1.A study on surface of various abutment screws.

STATEMENT OF PROBLEM: Regardless of any restoration, most of case, we used in screw connection between abutment and implant. For this reason, implant screw loosening has been remained problem in restorative practices. PURPOSE: The purpose of this study was to compare surface of coated/plated screw with titanium and gold alloy screw and to evaluate physical property of coated/plated material after scratch test in FESEM investigation. MATERIAL AND METHODS: GoldTite, titanium screw provided by 3i (Implant Innovation, USA) and TorqTite, titanium screw by Steri-Oss (Nobel Biocare, USA) and gold screw, titanium screw by AVANA (Osstem Implant, korea) - were selected for this study. Each abutment screw surface was observed at 100 times, and then screw crest, root, and slope were done more detailed numerical value, at 1000 times with FESEM. A micro-diamond needle was also prepared for the scratch test. Each abutment screw was fixed, micro-diamond scratch the surface of head region was made at constant load and then was observed central region and periphery of fine trace through 1000 times with FESEM. RESULTS: The surface of GoldTite was smoother than that of other kinds of screw and had abundant ductility and malleability compared with titanium and gold screw. The scratch test also showed that teflon particles were exfoliated easily in screw coated with teflon. Titanium screw had a rough surface and low ductility. CONCLUSION: It was recommended that the clinical use of gold-plated screw would prevent a screw from loosening.

2.Correlation between Chromosome Abnormalities and Genomic Imprinting in Developing Human 2) Independent Expression of Imprinted Genes, H19 and Insulin-like growth factor II (IGF2), in Androgenetic Hydatidiform Moles.

Human uniparental gestations such as androgenetic hydatidiform moles provide a model to evaluate the integrity of parent-specific gene expression,-i.e, genomic imprinting,- in the absence of a complementary parental genetic contribution. Several imprinted genes are characterized so far including the insulin-like growth factor-2 gene (IGF2) coding for a fetal growth factor and the Hl9 gene whose normal function is unknown but which is likely to act as an untranslated mRNA for its tumor-suppressing function. IGF2 is expressed exclusively from the paternal allele while Hl9 from the maternal allele. Such an alternate expression is quite interesting because both Hl9 and IGF2 genes are located close to each other on chromosome 11p15.5. An in situ hybridization analysis has shown strong expression of Hl9 and IGF2 alleles in nine hydatidiform moles. Especially, a prominent expression of Hl9 and IGF2 was detected in cytotrophoblast and the cellular localization was almost paralleled in Hl9 and IGF2 transcripts . Hl9 and IGF2 genes could be expressed either biallelically or monoallelically in the moles. However, IGF2 biallelic expression did not affect allele-specificity of Hl9 expression.. These results suggest that both H19 and IGF2 transcripts are expressed in the same cells and that the functional imprinting of H19 and IGF2 genes in hydatidiform moles can be controlled individually and independently of each other.

3.Mumps-induced Opsoclonus-Myoclonus Syndrome: A Case Report.

Opsoclonus-myoclonus syndrome (OMS) is a rare neurologic disorder characterized by progressive opsoclonus (irregular, rapid, horizontal and vertical eye movements), myoclonus, cerebellar dysfunction and severe hypotonia. Here we present the case of a 19-year-old man with OMS induced by mumps virus infection. Emergency physicians should know about the typical presentation of OMS and make a proper response.

4.Gamma Knife Radiosurgery in Recurrent Trigeminal Neuralgia after Other Procedures

