PURPOSE: To evaluate the clinical outcomes of implantable collamer lens (ICL) implantation with simultaneous full thickness astigmatic keratotomy (FTAK) for the correction of moderate to high myopic astigmatism. METHODS: Thirty-two eyes of 16 patients who had an ICL implantation with simultaneous FTAK were studied. Follow-up visits were at 1 week, 1 month, and 3 months. The outcome measures included the uncorrected distance visual acuity (UDVA), refractive error, efficacy, safety, and predictability. RESULTS: After the surgery, astigmatism was reduced by 74.83 ± 13.8%. The proportion of eyes with a spherical equivalent of 0.5 D or less was 87.5%, and all eyes had a spherical equivalent of 1.0 D or less at 3 months after the surgery. The proportion of eyes with a UDVA of 20/25 or better was 100%, and 20/20 or better was 81.25%. Reoperation was needed in one case (3.1%) because of undercorrection of the astigmatism, and no complications were observed. CONCLUSIONS: This study showed that ICL implantation with simultaneous FTAK is effective and safe for the correction of moderate to high myopic astigmatism.

The purpose of this study was to examine the influence of 12-week combined exercise program on carotid artery structure and function, and vascular endothelial growth factor (VEGF) in obese older women. All subjects were sixty years or older (66.90±4.2 years), performed the combined exercise training during 12-week consisting of aerobic exercise, band exercise, and yoga exercise for 70 minutes 3 times a week under the supervision of exercise specialist. Despite no statistically significant change in control group, percent body fat mass (3.26 %, p<.01), systolic blood pressure (6.2 mmHg, p<.05), diastolic blood pressure (5.6 mmHg, p<.001), and 10 m maximal walking time (0.56 sec, p<.05), TC (20.5 mg/dl, p<.05), and LDL-C (22.16 mg/dl, p<.05) were significantly decreased respectively after 12-week combined exercise in exercise group. In addition, sit-and-reach (3.6 cm, p<.01), oxygen uptake per weight (3.27 ml/kg/min, p<.05), VEGF (17.85 pg/dl, p<.001), and carotid artery LD (0.4 cm, p<.01), PFV (10.06 cm/sec, p<.05), EFV (6.04 cm/sec, p<.05) were significantly increased in the exercise group than in the control group. The VEGF had the significant correlation with LD (r=.389, p<.01), PFV (r=.427, p<.01), EFV (r=.264, p<.05). In conclusion, 12-week combined exercise program is effective to improve and/or physical function and body composition. And also, exercise can improve serum lipid metabolism, VEGF regulation, and carotid artery function and structure.
4.Deep Learning in Dental Radiographic Imaging
Journal of Korean Academy of Pediatric Dentistry 2024;51(1):1-10
Deep learning algorithms are becoming more prevalent in dental research because they are utilized in everyday activities. However, dental researchers and clinicians find it challenging to interpret deep learning studies. This review aimed to provide an overview of the general concept of deep learning and current deep learning research in dental radiographic image analysis. In addition, the process of implementing deep learning research is described. Deep-learning-based algorithmic models perform well in classification, object detection, and segmentation tasks, making it possible to automatically diagnose oral lesions and anatomical structures. The deep learning model can enhance the decision-making process for researchers and clinicians. This review may be useful to dental researchers who are currently evaluating and assessing deep learning studies in the field of dentistry.

PURPOSE: To compare the clinical outcome of combined small incision lenticule extraction and collagen cross-linking (SMILE Xtra) with SMILE. METHODS: This study included 30 eyes from 15 patients who had undergone SMILE Xtra and a random sample of 30 eyes from 15 patients receiving SMILE alone during the same period. We obtained the following parameters from all patients: uncorrected (UDVA) and corrected distance visual acuity (CDVA), spherical equivalent (SE), efficacy and safety index, and corneal high-order aberrations. RESULTS: The SMILE Xtra group had higher preoperative SE and thinner central corneal and residual stromal bed thickness and optic zone diameter compared to the control group (p < 0.001). At 6 months, there was no significant difference in UDVA or CDVA between the two groups. The efficacy indices were 0.97 ± 0.16 and 1.05 ± 0.17 in the SMILE Xtra and control groups, respectively (p = 0.044), and there was no significant difference in safety index between the two groups during the follow-up period. Total corneal high-order aberrations numbered 2.59 ± 0.56 and 2.02 ± 0.41 in the SMILE Xtra and control groups, respectively (p < 0.001), and there was significant increase in spherical aberration and horizontal corneal aberration in both groups compared to preoperative results. Corneal haze was observed in 20% of eyes in the SMILE Xtra group, and no complication such as corneal ectasia was observed during the follow-up period. CONCLUSIONS: SMILE Xtra had good early clinical outcome compared to SMILE alone. It appears that SMILE Xtra can be a good modality when the cornea is thin or SE is high. However, postoperative corneal haze should be considered.

