1.Total Micrografting Hair Transplantation Using a New Hair Implanter.

BACKGROUND: Hair transplantation is a continuously evolving era towards a more natural style and micrografting in greater quantities is becoming an unmistakable trend. OBJECTIVE: A rapid, simple method to make a micrograft implanter with low cost and a case of total micrografting using this new hair implanter are presented. METHODS: We devised a simple method to make a micrograft hair implanter using a spinal tap-ping needle and did total micrografting with this device 3 times on 1 patient. RESULTS: Cosmetically, hair transplantation using our device produces hair that is slightly less dense in a normal individual. However, our technique gives a good balance in distribution of hair with no scarring which contribute to a natural style appearance. CONCLUSIONS: Total Micrografting with our new implanter produces a natural appearance without the artificial look and scars typical of the minigrafts or punch grafts in Koreans. So we consider this device as a another alternative device in hair transplantation.

4.Supracondylar Osteotomy in Cubitus Varus and Cubitus Valgus

Cubitus varus and cubitus valgus is one of late complications of elbow fracture. Tardy ulnar nerve palsy often accompanies with cubitus valgus. The authors reviewed 40 supracondylar osteotomies (on cubitus varus 31, cubitus valgus 9) performed at the Department of Orthopedic Surgery, Seoul National University Hospital, during the period of 15 years, from 1962 till 1976. During long term follow up, correction of elbow deformity and its maintenance is possible even in growing child. So early operation is recommended for the prevention of neurologic compIication and elbow stiffness.

5.A Clinical Study of Baseball Player's Elbow in Adolescents

The popularity of baseball has resulted in an increase in medical attention directed toward participating players. This study focuses on injuries of the throwing arm in adolescents which occur during participation in organized youth baseball matches. History taking, physical examination and roentgenographic study of both elbows were made on 67 adolescent baseball players who visited the Department of Orthopaedic Surgery of Hanyang Vniversity Hospital from December, 1981 to January, 1982. The results obtained were as follows: 1. The average age was 12.4 years and average career playing baseball was 1 year 11 months. 2. There were 18(85.7%) overhand type pitchers. This was the most common pitching style. 3. 7(10.4%) complained of elbow pain. In all cases the pain was located on the medial aspect of the elbow. 4, On physical examination, hypertrophy of the forearm was seen in 19(28.4%), hypertrophy of the arm in 27(40.3%), limb length discrepancy in 28(41.8%), cubitus valgus deformity in 2(3.0%), and flexion contracture of the elbow in 6(9.0%) cases. 5. On roentgenographic study, cortico-trabecular hypertrophy of the humerus, and separation and fragmentation of the medial humeral epicondyle were the common findings. 6. The pain and flexion contracture of the elbow were more marked in pitchers. 7. To prevent these injuries of the elbow, the following recommandations were made by authors. Use a pitching machine in batting exercise. Restrict the number of innings pitched per game, Abolish curve ball throwing in this age group. Ensue an adequate warmup period before exercise or games. Inform parents and coaches of the possible harmful nature of excessive throwing.

6.Renovascular Hypertension in Children.

The application of fine needle aspiration (FNA) cytology to the soft tissue tumors had been neglected. In recent years, however, FNA has been used increasingly in the preoperative diagnosis of these tumors due to its usefulness and accuracy. We present 3 cases of liposarcoma, myxoid, myxoid with round cell, and pleomorphic, diagnosed by FNA cytology with histologic confirmation. Good correlation between his- tologic and FNA cytologic findings was found. Although the cytologic appearances of liposarcomas varied with histologic type, the main criterion was the presence of atypical multivacuolated lipoblast with characteristically scalloped nuclei.

8.Russell Body - Containing Plasma Cell Aggregations Mimiccking Signet Ring Cell Carcinoma of the Stomach.

This report presents a case with gastric ulcer whose gastric mucosal biopsy revealed heavy aggregations of Russell body containing plasma cells in the lamina propria and in peptic detritus. A sixty four year old female received an endoscopic examination for currently aggravated peptic ulcer symytoms, It revealed a small round ulcer with nodular margin along the greater curvature of high body, suggesting of early gastric carcinoma. Endoscopic biopsies (two times) confirmed heavy collections of Russell body-containing plasma cells which were once misinterpreted as signet ring cell carcinoma. Immonohistochemically they were strongly positive to both kappa and lambda light chains by pgtroxidase-antiperoxidase method. Each infiltrated cell contained one or more refractile intracytoplasmic eosinophilic globules, bcing associated with extracellular ones and heavy matare lympho-plasma cell infiltration. Differential diagnosis from neoplastic signet ring cells in endoscopic biopsy was diseussed.

9.Primary Malignant Lymphoma of the Gallbladder: A case of report.

Lymphomatous involvement of the gallbladder is rarely identified and manifests mostly as a local involvement of disseminated disease. There have been reported only about 20 primary malignant lymphomas of the gallbladder without involvement of other sites. A 63-year-old woman presented with jaundice, epigastric pain, and weight olss of two months duration. There was no systemic lymphadenopathy elsewhere. Abdominal computed tomograph revealed a focal thickening of the gallbladder wall. The removed gallbladder contained a well circumscribed, 5x3 cm-sized, slightly elevated firm area with accentuated mucosal papillary excrescences near the neck protion. The lesion involved whole thickness of the wall, and was made up of diffuse atypical lymphoid cell infiltrates of variable size accompanying plasmacytoid differentiation in areas. Some large tumor cells were pleomorphic and contained prominent nucleoli. To our knowledge, this is the first case of primary malignant lymphoma of the gallbladder in Korean literature.

10.A Study on Peripheral Blood T and B Lymphocytes in Leprosy.

In leprosy, it is generally believed that the defense against Mycobacterium leprae (M. leprae) is largely dependent on cell-mediated immunity (CMI) and there is an accumulating evidence that the patients with lepromatous leprosy have not only defective CMI specific for M. leprae, but also have generalized impairment of CMI. In contrast, the humoral immune reactivity, both specific and nonspecific, have been found to be normal or slightly increased. Recently, several investigators have enumerated the peripheral blood T and B lympocytes in patients with leprosy; however, there is no agreement as to the results. Furthermore, leprosy has been found to differ in its clinicai manifestations and immunological behaviors in different geographic areas and races. Nevertheless, there is yet no report in Korea concerning the T and B lymphocyte enumeration. The authors considered it is necessary to enumerate the peripheral blood T and B lymphocytes in Korean patients with leprosy to evaluate the immune status and the differences in the immunologic reactivity between the lepromatous and the tuberculoid spectrum of leprosy.
