1.The Degree of Bronchial Mucosal Damages Related to the Bronchial Blocker Cuff Pressure of Univent(R) Tube.

Background: Univent(R) tube was designed to overcome the disadvantages of double lumen endotracheal tube for one lung anesthesia. But overinflation of the blocker cuff of an endobronchial tube can cause pressure damage to the bronchus. The purpose of this study was to evaluate whether the blocker cuff pressure and the duration of ballooning using the Univent(R) tube is correlated with the degree of bronchial mucosal damage (BMD). Methods: A total of 13 men and 7 women intubated with the Univent(R) tube were enrolled for the study. The BMD were evaluated by direct visualization using fiberoptic bronchoscopy prior to intubation and following extubation and the degree of the BMD were scored from 0 to 4 as follows; 0: normal, 1: erythema, 2: swelling, 3: hemorrhage, 4: mucosal wall tearing. Results: No change in bronchial mucosa (score 0) were observed in 11 patients (55%), erythema (score 1) in 5 patients (25%), swelling (score 2) in 3 patients (15%), hemorrhage (score 3) in 1 patient (5%) and mucosal wall tearing (score 4) is none in 20 patients. The bronchial blocker cuff pressure required to "just sealing" the bronchus was 178.1+/-37.4 mmHg with corresponding cuff volume of 6.7+/-1.0 cc. The duration of ballooning was 115.5+/-26.4 min. The correlation coefficient between the blocker cuff pressure and duration of ballooning to the degree of bronchial mucosal damage were 0.125 and 0.137, respectively, which was not statistically significant. Conclusions: The bronchial blocker of Univent(R) tube doesn't cause any severe BMD and the degree of BMD are correlated with neither the bronchial blocker cuff pressure nor duration of ballooning of Univent(R) tube.

3.A Clinical Study on Trochanteric Fractures of the Femur

The increased incidence of the trochanteric fracture of the femur was accompanied by the development of the vehicle and the prolongation of the average life-span. Recently it has been also increased in an active person from thirties to forties. 87 cases of the trochanteric fracture of the femur in 86 patients who have been admitted and treated at the Department of Orthopedic Surgery of Hanyang University Hospital during 8 years and 10 months, from May, 1972 to February, 1981, were reviewed. The results of this study were as follows: 1. The trochanteric fracture of the femur was more common in male and was frequent in a person who was from thirties to forties and who had vigorous social activity. 2. An injury from traffic accident was the most common cause of the fracture from thirties to forties and an injury from slip down was most common in seventies. 3. The unstable fracture which was classified by Evans' classification, Boyd and Griffin Type II Fx. and Tronzo Type III Fx. were most common in the intertrochanteric fracture and the Fielding Type II Fx. was most common in the subtrochanteric fracture of the femur. 4. Other injuries were accompanied by the trochanteric fracture of the femur in 38 patients (44.2%) and the pelvic bone fracture was the most common associated injury in these patients. 5. The weight bearing was allowed earlier in the patient who had been given the surgical Tx. than in the patient who had been given the conservative Tx. 6. In adults, there was no significant differences in the duration to achieve the bony union between the conservative treatment and surgical treatment performed. 7. We considered that the diminution of the probable complications by early weight bearing after accurate open reduction in accordance with the Type of the Fx. followed by secure internal fixation with devices is an ideal method of the Tx.

4.Roentgenographic signs of massive ascites in the double-contrasted stomach

There are many established roentgen signs of ascites such as hepatic angle sign, generalized graying of the abdomen, frog belly appearance and Hellmer's sign. These signs are easily recognized in the standard flat abdomen film. It is however hardly possible to recognize such signs in small films of double contrasted upper GI series. By a retrospectroscopical observation we were able to find some interesting signs of massive ascites in small size films of double contrasted stomach and duodenum of upper GI series. The clinical materials consisted of 27patients with massive ascites and 30 normal subjects. The signs we observed were ;(1) Constriction deformity of the junction of the fundus and body of the stomach. We named this "waist" sign. This constriction was attended by convergence of mucosal folds. We called this "converging folds" sign. (2) The fundus assumed electric bulbappearance with its long axis directed vertically. We called this "electric bulb" sign. In normal subjects the fundus assumed beret-cap like appearance. These signs were only appreciated in the supine and RAO views and not in other views. Of these new signs of massive ascites where fundic view was obtained in supine or RAO position.

5.Partial Purification and Characterization of 41 kDa Serine Proteinase from Culture Filtrate of Trichophyton tonsurans.

