonography, plain radiography, magnetic resonance imaging and computed tomography with 3-dimensional reconstruction. We suggest a strategy for imaging work-up for the diagnosis, treatment planning and follow-up to minimize or avoid ionized radiation exposure to children by reviewing the current literature.

5.Clinical Evaluation of Supraclavicular Block with a Mixture of Local Anesthetics ; Comparison of the Effect on Onset and Duration.

In the case of brachial plexus block, mixtures of local anesthetics can combine better features of both components, rapid onset and long duration. Combining effects may influence the onset and duration of neural blockade. Our study was undertaken in order to compare the onset time (time of injection to time of loss of pain on pin prick) and duration of analgesia (time of return of sense of pain on pin prick minus time required for onset of analgesia) of a lidocaine and bupivacaine mixture with 5 minutes interval injection of lidocaine and bupivacaine. The patients admitted to our hospital for hand or forearm operations were divided into three groups. In Group 1, 9 patients were injected with 0.5% bupivacaine 150 mg only, in Group 2, 11 patients were injected with a mixture of 29: lidocaine 200 mg and 0.5% bupivacaine 100 mg, in Group 3, 10 patients were injected with 2% lidocaine 200 mg and 5 minutes later, 0.5% bupivacaine 100 mg was injected through the same needle. Group 3 had the shortest onset time (7.2+/-0.2 minutes) with moderately long duration (9.4+/-2.4hours). Group 2 had a moderately rapid onset time (9.4+/-2.3 mintes) with the shortest duration (8.6+/-1.6 hours). Group 1 had the slowest onset time (14.8+/-4.3 minutes) with the longest duration (11.3+/-2. 4 hours). The time for analgesia to reach the C7 dermatome was the slowest in group 1 and Group 2, but in Group 3, there was no difference in the time needed to achieve analgesia in all dermatomes.

Local cutaneous reactions have been reported at injection sites of interferon therapy, but these are usually erythema or rarely induration. Skin necrosis at the injection site is rare. We describe here a patient with multiple sclerosis who presented with cutaneous necrosis at the injection sites of interferon-β. Biopsy of the necrotic lesion showed dermal vessel thrombosis and complete ischemic coagulative necrosis of epidermis and dermis.

Rhabdomyosarcoma is the most common soft tissue sarcoma occurring in children, though retroperitoneal rhabdomyosarcoma is rare. We experienced a case of embryonal rhabdomyosarcoma of the retroperitoneum in a 43-month-old child, and describe the CT , MRI and pathologic findings.

10.Prediction Model for Health-Related Quality of Life of Elderly with Chronic Diseases using Machine Learning Techniques.

OBJECTIVES: The purposes of this study were to identify the factors that affect the health-related quality of life (HRQoL) of the elderly with chronic diseases and to subsequently develop from such factors a prediction model to help identify HRQoL risk groups that require intervention. METHODS: We analyzed a set of secondary data regarding 716 individuals extracted from the Korea National Health and Nutrition Examination Survey from 2008 to 2010. The statistical package of SPSS and MATLAB were used for data analysis and development of the prediction model. The algorithms used in the study were the following: stepwise logistic regression (SLR) analysis and machine learning (ML) techniques, such as decision tree, random forest, and support vector machine methods. RESULTS: Five factors with statistical significance were identified for HRQoL in the elderly with chronic diseases: 'monthly income', 'diagnosis of chronic disease', 'depression', 'discomfort', and 'perceived health status.' The SLR analysis showed the best performance with accuracy = 0.93 and F-score = 0.49. The results of this study provide essential materials that will help formulate personalized health management strategies and develop interventions programs towards the improvement of the HRQoL for elderly people with chronic diseases. CONCLUSIONS: Our study is, to our best knowledge, the first attempt to identify the influencing factors and to apply prediction models for the HRQoL of the elderly with chronic diseases by using ML techniques as an alternative and complement to the traditional statistical approaches.
