4.Correlation between Chromosome Abnormalities and Genomic Imprinting in Developing Human 2) Independent Expression of Imprinted Genes, H19 and Insulin-like growth factor II (IGF2), in Androgenetic Hydatidiform Moles.

Human uniparental gestations such as androgenetic hydatidiform moles provide a model to evaluate the integrity of parent-specific gene expression,-i.e, genomic imprinting,- in the absence of a complementary parental genetic contribution. Several imprinted genes are characterized so far including the insulin-like growth factor-2 gene (IGF2) coding for a fetal growth factor and the Hl9 gene whose normal function is unknown but which is likely to act as an untranslated mRNA for its tumor-suppressing function. IGF2 is expressed exclusively from the paternal allele while Hl9 from the maternal allele. Such an alternate expression is quite interesting because both Hl9 and IGF2 genes are located close to each other on chromosome 11p15.5. An in situ hybridization analysis has shown strong expression of Hl9 and IGF2 alleles in nine hydatidiform moles. Especially, a prominent expression of Hl9 and IGF2 was detected in cytotrophoblast and the cellular localization was almost paralleled in Hl9 and IGF2 transcripts . Hl9 and IGF2 genes could be expressed either biallelically or monoallelically in the moles. However, IGF2 biallelic expression did not affect allele-specificity of Hl9 expression.. These results suggest that both H19 and IGF2 transcripts are expressed in the same cells and that the functional imprinting of H19 and IGF2 genes in hydatidiform moles can be controlled individually and independently of each other.

6.Obesity and Perioperative Morbidity after Total Hip Arthroplasty in Osteoarthritis Patients.

PURPOSE: To evaluate the effect of obesity on perioperative morbidity following total hip arthroplasty in patients with severe osteoarthritis. MATERIALS AND METHODS: Between April 1987 and April 2007, 272 patients with severe osteoarthritis underwent total hip arthroplasty. One hundred seventeen patients were obese (body mass index, BMI> or =25 kg/m2), and 155 were not obese (BMI<25 kg/m2). Through retrospective medical record research we were able to determine length of hospital stay, operation time, total blood loss and replacement, and surgical and medical complication rates for the two groups. RESULTS: The obese and non-obese groups were similar in terms of length of hospital stay, operation time, total blood loss and replacement, and surgical complication rate (p>0.05). However, the medical complication rate was significantly higher in the obese group compared to the non-obese group (4.3% vs. 0.6%, p<0.005). CONCLUSION: In severe osteoarthritis patients undergoing total hip arthroplasty, obesity (BMI> or =25 kg/m2) has no observable effect on perioperative morbidity in terms of length of hospital stay, operation time, total blood loss and replacement, or surgical complication rate, but is associated with a higher medical complication rate.

7.A Case of Cerebral Malaria.

Presented a 39 year old man of cerebral malaria and pertinent literature were reviewed briefly. The patient was admitted because of fever and drowsy mental state. On admission, there were mild icteric sclerae and hepatomegaly. Brain C-T scan was normal. The cerebrospinal fluid study showed increased pressure (280 mmH20), and protein content (150 mg/dl) with mild pleocytosis. The EEG revealed diffuse delta waves throughout the head. At the 3rd hospital day, the patient was rapidly deteriorated to comatous state showing marked hyperbilirubinemia, thrombocytopenia and bleeding tendency and expired. Numerous plasmodium falciparum parasites were revealed in the peripheral blood smear.

8.Cementless Total Knee Arthroplasty with Posterior Cruciate Retention in Rheumatoid Arthritis.

