1.Concept Analysis of Illness Acceptance in Chronic Disease: Application of Hybrid Model Method

Purpose:
This study was done clarify the concept of illness acceptance in patients with chronic disease.
Methods:
This study was conducted using a hybrid model of concept analysis that consists of three phases. In the theoretical phase, a working definition was formulated through a systematic review. In the fieldwork phase, five participants who had diabetes or hypertension for at least one year were interviewed. In the third phase, the results were combined in the final analysis.
Results:
There are three phases of accepting an illness: experiencing the limits, disease management, and designing new life. At the experience of the limits and disease management stages, the attributes of physical, psychological and social domains were derived, but at the stage of designing their new life, integrated attributes of these three domains were derived.
Conclusion
Illness acceptance of chronic disease was defined as a continuous and dynamic process in three phases. First, patients experience limitations due to the disease, such as physical illness, psychological instability, and difficulty performing social roles. Second, patients manage the disease by engaging in self-management, psychological coping strategies, and establishing social support. Third, patients design their new life by seeking better health-related quality of life and integrating the illness into their everyday life. However, patients experienced negative changes when disease management was not performed properly. The fact that illness acceptance could be cyclic means the difference between illness and loss/death acceptance. Nurse should develop and provide an integrated nursing intervention that is appropriate for phases of illness acceptance.
2.Clinical Manifestations of Invasive Infections due to Streptococcus pyogenes in Children.

PURPOSE: Streptococcus pyogenes is an important cause of invasive diseases in children. We aimed to describe the clinical characteristics of invasive infections due to S. pyogenes in children in Korea. METHODS: A retrospective study of children under 18 years of age with invasive infections due to S. pyogenes at Seoul National University Children's Hospital between March 1992 and December 2012, and Seoul National University Bundang Hospital between March 2003 and December 2012 was conducted. Demographic factors, clinical characteristics, laboratory findings, treatment, mortality and morbidity of all patients were reviewed. RESULTS: A total of 30 among 36 cases identified as invasive disease due to S. pyogenes were available for review. There was a predominance for male subjects (male:female=2.75:1). The median age was 50 months (range 12 days to 15 years) and 53.3% were under 5 years of age. Skin and soft tissue infections (9/30, 30.0%), bacteremia without identified focus (4/30, 13.3%) and bone and joint infections (6/30, 20.0%) were the most frequent clinical presentations. Streptococcal toxic shock syndrome (3/30, 10.0%) pulmonary, abdomen and central nervous system infections (2/30, 6.7%) were also seen. There was a peak in number of patients in year 2012 (9/30, 30.0%). There were no cases of mortality. Erythromycin and clindamycin resistance rates were low by 3.8% and 7.5%, respectively. CONCLUSION: We studied the clinical presentations of invasive infections due to S. pyogenes during the past 20 years in Korean children. The findings of this study help us understand the characteristics of the disease, enhancing early recognition and prompting adequate antibiotic therapy which is important in reducing morbidity and mortality.

3.Analysis of Theory-applied Research in the Journal of Korean Academy of Nursing Administration (2007~2016)

PURPOSE: This study was done to analyze the trend of theory application in articles published in the Journal of Korean Academy of Nursing Administration (JKANA) over the last 10 years. METHODS: Four hundred and eighty-five articles were reviewed to determine the scope of theory-applied research. General characteristics, study design and keywords classification by nursing meta-paradigm were analyzed using established analysis framework. RESULTS: Twenty-four articles (4.9%) were identified as theory-applied research publications. More than half of the articles (54.2%) stated that a theoretical framework was used; the number of theories used in the 24 articles was 27 including nine nursing theories. The majority of the studies were conducted in hospital settings. In the keyword classification by nursing meta-paradigm, 28 environment-related keywords were identified but only 7 keywords were related to health. CONCLUSION: The results show that theory application in Korean nursing administration research is very limited. It is necessary to create a positive educational environment for graduate students to learn about nursing theories as well as theories from other disciplines. In addition, different levels of nursing theories should be continuously developed in the field of nursing administration.

4.Antimicrobial Resistance and Molecular Analysis of Staphylococcus aureus in Staphylococcal Scalded Skin Syndrome among Children in Korea

Background:
Staphylococcal scalded skin syndrome (SSSS) is a skin disease characterized by blistering and desquamation caused by exfoliative toxins (ETs) of Staphylococcus aureus (S. aureus).Although many countries show predominance of methicillin-susceptible S. aureus (MSSA), cases of methicillin-resistant S. aureus (MRSA) have been reported.
Methods:
Twenty-six children aged <15 years diagnosed with SSSS from January 2010 to December 2017 from three hospitals were included. S. aureus isolates from cases were analyzed for multilocus sequence types and ETs. Medical records were reviewed for clinical characteristics, treatment, and antimicrobial susceptibility patterns of S. aureus
Results:
Among the 26 cases, mean age was 2.3 years. According to skin manifestations patients were classified as generalized (n = 10, 38.5%), intermediate (n = 11, 42.3%), and abortive (n = 5, 19.2%). Among all cases, 96.2% (25/26) were due to MRSA and the macrolideresistance rate was 92.3% (24/26). ST89 (n = 21, 80.8%) was the most prevalent clone, followed by single clones of ST1, ST5, ST72, ST121, and ST1507. The eta gene was detected in one (3.8%) isolate which was MSSA. The etb gene was detected in 14 (53.8%) isolates, all of which were ST89. Nafcillin or first-generation cephalosporin was most commonly prescribed (n=20, 76.9%). Vancomycin was administered in four patients (15.4%) and clindamycin in nine patients (34.6%). Among MRSA cases, there was no difference in duration of treatment when comparing the use of antimicrobials to which the causative bacteria were susceptible or non-susceptible (9.75 vs. 8.07 days, P > 0.05).
Conclusion
S. aureus isolated from children with SSSS in Korea demonstrated a high prevalence of methicillin-resistant ST89 clones that harbored theetb gene. The predominance of MRSA suggests that antibiotics to which MRSA are susceptible may be considered for empirical antibiotic treatment in children with SSSS in Korea. Further studies on the role and effectiveness of systemic antibiotics in SSSS are warranted.
5.Development of Syllabuses for Biological Nursing Science Subjects based on Learning Outcomes: Structure and Function of Human Body, Pathogenic Microbiology, Pathophysiology, and Mechanisms and Effects of Drugs.