OBJECTIVES: Trigeminal neuralgia (TN) is undurable paroxysmal pain in the distribution of the fifth cranial nerve. Invasive treatment modalities for TN include microvascular decompression (MVD) and percutaneous procedures, such as, radiofrequency rhizotomy (RFR). Gamma Knife radiosurgery (GKRS) is a considerable option for patients with pain recurrence after an initial procedure. This study was undertaken to analyze the effects of gamma knife radiosurgery in recurrent TN after other procedures. METHODS: Eleven recurrent TN patients after other procedures underwent GKRS in our hospital from September 2004 to August 2016. Seven patients had previously undergone MVD alone, two underwent MVD with partial sensory rhizotomy (PSR), and two underwent RFR. Mean patient age was 60.5 years. We retrospectively analyzed patient's characteristics, clinical results, sites, and divisions of pain. Outcomes were evaluated using the Visual Analog Scales (VAS) score. RESULTS: Right sides were more prevalent than left sides (7:4). The most common distribution of pain was V1 + V2 division (n = 5) following V2 + V3 (n = 3), V2 (n = 2), and V1 + V2 + V3 (n = 1) division. Median GKRS dose was 80 Gy and the mean interval between the prior treatment and GKRS was 74.45 months. The final outcomes of subsequent GKRS were satisfactory in most cases, and at 12 months postoperatively ten patients (90.0%) had a VAS score of ≤ 3. CONCLUSIONS: In this study, the clinical result of GKRS was satisfactory. Invasive procedures, such as, MVD, RFR are initially effective in TN patients, but GKRS provides a safe and satisfactory treatment modality for those who recurred after prior invasive treatments.

5.Role of IGF2 Gene in Developing Human Ovary.

To understand the role of IGF2 gene in development of human ovary, IGF2 expression was detected by monoclonal antibody for IGF2 to its producted protein with immunohistochemical technique on human ovarian tissues. The results was as follows. IGF2 was highly expressed in ovum of mature follicle, IGF2 expression, however, was not high in granulosa and the cells. IGF2 was not highly expressed in ovum of primary follicle. Highly expressed IGF2 was found on corpus luteum and no expression of IGF2 was found in stroma and epithelial cells. These results suggest that IGF2 is important role in ovulation and in production of progesterone. Abnormal IGF2 expression may be concerned to carcinogenesis of ovarian tumor because most of all tumor from ovary is originated from epithelium.

6.Quantitative CT of Lung in Healthy Koreans :Evaluation with PULMO-CT.

PURPOSE: Spirometric gating quantitative CT, PULMO-CT, is an objective method for the measurement of pulmonary parenchymal attenuation at a constant level of inspiration. In order to obtain a normal value for quantitative CT of the lung, the authors therefore used PULMO-CT to evalvate the lung density of healthy koreans with different respiratiory status. MATERIALS AND METHODS: Twenty healthy Korean volunteers, 18 men and 2 women, ranging in age from 24 to 27 years and with normal chest radiography, were evaluated using the PULMO-CT option of Somatom Plus(Siemens, Erlangen, Germany). Spirometric gating HRCT images, at levels of 50% and 20% of vital capacity, were obtained at the level of the tracheal carina and at 5cm above and below this point. The images were analyzed by semiautomatic programs and the results were evaluated using the Student t-test. RESULTS: The mean attenuation value of lung parenchyma at 50% of vital capacity was -791HU27.1 and at 20% was -700HU+/-42.9. The difference in lung attenuation was 91HU and was statistically significant(p<0.001). The for 58% of participants, the highest reading for of lung attenuation at 50% of vital capacity was between -899HU and -800HU, and for 20.3% of participants, this reading was between -799HU and -700HU. At 20% of vital capacity, the largest proportion of participants(43.8%) had a reading of between -799HU and -700HU; the reading of 35.2% was higher than -699HU. CONCLUSION: Respiratory status significantly affects the quantitative accessment of pulmonary parenchyma; in order to determine, during quantitative HRCT, the parameters of a pathologic condition, lung parenchyma of differing respiratory status must be evaluated.

7.A Case of Spontaneous Bacterial Peritonitis as the Presenting Feature in a Patient with Nephrotic Syndrome.

Although spontaneous bacterial peritonitis is a frequent complication in the childhood nephrotic syndrome, it is very rare in adults with nephrotic syndrome. It frequently develops when the patients are either in relapse or receiving steroid therapy at the time peritonitis is diagnosed. We report an unusual case of a spontaneous bacterial peritonitis as the presenting feature in a 15-year-old male patient with nephrotic syndrome. He presented with diffuse abdominal pain and distension for 15 days. Abdominal paracentesis revealed the diagnostic laboratory findings of peritonitis, and the bacterial culture of the ascites showed a mixed growth of Escherichia coli and Pseudomonas aeruzinosa. His serum albu- min level was 1.6gldL and the amount of 24 hours proteinuria was 21.0g/day. Although he was treated with adequate antibiotics for 3 weeks, the peritonitis was more aggravated. We decided to insert a catheter into the peritoneal cavity for continuous drainage of the intractable ascites. Two weeks after drainage, the peritonitis improved as the peritonitis subsided, the proteinuria disappeared completely without a steroid therapy. Six months after spontaneous remission, the proteinuria have recurred, and the kidney biopsy then showed focal segmental glomerulorsclerosis.

