Angiogenesis is a crucial step in tumor growth and progression. Scarce data is available on angiogensis in gastrointestinal tumors. We studied 16 normal colon, 44 adenomas and 29 carcinomas to evaluate angiogenesis in colorectal tumors and to assess the correlation among p53 protein, proliferative activity and other clinical prognostic parameters. Endothelial cells were immunostained with an anti-Factor VIII mAb; in each case three microscopic fields(x 200) were counted: average number of the three fields was defined as microvessel density (MVD). p53 protein expression was 45.5%(20/44) in adenomas, and 79.3%(23/29) in carcinomas (p<0.01). p53 protein expression of carcinomas was 57.1%(4/7) in diploid tumors, 100%(8/8) in aneuploid tumors (p=0.07), 100%(8/8) in well differentiated tumors, and 50%(2/4) in poorly differentiated tumors (p=0.09). MIB-1 score was 2.3+/-0.7(38) in adenomas, 3.4+/-0.5(29) in carcinomas (p<0.01). There was no significant correlation between p53 protein and MIB-1 score. MVD was 10.4+/-4.1(16) in the normal mucosa, 21.5+/-7.9(39) in the adenomas, 35.3+/-9.7(26) in carcinomas (normal versus adenomas, p<0.01; adenomas versus carcinomas, p<0.01). MVD was 25.8+/-5.4(2) in carcinomas confined to mucosa, and 36.1+/-9.6(24) in carcinomas with transmural invasion. The higher MIB-1 score was in carcinomas the more MVD increased but there was no statistical significance (r=0.38, p=0.055). MVD of carcinomas was not associated with nodal metastasis, p53 expression, and DNA ploidy. p53 protein and MIB-1 expression are useful methods for the evaluation of malignancy, and tumor angiogenesis is an early event in a colorectal tumor but MVD does not correlate with prognostic parameters except for the tumor depth.

Intestinal metaplasia (IM) have long been thought to play a role in the pathogenesis of gastric intestinal adenocarcinoma, but not in that of diffuse cancer. We studied 20 normal gastric mucosa, 90 IM, 39 atypia (dysplasia or adenoma), and 51 adenocarcinoma to evaluate the expression of p53, erb B2, and H-ras p21 proteins and to assess the correlation with IM (esp. type III IM, revealing positive HID-AB/PAS for sulfomucin). Positive rate of HID-AB staining revealed an increased trend in comparison between IM, atypia and adenocarcinoma. It was the highest in mucinous carcinoma, but it was not correlated with positive oncoprotein expressions. Positive rates of oncoproteins revealed increased trends in comparison between IM, dysplasia or adenoma and adenocarcinoma in c-erb B2 and p53 (P<0.01). The positive rates were highest in intestinal adenocarcinoma (50.0% and 54.2%, respectively). Rates were lowest in biopsy tissue of IM (4.4% and 8.7%, respectively). The expression of H-ras p21 was not significant in gastric carcinogenesis. There was no significant correlation between oncoproteins and other clinical parameters, such as depth of invasion, differentiation, size and nodal metastasis of the tumors. Therefore, we suggest that p53 and erb B2 may play a role in the carcinogenesis of gastric intestinal adenocarcinoma.

Acute disseminated encephalomyelitis (ADEM) is a demyelinating disease of probable autoimmune etiology. The MR images of patients with clinically suspected ADEM were retrospectively reviewed. The clinical symptoms occurred 5 days to 1 month after viral upper respiratory infection (4) and Coxsakie viral infection(1). The symptoms had begun with fever(3), headache(3), sore throat(1), and drowsy mental state (1), which progressed with monophasic course to altered mental change(2), extremity weakness(2), seizure(1) and/or cerebellary symptom(I) MRI findings of ADEM showed patchy(4), nonhemorrhagic(5), asymmetric(5) high signal intensity lesions on T2-weighted images. The number of the lesions was mostly multiple(4). The lesions mainly involved the brain stem(3) and subcortical white matter(3). Follow-up MR images of 13 days to 20 days after high dose steroid therapy showed marked improvement in two of three, which well correlated with clinical manifestations. MR findin of multiple, patchy, nonhemorrhagic and asymmetric lesions in subcortical white matter and brain stem on T2-weighted images seem to be characteristic features of ADEM, but nonspecific. Therefore, clinical correlation is required in evaluating ADEM.

