2.A Study on the Roentgenographic Measurement of the Pelvis and Hip in Koreans

An appreciation of the range of normal variation and normal values for the pelvis and hip remains a continuing problem for the orthopedic surgeon in Korea. To help overcome this problem and to provide a comprehensive single source for this information, I have undertaken a study of 1037 unselected Korean subjects (605 normal males and 432 females) who varied in age from newborn to 86 years. The present study is concerned with normal angles and distances of the pelvis and hip, examined roentgenographically for statistical survey from January 1965 to August 1971 at Severance Hospital in Korea. The results of this study are as follows: 1. The value of the C-E angle, distance h, and distance d is increased according to increment of age but the value of the femoral neck shaft angle, acetabular angle, iliac index and interval of symphysis pubis is decreased with age. 2. The acetabular index in all age groups, iliac angle in the 1–3 months group, C-E angle in the 6–10 months group and the adult group over 18, interval of the symphysis pubis in the 4–22 year group, distance h in the 3–6 months group and the 4–8 year group, and distance d in the 1–3 months group show a difference between males and females. 3. There are differences between normal females and pregnant women in the interval of the symphysis pubis. Also the degree of relaxation of joints of the pelvis is higher in primigravida than multigravida. 4. The value for the femoral neck shaft angle is higher in Koreans than Europeans.

3.The Incidence of Scoliosis in Korea Part II : The Incidence of Scoliosis in the Middle and High School Male Students

Early detection of spine deformities by school screening has proven to be valuable for early diagnosis and prevention of severe deformities. Authors screened 1,620 middle and high school male students Seoul who were chosen arbitrarily, to determine the incidence rates of scoliosis. Authors diagnosed scoliosis on a combination of at least one positive physical sign and a lateral curvature more thar 5 degrees on an antero-posterior standing X-ray. According to this criteria 12 students were diagnosed as having scoliosis. They were analyzed for positive physical signs, etiology, degree of curvature. distribution of size of curvature, patterns of curvature, direction of curvation, and following results were obtained. 1. The overall incidence of scoliosis was 0.74%. 2. Idiopathic scoliosis was found in 10 students, congenital scoliosis in 1, and paralytic scoliosis in 1. 3. Rib humps, whose right to left ratio was 5, were found in 6 students, lumbar humps, whose right to left ratio was 0. 2. in 6 students, and shoulder elevation, whose direction were all to right, in 4 students. 4. Close observations were required for all the 12 students and treatment was required in 2 students who had more than 20 degrees, one for Milwaukee brace, and the other for surgical correction. 5. The most common pattern of curvature was thoracic curve. 6. in single curve right to left ratio was 2.7.

4.The Effect of Tibial Lengthening on the Muscle: A comparison between the tibialis anterior and gastrocnemius muscle.

In the limb lengthening by gradual distraction techniques, permanent tissue damage and joint contracture or subluxation can occur if the bone is lengthened beyond a certain safety limit. The main cause of those problems seems to be the difference in the reaction to distraction between bone and muscle. The purpose of this study was to investigate the difference between the tibialis anterior, a monarticular muscle and extensor of ankle joint, and gastrocnemius, a biarticular muscle and flexor of knee and ankle joint, in their response to bone lengthening in relation to the percentage of lengthening of the tibia by callotasis. Fifteen growing rabbits were divided into three lengthening groups; group I (10% lengthening), group II (20% lengthening), and group III (30% lengthening). The lengthening was done on the left tibiae and the right tibiae were used as a control. Length of the muscle belly, length of the tendon, and weight of the muscle were measured. Histopathologic studies by hematoxylin eosin and Masson trichrome stain were done on the mid-portion of muscle belly and distal musculotendinous junction of each muscle. Scoring system based on five parameters (atrophy of muscle fibers, the internalization of nuclei of muscle fibers, the degeneration of muscle fibers, the regeneration of muscle fibers, and the endomysial fibrosis of muscle fibers) was utilized for semi-quantitative analysis of histopathologic study. The following results were obtained.; 1. The length of muscle belly significantly increased in the experimental side compared to the control in group II, group III of tibialis anterior and in group III of gastrocnemius (P<0.05). 2. The length of tendon did not significantly increase in the experimental side compared to the control in any group of tibialis anterior and gastrocnemius (P>0.05). 3. The weight of muscle did not significantly increase in the experimental side compared to the control in any group of tibialis anterior and gastrocnemius (P>0.05). 4. The score of any 5 parameters did not significantly increase in the experimental side compared to the control in the mid-portion of muscle belly and musculotendinous junction of tibialis anterior and gastrocnemius (P>0.05). 5. The sum of 5 parameters did not significantly increase in the experimental side compared to the control in the mid-portion of muscle belly and musculotendinous junction of group I of either tibialis anterior or gastrocnemius (P>0.05), but significantly increased in the mid-portion of muscle belly and musculotendinous junction in group I and group II of both tibialis anterior and gastrocnemius (P<0.05). The results suggest that significant histopathologic changes can occur beyond 20% lengthening in tibialis anterior and gastrocnemius, and that gastrocnemius is less compliant to distraction than tibialis anterior.

Spontaneous Infarction of fibroadenoma of the breast is very uncommon and may lead to difficulties in clinical and pathological diagnosis. Most reported cases occured in young women during pregnancy or lactation. This report describes imprint cytologic features of an infarcted fibroadenoma in a 19-year-old young woman without evidence of pregnancy. The smears revealed many individually scattered degenerated or necrotic epithelial or spindle stromal cells and naked nuclei on dirty necrotic background. A few sheets of cohesive uniform epithelial cells and a few fragments of stromal cells were also present. Most of the epithelial cells had pyknotic and hyperchromatic nuclei, however, cellular atypism such as pleomorphism, prominent nucleoli or mitosis were not present. Though the necrotic ductular and glandular outline of this case may bear a superficial resemblance to adenocarcinoma, obvious cytologic atypia or mitosis, even in the necrotic areas, were not present.

In our hospital, we experienced a case of left solitary cystic kidney. She was sixty years old woman. Her chief complaints were dull pain on left upper abdomen, terminal dysuria. tenesmus. and frequency. Cyst was situated to lower pole of kidney. Cyst was dissected from renal parenchyma. It contained about 250cc serous and clear cystic fluid, Post-operative general condition was good and her chief complaints were disappeared almostly.

7.Tumor of Follicular Infundibulum.

We report a case of a tumor of the follicular infundibulum in a 54-year-old woman who had a papule on the medial side of the right upper eyelid for four months. Histopathologic findings were characterized by proliferation of follicular infundibular epithelium in the form of a thin subepidermal plate and small hair follicles in the tumor plate.

9.Successful Use of Extracorporeal Membrane Oxygenation in Diffuse Alveolar Hemorrhage Secondary to Systemic Lupus Erythematosus.

Diffuse alveolar hemorrhage (DAH) is an uncommon complication in patients with systemic lupus erythematosus (SLE), and mortality remains high. In recent years, cases of DAH due to SLE treated with extracorporeal membrane oxygenation (ECMO) have rarely been reported. The authors present a case of a 43-year-old woman with SLE who had rapidly aggravating dyspnea and hemoptysis. She was diagnosed as having DAH with refractory respiratory failure and was successfully managed with veno-venous ECMO. We propose ECMO as a useful salvage therapy in patients with alveolar hemorrhage secondary to SLE who are failing conventional ventilatory support.
