Primary hyperparathyroidism is chronic disease which exhibits variable complex of symptoms suggestive of multiple system involvement. The authors have treated a case of primary hyperparathyroidism which was caused by parathyroid adenoma. The patient, 48 years old male, has been complained of back pain and polyarthralgia for five years. Radiologically, subperiosteal resorption along the radial margin of the middle phalanges and disappearance of lamina dura were noticed. In laboratory findings, serum calcium was increased, serum phosphate was decreased and alkaline phosphatase was markedly increased. Parathyroid adenoma was excised under the diagnosis of parathyroid adenoma. Four months after removal of parathyroid adenoma, improvement was noted in clinical feature, laboratory findings and radiologic findings of bone.

Belt injury is an increasingly serious injury and one that frequently results in undesirable disabilities. We reviewed 20 cases of belt injury which had been treated in Kyungpook National University Hospital during the five and a half year period from Auguat, 1972 to January, 1978. 1. This injury is more prevalent in males (M:F = 4:1) with the peak incidence between the second and fourth decades (85%). 2. Upper extremities are involved in almost all cases (95%). 3. Tuberculous tenosynovitis affecting flexor tendon sheaths of the hand is a rare condition and quite disabling to the patient like tuberculosis elsewhere. It should be attacked by operation and antituberculous chemotherapy, but has a tendency to recur after surgery. The authors have encountered a case of tuberculous tenosynovitis affecting flexor tendon sheaths in the palmar area of the left hand, and treated the case with surgery and anti-tuberculous chemotherapy.

In clinical practice fractures of the forearm bones are encoutered as frequently as fractures of other bones. As has been pointed out in many articles, however, the surgical anatomy of the forearm evokes problems in dealing with the diaphyseal fractures of the forearm bones not found in the treatment of diaphyseal fractures of other long bones, The authors have experienced 107 cases of diaphyseal fractures of the forearm bones during the fiveyear period from January, 1972 through December, 1976. A comparison has been made between the two groups one treated by conservative method and the other by open reduction and internal fixation. The results are as follows: 1. The time required for the healing of the fractures was shorter in the conservatively treated group. 2. Restoration of function was more satisfactory in the surgically treated group. 3. Rotational and angulatory deformities were less in the surgically treated group. 4. Forty-one fractures were internally fixed with compression plates, the union rate of which was 100%.

Tumoral calcinosis was coined by Inclan in 1943 and thereafter about 40 cases have been reported in the literature, but there has not been reported on a case which involved knee joints. The majority of cases hitherto reported were found in Negro. There was no report in yellow race. Authors presented a case of tumoral calcinosis that involved boih knee joints of a 55 years old Korean housewife. On physical examination chronic draining sinuses with spontaneous extrusion of calcific material was noted on the left infrapatellar region. A radiograph of the both knees showed dense, multinodular deposits of calcium. The masses were confined to the soft tissues; the contiguous joint showed no pathological change. En-block excisianal biopsy was done. The histological section of the excised mass disclosed the foreign-body giant cell with inflamed connective tissue surrounding calcific deposits.

Injury of the tarsometatarsal joints is known to be very uncommon. Injury was most frequently seen as the result of high energy trauma and appear to be occuring more frequently. Early recognition was imperative, since significant long-term disability can result from inadequate treatment. We have reviewed and clinically anafysed 14 cases of tarsometatarsal Fracture-Dislocation treated at our clinic during the period frrqn March 1980 to March 1985. The following result were obtained. 1. Majority of cause of injury was traffic accidents in 10 cases(71%). 2. According to the Wilsons classification, First stage of supination was most common. 3. Associated injuried was 92%. 4. There was no significant difference between operative treatment and conservative treatment.

We report a 58-year-old man who developed synchronous gastric non-Hodgkin`s lymphoma (NHL) and renal cell carcinoma. He presented with epigastric discomfort for 2 months. Endoscopic finding of the stomach disclosed a large inegular ulceration with nodular margin on the upper body. Constrast enhanced CT scan of the abdomen showed an ulceration and focal wall thickening in the greater curvature side of stomach, and an enhanced bulging mass in the left kidney incidentally. The tissue obtained by radical proximal gastrectomy and nephrectomy showed diffuse large B-cell lymphoma on stomach and chromophobic type of renal cell carcinoma on kidney. To our knowledge, this is the first report of synchronous gastric NHL and renal cell carcinoma in Korea.

Here we report an autopsy case of diabetic ketoacidosis (DKA) with severe hypertriglyceridemia (12,900 mg/dl). A 29-year-old woman with a history of type 1 diabetes was found dead at a motel. There was no injury on external inspection, but a lump of cheese-like material was noted in the heart at autopsy and peripheral blood plasma had a creamy appearance. After postmortem biochemical analysis, we made a diagnosis of DKA with severe hypertriglyceridemia and concluded that these unusual autopsy findings were caused by DKA and postmortem change. Uncontrolled diabetes often causes DKA and hypertriglyceridemia. To the best of our knowledge, this is the first report in Korea of DKA with severe hypertriglyceridemia diagnosed by autopsy.

OBJECTIVES AND METHODS: The presence of aneuploidy or high proliferative activity in cytologic specimens is considered as complementary for the diagnosis of malignancy. To evaluate the diagnostic usefulness of DNA ploidy and cell cycle analysis in lung cancer, we compared the diagnostic yielding rates of DNA ploidy test by brushing specimens using flow cytometry with bronchoscopic forceps biopsy and brushing cytology. RESULTS: Of the seventy-six cases, 55 cases proved to have malignant diseases(squamous cell cancer: 27, adenocarcinorna: 7, large cell cancer: 1, undifferentiated: 4 and small cell cancer: 16). The incidence of aneuploidy in lung cancer..patients was 32.y %(18/55), as opposed to no cases in benign disease. And the proportion of high proliferative activity(S+GEM>22%) in lung cancer patients was 42.9% (15/35), but none in benign diseases. In (iffy-six of 75 cases(74.7%), cytology of brushing specimens and DNA analysis(either aneuploidy or high proliferative activity vs. diploidy and low proliferative activity) were in concordance. The sensitivity with only brushing cytology was 41.8%(23/55), but with the addition of DNA analysis, it was increased to 56.4%(31/55), without decreasing the specificity(100%). And there was a case whose clue for malignancy was absent except aneuploidy, and he was confirmed to have squamous cell cancer following open thoracotomy There were no differences in the frequency of aneuploidy or high proliferative activity between histologic subtypes of bronchogenic malignancy. CONCLUSIONS: The diagnostic detection rate of lung cancer was improved with the addition of DNA ploidy and cell cycle analysis, and the presence of aneuploidy or high proliferative activity was a relatively specific indicator of malignant disease. It would be useful to test DNA ploidy and cell cycle analysis with brushing specimen for the diagnosis of bronchogenic malignancy particularly in patients whose biopsy specimen could not be obtainable.

The crush syndrome was first defined as a clinical entity in 1941. The term myorenal syndrome is preferred to the term crush injury or crush syndrome. The syndrome occurs in sjambok, deep burns and crush injuries. Crush injury is caused by continuous prolonged pressure on the limb. The main injury is to muscles of the limbs. Crush syndrome, which is the general manifestation of crush injury, has been accompainment of crushing injuries following disaster situations and war. However, we report a case of crush syndrome after physical assaults.