PURPOSE: The result of cementless total knee arthroplasty with posterior cruciate ligament retention in rheumatoid arthritis patients was evaluated. MATERIALS AND METHODS: Thirty patients (fifty knees) followed up after cementless total knee arthroplasty with posterior cruciate ligament retention was evaluated clinically and radiologically. Average follow up was 9.2 years(5.1~14.3). RESULTS: The mean HSS knee score improved from 47.4 preoperatively to 81.8 postoperatively, and the mean range of motion of 89.7 degrees preoperatively to 101.2 degrees postoperatively. The mean tibiofemoral angle was 4.9 degrees valgus. Posterior instability was observed in 1 case(2%) at the most recent follow up. Radiolucent line was observed in 41 cases(82%), and osteolysis was observed around tibial component in 13 cases(26%), and around femoral component in 4 cases(8%). Eight cases(16%) of revision was due to 1 case of aseptic loosening and osteolysis each, and the other due to polyethylene wear of patellar or tibial component. CONCLUSION: Although there are osteopenia in rheumatoid arthritis compared to osteoarthritis, cementless total knee replacement with posterior cruciate retention showed stable fixation. However, the new method for initial fixation of tibial component is needed because osteolysis around tibial component tended to start around cancellous screws.

9.The inhibitory effects of recombinant plasminogen kringle 1-3 on the neovascularization of rabbit cornea induced by angiogenin, bFGF, and VEGF.

Angiostatin is a potent angiogenesis inhibitor that is composed of the first four kringles of plasminogen fragment. Angiostatin with one less kringle molecule (kringle 1 to 3) was recently demonstrated to be an effective angiogenic inhibitor. To determine whether recombinant plasminogen kringle 1-3 (rPK1-3) can inhibit the corneal neovascularization induced by potent angiogenic factors; angiogenin, bFGF, or VEGF, hydron polymer discs each containing 2.0 microg of angiogenin, 500 ng of bFGF, or 500 ng of VEGF respectively were implanted into the corneal stroma of 138 rabbit eyes, and then discs each containing 10 microg, 12.5 microg, 20 microg or 30 microg of rPK1-3 were implanted randomly. Discs containing phosphate buffered saline were also implanted as a control. The angiogenesis score on number and length of newly formed vessels on the each of the rabbit's cornea were recorded daily by two observers (blinded). The treated corneas were also examined histologically. Recombinant PK1-3 treated corneas showed less neovascularization induced by all angiogenic factors (p < 0.05). and the extent of inhibition of neovascularization was proportional to the concentration of rPK1-3 (p < 0.05). Histologic examination showed leukocyte infiltration into the corneal stroma on the PBS treated eyes whereas rPK1-3 treated eyes showed only traces of leukocytes. These results of the effective rPK1-3 inhibition of corneal neovascularization induced by angiogenin, bFGF, or VEGF suggest that this angiostatin related fragment, rPK1-3, may be useful in the treatment of various neovascular diseases. 10.A comparison of postoperative pain in patients undergoing liver transplantation, donor hepatectomy, and a major hepatectomy for a tumor.

BACKGROUND: Because liver transplantation (LT) essentially accompanies the deterioration, total absence, and recovery of hepatic function, and donor hepatectomy (DH) is performed in otherwise healthy individuals, we planned to compare the postoperative pain in patients undergoing LT, DH, and a major hepatectomy (MH) for a tumor. METHODS: Postoperatively, all patients were permitted to administer patient-controlled fentanyl with a standard dosing regimen. Breakthrough pain was treated with a 25microgram fentanyl bolus. Verbal rating scale (VRS) pain scores at rest (static) and during coughing (dynamic), total fentanyl consumption, and side effects were assessed at 2, 4, 6, 12, 24, and 48 h postoperatively. RESULTS: Thirty patients (11, 10, and 9 in the MH, DH, and LT groups, respectively) were included in the study. The LT group showed a lower static pain VRS score 2, 4, 6, and 12 h postoperatively, and a lower dynamic pain VRS score 12, 24, and 48 h postoperatively than the MH group. Total fentanyl consumption was significantly less in the LT group than the other two groups throughout the study period. Postoperative nausea and vomiting were reduced significantly in the LT group than in the other two groups. No significant differences in postoperative fentanyl requirement, or static and dynamic pain, existed between the MH and DH groups. CONCLUSIONS: LT patients experienced less static pain during the early postoperative period and less dynamic pain during the late postoperative period than MH patients. DH patients had similar pain intensity and postoperative fentanyl requirements to the MH patients.
