1.A Clinical Study of Calcipotriol Ointment in the Treatment of Psoriasis Vulgaris.

BACKGROUND: Calcipotriol, a new vitamin D3 analogue, is a potent growth inhibitor for human keratinocytes and has been shown to be effective in the topical treatment of psoriasis with no major disturbances of calcium homeostasis. OBJECTIVE: An open multicenter trial was conducted to assess the efficacy, safety, and tolerability of topical calcipotriol ointment(50ug/g) twice daily in the treatment of psoriasis vulgaris. METHODS: Op patients recruited from the Department of Dermatology of 30 universities and raining hospitals in Korea, 108 patients who could be followed up at the end of study were evaluated. Efficacy, as measured by the Psoriasis Area and Severity Index(PASI), and safety were assessed at 2, 4, and 6 week. RESULTS: Reduction of PASI score was stastically significant at all time points of treatment(p<0.01). At the completion of 6 weeks of treatment, the mean PASI reduction was 76.7%. The scores of thickness and scale decrease more than that of erythema. An analysis of patient and clinician overall self-assessment at 6 weeks showed cleared, marked improvement and moderate improvement in 65.2% and 62.9% at each. Some patients developed minimal irritation of lesional or perilesional skin. Laboratory tests did not show any significant changes particularly, serum calcium levels. CONCLUSION: Calcipotriol ointment was effective as measured by PASI score and the self-assessment in patients with psoriasis vulgaris and was well tolerated.

2.Autoimmune progesterome dermatitis.

Autoimmune progesterorie dermatitis describes an eruption which is influenced by the hormonal changes of the menstrual cycle. Recurrent premenstrual eruptions occur which correspond to the postovulation rise in serum prrigesterone. A 32-year-old women feveloped cyclic hives premenstrually and followed by spontaneous regression. Intradermal skin testing with crystal progesterone produced a generalized urticarial response after eight hours. The patient was treated with systemic corticostirod and antihistamine.

3.A clinical and pathological study of Becker's nevus.

BACKGROUND: Beckers nevus is a benign lesion that may be congental or acquired, macular or papular, hairless or hypertrichotie. OBJECTIVE: Our pcrpose was to describe the clinical findings of Eeckers nevus and to establish a method for the histologic diagnosis of Beckers nevus. MEHTODS: We reviewed PF skin biopsies and their clinical finding from 28 patients with Beckers nevus during a 10 year period from January 1981 to January 1991. Results : The results were summarized as follows. A Clinical characteristics 1. Sex distribution showed of male predominence with male female a ratio of 1.5:1. Age of onset was predominent in less that 5 years old and puberty. 2. The lesions were located on the trunk(24 cases), head & neck(8 cases), extremities(6 cases), and were repsilateral in 26 cases, bilateral in 2 cases. 3. The lesions were accompanied by hair in 17 cases(61%). The shaies of the lesions were reticulated (75 %), patch(18%), mottled(7%). 4. Hairs were distributed as follows : abdomen(100%), scapula(100 %), extremities (83%), cheek(80 %), neck(67 %), chest(40%). All lesions of the face had follicular papules. 5. Associated skin diseases were : smooth muscle hamartoma(3 cases), striae distensa(2 cases), steroid acne(2 cases), pigmented nevus(2 cases), and verruca plana(1 case). B. Histopathologic characteristics 1. The lesions were accompanied with plugs in 21 cases, which were follicular, acrosyringeal, epidermal. 2. Elongation of the rete ridge and hyperpigmented basal cells were observed in the epidermis in all cases. 3. The shape of the rete ridge in Beckers nevus has its characteristic appearance, which is single, bridging and reticulated. Single, bridging and reticulated shapes cexisted in 17 cases(61%). 4. Inflammatiry cell infiltrations were present in all of the upper drmis, in which melanophages(21 %), hair follicles(89%) and smooth muscle hamartoma(3cases) w ere observed CONCLUSION: These findings indicated that the shape of the rete ridge in Beckers nevus has its characteristic appesrance and can be useful for the diagnosis of Beckers nevus.

4.Tumor of Follicular Infundibulum.

We report a case of a tumor of the follicular infundibulum in a 54-year-old woman who had a papule on the medial side of the right upper eyelid for four months. Histopathologic findings were characterized by proliferation of follicular infundibular epithelium in the form of a thin subepidermal plate and small hair follicles in the tumor plate.

6.The Effect of Ultraviolet Irradiation on the Morphological Changes in Epidermal Keratinocytes.

A total of 40 adult black-mice was used and divided into two groups for expeiment. Group A was irradiated by UVB only and Group B had SPF 15 sunscreen a.pplied to the back followed by irradiation by UVB. Each group was divided again into 5 subgroups according to the days of UVB irradiation frcm 2 to 10 days. A Waldmann combination UVA+UVB Radiation Treatment Cabin 8001 was used as the light source and the UVB dosage was 50 mJ/cm2 daily. Skin specimens were taken 24 hours after the last irradiation. Histologic changes in epidermis were reviewed by the light microscope. In group A, the characteristic sunburn cells(SBC) were observed with 100 mJ/ cm2. SBC number was maximum with 400 mJ/cm2. The other epidermal changes were parakerat.osis, crusts, atypical cells, and mitoses of basal cells, which showed graded responses to the UVB doses. Pretreatment with the sunscreen completely prevented these changes.

7.An Unusual Case of Transepidermal Elimination of Calcinosis Cutis.

A 45 year-old Korean woman presented with multiple pruritic umbilicated papules on both ankles of 3 months duration. Histopathologic findings showed calcium deposits in the upper dermis and the transepidermal elimination of calcium. This is a very rare presentation of calcinosis cutis.

8.A Case of Guttate Morphea with Secondary Cutaneous Mucinosis.

The cutaneous mucinose are a group of connective disorders characterized by the deposition of mucin in the dermis. Thc disease may be a primary (metabolic) or secondary (catabolic). Secondary mucinosis is common in lupus erythematosus and dermatomyositis but rare in morphea. We report a case of guttate morphea with secondary cutaneous mucinosis.

9.Multiple Subcutaneous Granuloma Pyogenicum Combined with Cherry Angioma.

A 51-year-old man had multiple nonspecific subcutaneous nodules and small red hemorrhagic papules on his trunk. Histopathologically, the subcutaneous nodules were shown to be lobular capillary hemangiomas with edematous stroma throughout the entire subcutaneous fat. Multiple subcutaneous granuloma pyogenicum is a rare variant of granuloma pyogenicum. The addition of cherry angioma in the same patient also makes it a very unusual case.

10.Hereditary Hemorrhagic Telangiectasia in a Family.

We report three cases of hereditary hemorrhagic telangiectasia in one family. The punctiform, spider-like and nodular telangieectatic lesions were presented in these all patients, the father, son an daughter. But nevus flammeus-like lesion which was the unusual manifestation of hereditary hemorrhagic telangiectasia was noticed on daughters chest. There were no abnormal laboratory findings and no evidence of systemic disease. Histological findings showed only dilated telangiectatic blood vessels in the upper dermis.
