2.Clinical Observation of Supracondylar Fracture in Children

Supracondylar fracture is the most freqent injury of the elbow and frequently accompany complications in children. This injuries require meticulous care during first week period for the prevention of complications. Sixty cases of supracondylar fracture in children treated and managed at the Orthopedic Surgery, Taegu Presbyterian Hospital during the period from 1962 to 1970 The most cases were treated conservatively and various method of treatment were adapted according to the condition of the fracture. All obtained satisfactory bone union but six cases of them resulted gun-stock deformity (reversed carrying angle) of the elbow, Two of these underwent supracondyIar wedge osteotomy and gained satisfactory corrections.

3.Simultaneous Occurrence of Chromophobe Renal Cell Carcinoma and Urothelial Carcinoma in the Same Kidney.

The simultaneous occurrence of a renal cell carcinoma and a urothelial carcinoma in the same kidney is uncommon. Here we report the case of a 79-year-old woman with ipsilateral synchronous renal cell carcinoma and urothelial carcinoma. She was referred to our hospital for gross hematuria and right flank pain. A computed tomography scan showed a 15x20 mm enhanced lesion on the upper calyx and a 12x15 mm mass on the lateral aspect of the right kidney. We thus suspected a renal pelvis tumor and performed right hand assisted laparoscopic nephroureterectomy with bladder cuff excision (HALSNU). Gross findings were multiple, pale yellowish papillary masses on the upper and lower major calices, of which the largest one measured 16x20 mm. A separated solid mass measuring 12x16 mm was also noted on the anterior midportion of the kidney. The former was a urothelial carcinoma and the latter was a chromophobe renal cell carcinoma. We present a rare case of a chromophobe renal cell carcinoma and a urothelial carcinoma in the same kidney.

4.A Case of Facial and Axillary Lupus Miliaris Disseminatus Faciei.

Lupus miliaris disseminatus faciei (LMDF) is a papulo-pustular eruption typically affecting the face of young adults. It is characterized histologically by the presence of caseating granuloma in the dermis. The lesions are typically described to affect the periorbital and muzzle skin of the face, however, only three cases with axillary involvement have been described in the literature. We report a case of LMDF on the face and left axilla of a 44-year-old female. She presented with a 7-month history of a mildly-pruritic papulopustular eruption on the face. The histologic examination showed the homogenous necrosis surrounded by granulomatous inflammation containing epithelioid macrophages, lymphocytes, and multinucleated giant cells in the dermis. Two months later, a solitary palpable nodule developed on the left axilla and its histological features showed similarity with the eruption on the face. A diagnosis of LMDF was made based on the clinical and histological findings. We hereby report an interesting and rare case of LMDF with axillary involvement.

5.A Case of Contact Dermatitis from Henna Tattooing.

The art of body adornment by body piercing and tattooing has become increasingly popular in our modern culture. Temporary henna tattoos are readily available worldwide, last several weeks on the skin, and offer a self-limited, convenient alternative to a permanent tattoo. The addition of para-phenylenediamine (PPD), which is widely recognized as a sensitizer, increases the risk of allergic contact dermatitis from these henna tattoo mixtures, and a number of cases have been reported. We report a case of contact dermatitis from a henna tattoo mixture, which was diagnosed by the patch test and identification and quantification of PPD and nickel in a temporary henna tattoo.

6.Multiple Meningiomas: Case Report.

Multiple meningiomas are a seldom encountered neoplasm. Most authors report an incidence ranging from 1 to 3% of all meningioma prior to the introduction of computerized tomography(CT). The routine use of CT has resolved diagnostic problems involved with multiple meningiomas and the incidence was 8% since the introduction of CT scan. In fact, it may be difficult to differentiate "true multiple meningioma" from simple recurrences or the special variant(forme fruste) of Von Recklinghausen's disease, and the pathogenetic factors related to true multiple meningioma is unknown, but various theroies of the etiology are explored by literature reports. We report the rare case of multiple meningiomas which diagnosed by CT scan and confirmed by operation.

7.A Case of Androgenetic Alopecia Associated with Cushing's Disease.

Cushing's disease is an ACTH-producing pituitary corticotrope adenoma. This pituitary adenoma exhibits an unrestrained ACTH secretion resulting in hypercortisolemia. A 27-year-old female visited us with a 3-year history of diffuse hair loss on the scalp. Her past medical history included recently-developed hypertension and 10 years of increased weight gain and oligomenorrhea. Central obesity, hirsutism, acne, violet striae on the abdomen, lower and upper extremities, and christmas tree-shaped alopecia on the scalp were observed upon physical examination. The secretion of cortisol was not suppressed in both a low and high dose dexamethasone suppression test. A brain MRI detected a pituitary microadenoma in the sella turcica. Based on the clinical and laboratory findings, we diagnosed androgenetic alopecia associated with Cushing's disease. After surgical removal of the pituitary adenoma, the alopecia showed a remarkable improvement. We report a case of Cushing's disease found in a patient with androgenetic alopecia.

8.MRI of the TSH(Thyroid Stimulating Hormone) -Secreting Pituitary Adenoma.

PURPOSE: To demonstrate and evaluate the value of MRI findings of the TSH(Thyroid-Stimulating Hormone, TSH, Thyrotropin)-secreting pituitary adenoma. MATERIALS AND METHODS: The authors reviewed retrospectively the MR images of 4 patients with TSH-secreting pituiatary adenoma. Evaluation of the anatomical location, signal characteristics, enhancement patterns, size, shape and circunferential changes were made. RESULTS: No characteristic common MR findings in size, shape, signal intensity, and circumferential changes of TSH-secreting pituiatary adenoma waere observed among 4 cases(size; 5x7 mm to 10x11 mm, shape;ovoid to round, signal intensity;high in 1 case on T1 and T2WI, isosignal intensity in the other 3 cases, circumferential change; stalk deviation in 1 case, no stalk deviation in 3 cases). But, the tumors were centrally located at the anterior pituitary gland and showed relatively homogeneous signal intensity on MR images of all 4 patients. CONCLUSION: We conclude that centrally-located mass at the anterior pituitary gland with homogeneous signal intensity on MR image may be suggestive of the TSH-secreting pituitary adenoma, although the MR findings are not specific for the disease.

The kinetics of strontium metabolism in man has been extensively reviewed by Bauer and his collegues. Photoscanning of bone lesions utilizing (Sr-85) have been a recent contribution to the clinical usefulness. The authors studied 60 cases of various skeletal lesions with strontium 85 bone scanning. Increased uptake of Sr-85 was observed on the lesions of metastatic cancer, osteosarcoma, giant cell tumor, chondroblastoma, reticulum cell sarcoma, chloroma, delayed union of fracture, healing fracture, postradiation fracture, chronic osteomyelitis (including 5 infected compound fracture), chronic arthritis, aseptic necrosis of femur head, osteoarthritis of spine, ricket, osteogenesis imperfecta, hemophilic arthropathy, epiphysiodesis, and more unconfirmed diseases.