8.Vesicoureteral Reflux Secondary to a Simple Diverticulectomy: A Case Report.

A case of congenital periureteral bladder diverticulum is described. Bladder outflow obstruction and urinary tract infection were the presenting symptoms of a 2 year old boy. There were two diverticula in the bladder. The one was small (1 x 2cm) and located just above the right ureteral orifice and the other was large (8 x 10cm) and located at the left trigone. There was no hydronephrosis, and vesicoureteral reflux, neither. A simple diverticulectomy was performed. The left ureteral orifice was found at the medial side of the diverticulum's neck during operation. During follow up, bilateral high grade vesicoureteral reflux were discovered and urinary tract infections developed frequently. Eventually, ureteral reimplantation was done one year after the diverticulectomy.

9.Usefulness of the NMP22BladderChek Test for Screening and Follow-up of Bladder Cancer.

PURPOSE: We evaluated the usefulness of the nuclear matrix protein 22 BladderChek (NMP22BC) test for the screening and follow-up of bladder cancer. MATERIALS AND METHODS: From February 2006 to September 2009, we enrolled 1,070 patients who had hematuria or who were being followed up for bladder cancer. We compared the sensitivity and specificity of the NMP22BC test with those of urine cytology. RESULTS: The sensitivity of the NMP22BC test (77.5%) was significantly higher than that of urine cytology (46.3%). The specificity of the NMP22BC test was 88.8%, compared with 97.9% for urine cytology. The sensitivity of the NMP22BC test (81.8%) in non-muscle-invasive bladder cancer was higher than that of cytology (36.4%). However, the sensitivity of the NMP22BC test and of urine cytology in invasive bladder cancer were 57.1% and 92.9%, respectively. The sensitivity of the NMP22BC test was higher for low-grade bladder cancer (83.9%) than for high-grade (62.5%), and the sensitivity of cytology was higher for high-grade bladder cancer (66.7%) than for low-grade (37.5%). Follow-up bladder cancer was detected in 262 patients. The sensitivity of the NMP22BC test in that group (72.7%) was decreased and the specificity (91.7%) was increased. The sensitivity of cytology (54.5%) in the follow-up group was increased and the specificity (95.6%) was decreased. The presence of pyuria was significantly associated with the lower specificity of the NMP22BC test. CONCLUSIONS: The greater sensitivity of the NMP22BC test may be more useful for the diagnosis of non-muscle-invasive bladder cancer and low-grade bladder cancer than for the diagnosis of invasive or high-grade bladder cancer. If the NMP22BC test is performed in the absence of pyuria, it may play a compensatory role for urine cytology.

10.Hydrogenosomal activity of Trichomonas vaginalis cultivated under different iron conditions.

To evaluate whether iron concentration in TYM medium influence on hydrogenosomal enzyme gene expression and hydrogenosomal membrane potential of Trichomonas vaginalis, trophozoites were cultivated in irondepleted, normal and iron-supplemented TYM media. The mRNA of hydrogenosomal enzymes, such as pyruvate ferredoxin oxidoreductase (PFOR), hydrogenase, ferredoxin and malic enzyme, was increased with iron concentrations in T. vaginalis culture media, measured by RT-PCR. Hydrogenosomal membrane potentials measured with DiOC6 also showed similar tendency, e.g. T. vaginalis cultivated in iron-depleted and iron-supplemented media for 3 days showed a significantly reduced and enhanced hydrogenosomal membrane potential compared with that of normal TYM media, respectively. Therefore, it is suggested that iron may regulate hydrogenosomal activity through hydrogenosomal enzyme expression and hydrogenosomal membrane potential.