PURPOSE: Kimura's disease is an inflammatory condition of unknown etiology which usually occurs in salivary glands and cervical lymph nodes. This study was undertaken to evaluate the radiologic characteristics of Kimura's disease using CT and MRI. MATERIALS & METHODS: We evaluated CT & MR of 10 patients with histologically proved Kimura's disease in the head and neck. Six males and four females between 16 and 58 years old (mean age, 37.6 years) were included. Contrast enhanced CT scan was performed in all, and spin-echo MRI was performed in 2 patients. RESULTS: Both CT and MRI demonstrated the distinctive sites of involvement that were major salivary glands (4 cases) and lacrimal gland (1 case), periglandular soft tissue (8 cases), and cervical lymph nodes (8 cases). Majority of soft tissue lesions were ill-defined with infiltration to adjacent fascial planes, but nodal lesions were relatively well-defined and homogenous. The lesions were iso-intense on Tl-weighted image and hyper-intense on T2-weighted image. All but a few nodal lesions showed variable degree of contrast enhancement on CT and MRI. CONCLUSION: In diagnosis of Kimura's disease, CT & MRI of head & neck, especially salivary glands, are useful. In cases of recurrent periglandular soft tissue mass with contrast-enhancing cervical lymphadenopathy in adult patients, Kimura's disease should be included in the differential diagnosis.

This paper contains the results of clinical and neuroradiologic studies in 9 cases of histopathologicallyverified intracranial cavernous hemangiomas. The most common clinical presentations were focal seizure andheadache which were accompained by focal neurologic deficits. Skull films revealed abnormality in 3 cases andcerbral angiography revealed an avascular area in 8 cases, but both were of little diagnostic significance. CTusually demonstrated a hyperdense, cystic and calcified lesion without significant mass effect, and mild degereeof tubular or ring like enhancement after contrast injection. The follow-up CT findings in 4 cases over a 6-monthperiod were compatible to repeated minute hemorrhage and resolution. The above clinical and CT findings and , ifpossible, repeated CT could suggest the diagnosis of cavernous hemangioma and provide the proper management,including the avoidance of unnecessory radiation therapy and biopsy related complications.

Dysplasia epiphysealis multiplex is a rare syndrome, first discribed by Fairbank in 1935, but numerous publications have made it a well-recognized entity. The syndrome is caused by a congenital developmental error of unknown etiology, characterized by changes in the developing epiphyses, dwarfism and stubby digits in children of normal intelligence. Diagnosis is mainly by roentgenographic apperences of the epiphyses before fusion to the shaft. The centers of ossification of the epiphyses are late in appearing, slow in developing, deformed in shape and irregular in density. The spine is never affected, blood and biochemicaI studies show no abnormality. A case of dysplasia epiphysealis multiplex which revealed typical roentgenographic pictures is presented with a brief review of a literature.

Sixty four leprous claw hands had been operated on for intrinsic replacement extensor-extensor many tailed at the Department of Orthopedic Surgery, Kyungpook National University Hospital during the period from 1966 to 1970. The follow up study was carried out for fourty-one hands from one year to four years and nine months after operation, and evaluated by the following assessments. 1. Fully open hand assessment 2. Lumbrical position assessment 3. Hook position assessment 4. Closed fist assessment 5. Mechanics of closing 6. Separate finger movement in lumbrical position The results obtained were as follows: 1. The most of the cases were median-ulnar palsy. 2. The majority of them were in third decade young men. 3. Satisfactory result was 63.5% in appearance, 70.8% in function, 62.8% according to the patients statement, and 61.1% in overall total assessment. 4. As complication two cases of severe adhesion of the grafted tendons and significant lateral deviation on nine fingers were noted. 5. Preoperative and postoperative physiotherapy contributed a important roll in the end results.

One hundred and sixty five cases of the fracture of tibial shaft in adult have been treated and managed in the Department of Orthopedic Surgery, Kyunghook National University Hospital during the period from January, 1965 to December, 1970. Seventy one of 165 patients were analyzed clinically and we came to the following conclusions. 1. Most of the injuries were the results of traffic accident (67.6%). 2. The incidence of trauma was high in the young man, most frequent in the 3rd decade. 3. The closed fractures were three times more than open fractures and the middle third of tibial shaft was the most frequent site of fracture. 4. The average healing period was 16 weeks in mild case and 28 weeks in severe case. 5. Of 71 cases, we experienced 4 cases of non-union, 8 cases of residual joint stiffness at the knee and the ankle and 3 cases of leg shortening (more than 2 cm.) as complication.
