1.Difficulties and overcoming strategies experiencedby immigrant medical interpreters

Introduction
The objective of this study was to explore experiences of immigrant medical interpreters working in Japanese hospitals and their view on how to improve medical interpreting services.
Methods
We conducted semi-structured interviews to five Brazilian and Argentinean interpreters working at two hospitals in Aichi Prefecture. The interviews explore their experiences since they started working as an interpreter focusing on difficulties and strategies to overcome them. We also asked how they saw medical interpreter system in Japan and how it can be improved. The interviews were recorded and transcribed. Descriptions related to difficulties and overcoming strategies were extracted from the transcription, coded and summarized into subcategories, then, to categories.
Results
Forty subcategories were extracted concerning difficulties and strategies immigrant medical interpreters experienced. They were integrated into eight categories. Their experiences were described as follows (« »shows categories). Immigrant medical interpreters started their career with«Groping due to lack of relevant experiences and training», therefore, they experienced«Initial difficulties and hardship». They devised various«Initial strategies to overcome difficulties»and improved their skills. Then they faced«Bafflement and dilemma as experienced». They struggle to bring«Change and contrivance as experienced»to themselves and to their colleagues. As they overcame serious difficulties, they gradually equipped with«Concern and consideration as a medical interpreter». Through experiences as an interpreter, they found out«Meaning of being an interpreter», and started«Taking on a new role»beyond what were assigned to them.
Conclusion
This study portrayed immigrant medical interpreters’ experiences on difficulties they faced as well as strategies to overcome them. It is important to enhance social recognition of medical interpreters and establish medical interpreter system in order to improve access of immigrants to health care.
2.Impact of Meals for Nutritional Support in Patients with Cancer

Nutritional support for patients who develop cancer is necessary to increase their food intake, which is often insufficient due to the adverse effects of anticancer drugs and radiation treatment. In December 2011, our center introduced Bocchiri meals for cancer patients to improve their quality of life and support ongoing treatment. In this study, we examined the efficacy of Bocchiri meals, which were developed to help increase the amount of food intake during cancer treatment. Target patients were those who could eat half or less than half of their daily required food intake, averaged over 3 days.Patients could order the meal 2 h before serving time and it was served at the temperature and amount that individual patients requested.We evaluated age, sex, main disease, use of anticancer drugs, radiation treatment, time taken to eat the Bocchiri meals, number of orders made, average energy intake before and after eating the Bocchiri meals, and rate of eating a Bocchiri Meal with a side menu order. Average energy intake was increased from 226.6±14.8 kcal with regular meals to 294.1±15.3 kcal with Bocchiri meals. Furthermore, a strong correlation was seen after the Bocchiri meals were introduced between the rate of eating Bocchiri meals with a side menu order and average energy intake. These results suggest that Bocchiri meals may be an effective supportive nutritional measure to enhance food intake among cancer patients.
3.Pregnancy outcome following first trimester exposure to Terbinafine Hydrochloride : A case series survey

Objective: Treatment of tinea unguium with topical drugs alone is difficult, and medication with oral terbinafine hydrochloride is necessary in many cases. Since Trychophyton infects the nails in not only men but also women at a reproductive age, the influence on a fetus of the drug ingested during early pregnancy is a matter of concern of many women. The objective of this study was to investigate the influence of oral terbinafine hydrochloride on fetuses.
Methods: The outcomes were investigated in pregnant women who ingested terbinafine hydrochloride and visited the ‘Outpatient Services for Consultation on Pregnancy and Drugs (Teratology Information Services)’ of Toranomon Hospital, and cases were collected.
Results: Fifteen pregnant women ingested the drug during the organogenic period, in which the teratogenic risk is the highest. All 15 pregnant women gave birth to healthy babies.
Conclusion: This is the first survey concerning newborns delivered by women who ingested terbinafine hydrochloride during pregnancy in Japan. All investigated women gave birth to healthy babies, and the incidence of congenital abnormalities did not markedly exceed that in the general population. Since the number of cases was small, it is necessary to perform a cohort study to investigate details of the safety for fetuses.
4.Survey regarding pregnant women receiving novel antipsychotic agents

Objective: Novel antipsychotic agents less frequently cause extrapyramidal side effects compared to conventional antipsychotic agents, contributing to improvement in the QOL. Recently, these agents have also been increasingly prescribed to females who may become pregnant. In Japan, no epidemiological survey regarding the application of novel antipsychotic agents in pregnant women has been published. In this study, we investigated the influence on fetuses in pregnant women receiving novel antipsychotic agents.
Methods: In pregnant women on novel antipsychotic agents who consulted the Pregnancy and Drug Consultation Outpatient Clinic of Toranomon Hospital, the outcome of pregnancy was confirmed.
Results: Twenty-nine pregnant women took novel antipsychotic agents in the organogenesis phase, in which the risk of teratogenicity is the highest. The agents consisted of olanzapine in 8 patients, risperidone in 11, quetiapine fumarate in 7, and perospirone hydrochloride hydrate in 4. The outcomes of pregnancy were full-term delivery in 24 patients, premature delivery in 1, spontaneous abortion in 2, and artificial abortion in 2. Of the 29 patients, 15 (60%) had continuously taken these agents until delivery. All 25 neonates were healthy without malformation.
Conclusion: This report describes the first prospective survey in Japan regarding infants delivered by pregnant women receiving novel antipsychotic agents. All patients delivered healthy neonates; the incidence of congenital anomalies did not exceed that in the general population. This survey included a small number of patients; cohort studies should be conducted to evaluate the safety in fetuses.
5.Establishment of "Fujita–style" problem–based learning with an emphasis on the use of a monitoring room to support tutors

1)We reproduced a problem–based learning (PBL) tutorial at our school and developed our own PBL tutorial, which we call "Fujita–style PBL." This is a clinical problem-solving type of PBL, in which both a monitoring room and small–group learning rooms are used.
2)To maintain the present number of PBL lessons despite the limited number of tutors, one tutor supervises several groups simultaneously. Coordinators observe the progress of PBL from a monitoring room and support the tutors.
3)Students learn the given scenario and identify their learning issues. After they study the learning issues by themselves, the students return to tutorials to explain their learning issues. Thereafter, each group's findings are presented to the groups supervised by one tutor.