Dermatophytes infect the human hair, skin, nail and cause the dermatophytosis. The extracellular and intracellular proteinases of the dermatophytes commonly occur in the genus Trichophyton like T. rubrum, T. mentagrophytes, and T. granulosum. These enzymes play a prominent role in growth, multiplication and infection of the host tissue. Extracellular proteinases have been purified from the species of Trichophyton and Microsporum. We purified the proteinase partially from the culture filtrate of the Trichophyton tonsurans through Mono-Q and Superose 12 column and investigated its biochemical and enzymatic characters. The molecular size of the proteinase was estimated to be 41 kDa by SDS-PAGE. And pI was 3.2. The optimal temperature and pH for an enzymatic activity were 27C and 7.5, respectively. The purified porteinase degraded the keratin, bovine serum albumin, hemoglobin. The serine proteinase inhibitor like PMSF and DFP inhibited the proteolytic activity of the purified enzyme whereas the cysteinase inhibitor did not. These results demonstrated that the purified proteinase is a serine proteinase and can contribute the tissue invasion.

6.Three cases of epithelial skin cancer treated with high energy electron beam.

Higt energy electron beam therapy is a method which is used for the treatment of superficial tumors (less than 5 cm deep) with a characteristically sharp drop-off in dose beyond the tumor. This method offers distinct advantages in dose uniformity and in minimizing the dose to deeper tissues. We report herein three cases of epithelial skin cancer treated with high energy electron beam. The first patient was a 79-year-old male who had primary basal cell carcinoma(BCC) on the right lateral canthus. The second patient was a 67-year-old male who had recurreiit BCC on the right cheek. Both of them received electron beam therapy on the lesion and there were no clinical relapse signs over 1 year. The third patient was a 46-year-old male who had squamous cells, carcinoma on the lower lip. He also received electron beam therapy on the lesion, but it recurred.

8.Blocking Action of Nicardipine on alpha1-Adrenoceptors in Circulatory System of Rabbit.

To investigate relationship between alpha1-acrenoceptors and nicardipine, an 1,4-dihydropyridine calcium antagonist, effects of nicardipine, on phenylephrinne(PE)-induced vasoconstriction in isolated arterial rings and pressor response of rabbits were observed. In normal physiological salt solution(NPSS), 35mM KCI produced persistent contractions of thoracic aorta and carotid artery and the contractions were dose-dependently inhoboted by cumulative admini-stration of nicardipine in the range of doses from 10(-10)M to 10(-4)M, IC50s of nicardipine in the thoracic aorta and carotid artery were 3.3x10(-7)M and 4.6x10(-7)M, respectively and there was no difference between both values. Constant contractions induced by 10(-5)M PE in both rings were inhibited by the same doses of nicardipine in a dose-dependent fashion. The IC50s were 2.8x10(-4)M in thoracic aorta and 2.9x10(-5)M in carotid artery respectively, and the former was about 10 times greater than the latter. In Ca2+ free PSS, KCI did not produce any contractionn. Though constant and reproducible in NPSS, PE-induced contraction was transient and not reproducible in Ca2+ free PSS. The contraction in both rings were weakened to about 70% of those in NPSS. Pretreatment with nicardipine in the range of doses from 10-8M to 10-5M hardly affected the PE-induced contraction and the largest dose 10(-4)M slightly inhibited the contraction. Intravenous injection of nicardipine 10 to 1000microg/kg decreased blood pressure and heart rate of rabbits in a dose-dependent manner. The % decrease of heart rate was much smaller in comparison with the %decrease of blood presseure. Pressor effect of 30microg/kg PE was dose-dependently inhibited after treatment with nicardipine 10 to 1000microg/kg and ID50 was 314microg/kg(6.1x10(-7)mole/kg). Above results suggest that nicardipine blocks extracellulr Ca2+ influx by membrane depolarization and in a part by alpha1-adrenceptors, then relaxes arterial smooth muscles in rabbits.

9.An innovative nipple reconstruction technique for minimizing postoperative scars: The teardrop flap

Nipple reconstruction methods include various techniques, such as the local flap technique, free nipple grafting, and filler injection. The local flap technique can provide less donor site morbidity than a free nipple graft, but leaves an additional scar near the nipple. We present a novel method for reconstruction of the nipple using a flap located on one side of the nipple. The flap has a teardrop shape consisting of a circle and two wings folded to one side. The two wings form a pillar and cap, and the de-epithelialized tip of one wing fills the internal dead space of the new nipple. We applied this nipple reconstruction technique in the case of a 61-year-old patient who had a vertical scar due to inverted-T reduction mammoplasty. The patient had lost her nipple in previous breastconserving surgery. The immediate postoperative nipple projection was 10 mm. At the 7-month follow-up visit, the nipple projection was 7.5 mm. The teardrop flap is an innovative technique that leaves no additional scar by using the scar already present on one side of the nipple.
10.Two Cases Report of Cleidocranial Dysostosis

The cleidocranial dysostosis is relatively rare congenital and familial disorder with autosomal dominant inheritance. After the first report of a case of this was that of Cutter in 1870, which was quoted from Soule, A., in 1946, Marie and Sainton supplied the name of the Cleidocranial dysostosis with the report of 2 cases of this in 1897. Since that time approximately 350 cases have been described in the literature. This condition is characterized by deficient formation of the clavicle, delayed and imperfect ossification of the cranium and less often involvement of other bones, We experienced 2 cases of this and reported them in this paper with a brief review of the literatures.