This study aimed to evaluate the effectiveness of deep convolutional neural networks (CNNs) for diagnosis of interproximal caries in pediatric intraoral radiographs. A total of 500 intraoral radiographic images of first and second primary molars were used for the study. A CNN model (Resnet 50) was applied for the detection of proximal caries. The diagnostic accuracy, sensitivity, specificity, receiver operating characteristic (ROC) curve, and area under ROC curve (AUC) were calculated on the test dataset. The diagnostic accuracy was 0.84, sensitivity was 0.74, and specificity was 0.94. The trained CNN algorithm achieved AUC of 0.86. The diagnostic CNN model for pediatric intraoral radiographs showed good performance with high accuracy. Deep learning can assist dentists in diagnosis of proximal caries lesions in pediatric intraoral radiographs.

The purpose of this in vivo study was to assess the clinical screening performance of a quantitative light-induced fluorescence (QLF) device in detecting proximal caries in primary molars. Fluorescence loss, red autofluorescence and a simplified QLF score for proximal caries (QS-proximal) were evaluated for their validity in detecting proximal caries in primary molars compared to bitewing radiography.
Three hundred and forty-four primary molar surfaces were included in the study. Carious lesions were scored according to lesion severity assessed by visual-tactile and radiographic examinations. The QLF images were analyzed for two quantitative parameters, fluorescence loss and red autofluorescence, as well as for QS-proximal. For both quantitative parameters and QS-proximal, the sensitivity, specificity and area under receiver operating curve (AUROC) were calculated as a function of the radiographic scoring index at enamel and dentin caries levels.
Both quantitative parameters showed fair AUROC values for detecting dentine level caries (△F = 0.794, △R = 0.750). QS-proximal showed higher AUROC values (0.757 - 0.769) than that of visual-tactile scores (0.653) in detecting dentine level caries.
The QLF device showed fair screening performance in detecting proximal caries in primary molars compared to bitewing radiography.

Journal of Stroke 2016;18(2):220-226
BACKGROUND AND PURPOSE: We aimed to investigate the usefulness of combining transcranial magnetic stimulation (TMS) and diffusion tensor tractography (DTT) to evaluate corticospinal tract (CST) integrity and subsequently predict ambulatory function after middle cerebral artery (MCA) stroke. METHODS: Forty-three patients with first MCA stroke underwent TMS and DTT to evaluate CST integrity. Patients were classified into four groups according to the presence of motor-evoked potentials (MEPs) obtained from the tibialis anterior muscle and CST integrity. Motor impairment and functional status were assessed using the Fugl-Meyer Assessment, Functional Ambulation Category, and Korean modified Barthel Index, both at the time of admission and after 4 weeks of rehabilitation. RESULTS: Patients with the presence of both measurable MEPs and a preserved CST showed better motor recovery and ambulatory function than other groups at the 4-week follow-up. Intact CSTs were not visualized in patients without detectable MEPs. Among the patients displaying MEPs, those with preserved CSTs showed better recovery of paretic lower extremities. CONCLUSIONS: Combined assessment using TMS and DTT to evaluate CST integrity confers advantages in predicting motor and ambulation recovery in patients with MCA stroke.

Annals of Rehabilitation Medicine 2016;40(4):637-646
OBJECTIVE: To determine the supratentorial area associated with poststroke dysphagia, we assessed the diffusion tensor images (DTI) in subacute stroke patients with supratentorial lesions. METHODS: We included 31 patients with a first episode of infarction in the middle cerebral artery territory. Each subject underwent brain DTI as well as a videofluoroscopic swallowing study (VFSS) and patients divided were into the dysphagia and non-dysphagia groups. Clinical dysphagia scale (CDS) scores were compared between the two groups. The corticospinal tract volume (TV), fractional anisotropy (FA) and apparent diffusion coefficient (ADC) values were calculated for 11 regions of interest in the supratentorial area—primary motor cortex, primary somatosensory cortex, supplementary motor cortex, anterior cingulate cortex, orbitofrontal cortex, parieto-occipital cortex, insular cortex, posterior limb of the internal capsule, thalamus, and basal ganglia (putamen and caudate nucleus). DTI parameters were compared between the two groups. RESULTS: Among the 31 subjects, 17 were diagnosed with dysphagia by VFSS. Mean TVs were similar across the two groups. Significant inter-group differences were observed in two DTI values: the FA value in the contra-lesional primary motor cortex and the ADC value in the bilateral posterior limbs of the internal capsule (all p<0.05). CONCLUSION: The FA value in the primary motor cortex on the contra-lesional side and the ADC value in the bilateral PLIC can be associated with dysphagia in middle cerebral artery stroke.
