3.Two Case Reports of Bone-invading Basal Cell Carcinoma.

Basal cell carcinoma is the most common skin cancer, especially on the eyelid and nose. As it rarely invades to a underlying bone or metastasizes an distant site, and is usually found at an early stage, it is regarded as a curative disease. However, basal cell carcinoma on the eyelid and nose may be resected incompletely due to efforts to preserve important structures or as a result of esthetic considerations. We experienced two cases of basal cell carcinoma with local invasion to underlying bone. One was a recurred case on the nasal area extending to the nasal bone arts medial wall of the ethmoid sinus. The lesion was widely resected and covered with a radial forearm free flap. The other was on the eyelid extending to the orbit. It was treated with orbital exenteration and resection of the involved eyelid. The defect was reconstructed with the temporalis muscle flap with split-thickness skin graft. These patients were followed up for 7 months with no evidence of recurrence. Since basal cell carcinoma can invade to the bone and metastasize to a distance site, it should be resected radically in the regions of the eyelid and nose.

5.Consideration of Prognostic Factors in Hypoglycemic Encephalopathy.

BACKGROUND: Recently, there are many reports about the association of Diffusion Weighted Imaging (DWI) and the prognosis of hypoglycemic encephalopathy (HE), but those relationships have not yet been completely determined. As such, we researched for prognosis, according to a variety of clinical data, and the lesion's distribution on DWI. METHODS: We retrospectively reviewed 19 patients who were diagnosed as HE. In addition, those prognoses were analyzed by a variety of clinical data and characteristics of lesion's distribution, which were evaluated on DWI and Apparent Diffusion Coefficient (ADC) maps. Three months later, those prognoses were determined by each Modified Rankin Scale. Further, the time-dependent average Glasgow Coma Scale (GCS), among the groups according to the characteristics of lesion's distributions in the initial DWI, was estimated. RESULTS: In this study, the difference of prognosis was not shown, according to all the clinical data, such as the severity or duration of the hypoglycemic state, but the group that did not have any pathologic lesion on the initial DWI demonstrated a better prognosis, in comparison with the groups-that exhibited any positive lesion on the initial DWI (p = 0.006). The group that had a focal pathologic lesion on the initial DWI showed a better prognosis than the diffuse lesion's group (p = 0.045). The groups with no lesion or focal lesion showed a faster recovery of GCS than the other groups with a positive lesion or diffuse lesion within the initial 1 week. CONCLUSIONS: We can identify that the characteristics of the lesion's distribution of DWI can be more helpful to predict of prognosis in HE than a variety of clinical data, such as the severity or duration of the hypoglycemic state.

7.A Survey of Health Consumers' Attitude of Personnel Health Management Service Using PHR.

OBJECTIVE: This study was conducted to investigate public opinion regarding personal health management service using Personal Health Record (PHR) in Korea. Specifically, the relationships between consumer's demographic characteristics, need for health information, consumer's benefit and concern, type of health information, type of management and security of patient information using PHR and attitude to PHR were explored. METHODS: A survey was conducted, using questionnaire developed by the study team based on literature review. Structured questionnaires were completed through a telephone survey for 715 consumers. RESULTS: A key finding was the significant difference for customer's attitude towards using PHR by customer's characteristics. Regarding factors affecting attitudes of customers with higher concern about their health, considering authorization level to access their private information and using reliable method to certificate were more likely to prefer PHR system positively. CONCLUSION: Public opinion is reflected in policy of health information. Meeting consumers' needs sufficiently, the goal of health information service to improve consumers' accessibility to and satisfaction will archive successfully.

The color stability of current prosthetic composites is unknown, even though the quality of com-posite materials has been improving. This study examined the intrinsic color stability of various current prosthetic resins (ceramic-polymers) after an accelerated aging process and the extrinsic color stability after immersion in a coffee solution. By comparing the amount of dis-coloration after aging with that without aging, the effect of the accelerated aging process on external discoloration could be evaluated. Three current prosthetic composites (Artglass, Targis, Sculpture), one light polymerized direct composite (Z100) and one dental porcelain control (Ceramco) were assessed. The col-or changes (delta E) of all the specimens were determined using the CIE L*a*b* color order system with a reflected spectrophotometer. The results were as follows: 1. The prosthetic composite materials subjected to the accelerated aging test showed no sig-nificant difference in color changes (p> .05). 2. In the coffee solution immersion test after the aging process, the color changes of the Targis and Artglass groups were not different from that of the Z100 group, which showed the highest color change. 3. In the immersion only test, a significantly high color change was observed in the sculpture glazing group. 4. The aging process influenced on the color changes more in the Targis, Artglass and Z100 groups than in the Sculpture and Ceramco groups.