PURPOSE: The purpose of this study was to develop syllabuses of basic nursing science subjects (Structure and Function of Human Body, Pathogenic Microbiology, Pathophysiology, and Mechanisms and Effects of Drug) based on learning outcomes. METHODS: We developed a 3-phase plan to develop the syllabuses. In the first phase, The Korean Society of Biological Nursing Science (KSBNS) held a workshop in May, 2013 in which professors who are in charge of basic nursing science subjects shared opinions about learning outcomes. As a result, initial prototype syllabuses came out. In the second phase, revised syllabuses based on learning outcomes were presented and discussed in the conference held by KSBNS in November, 2013. In the last phase, a research team who taught basic nursing science subjects finalized the syllabuses during the meetings. RESULTS: Syllabuses of 4 basic nursing science subjects were developed. CONCLUSION: These syllabuses of 4 subjects need to be disseminated throughout nursing colleges. Further revision needs to be made according to the circumstances and context of each school.

6.Pertussis Accompanying Recent Mycoplasma Infection in a 10-Year-Old Girl.

Recently, the incidence of pertussis has been increasing; however, reports on mixed infection of pertussis with other respiratory pathogens are rare in highly immunized populations. We report the case of a 10-year-old girl who presented with cough, post-tussive emesis, and fever. She was subsequently diagnosed with bronchopneumonia. Although she had received five doses of diphtheria-tetanus-acellular pertussis vaccine, polymerase chain reaction of her nasopharyngeal aspirate confirmed Bordetella pertussis infection. In addition, serologic testing for Mycoplasma pneumoniae was also positive. The patient was treated with roxithromycin without any complications. This is the first report of mixed B. pertussis and M. pneumoniae infection in Korea. To avoid under-diagnosis, pertussis should be considered in patients with chronic cough even when other respiratory pathogens have been documented.

7.Successful Treatment of Intractable Hiccup with Levetiracetam

Hiccups may be considered a form of myoclonus of diaphragm. Valproic acid has been considered the drug of choice in treatment of intractable hiccups; however, its various adverse events limit its use in the clinical basis, especially in elderly or complicated patients. Levetiracetam was known as a safe antiepileptic drug. However, the anti-hiccup property of the levetiracetam has been rarely reported. We report a 69-year old male patient who developed sudden persistent hiccups and was successfully treated with levetiracetam. Levetiracetam should be considered as alternative treatment option in selected patients with intractable hiccups.

Objectives:
This study investigated the effectiveness of the intervention program for elementary school students with reading disorders.
Methods:
The intervention program focused on phonological awareness, phonics, and decoding training applied individually to 25 children with a reading disorder by the teachers in charge for four months. To measure the efficacy, this study evaluated the word reading accuracy, fluency, and the related cognitive functions, including phonemic awareness, phonics, and rapid automatized naming using the Computerized Comprehensive Learning Test-Reading before and after the program.
Results:
After the intervention, improvements were observed in the following: the reading fluency score and fluency percentile in the word attack test; reading accuracy rate, fluency score, and fluency percentile in the nonword decoding test; fluency score and fluency percentile in the rapid automatized naming tests; accuracy rate in the letter-sound matching test; accuracy rate in the nonword repetition test. On the other hand, there was no significant difference in the paragraph reading fluency test. According to the subtypes of reading disabilities, children with dyslexic or mixed types improved after the intervention.
Conclusion
Reading accuracy and fluency of school-aged children with reading disorders improved through the intervention program by the schoolteachers.

10.Cerebrospinal fluid flow in normal beagle dogs analyzed using magnetic resonance imaging

Background:
Diseases related to cerebrospinal fluid flow, such as hydrocephalus, syringomyelia, and Chiari malformation, are often found in small dogs. Although studies in human medicine have revealed a correlation with cerebrospinal fluid flow in these diseases by magnetic resonance imaging, there is little information and no standard data for normal dogs.
Objectives:
The purpose of this study was to obtain cerebrospinal fluid flow velocity data from the cerebral aqueduct and subarachnoid space at the foramen magnum in healthy beagle dogs.
Methods:
Six healthy beagle dogs were used in this experimental study. The dogs underwent phase-contrast and time-spatial labeling inversion pulse magnetic resonance imaging. Flow rate variations in the cerebrospinal fluid were observed using sagittal time-spatial labeling inversion pulse images. The pattern and velocity of cerebrospinal fluid flow were assessed using phase-contrast magnetic resonance imaging within the subarachnoid space at the foramen magnum level and the cerebral aqueduct.
Results:
In the ventral aspect of the subarachnoid space and cerebral aqueduct, the cerebrospinal fluid was characterized by a bidirectional flow throughout the cardiac cycle. The mean ± SD peak velocities through the ventral and dorsal aspects of the subarachnoid space and the cerebral aqueduct were 1.39 ± 0.13, 0.32 ± 0.12, and 0.76 ± 0.43 cm/s, respectively.
Conclusions
Noninvasive visualization of cerebrospinal fluid flow movement with magnetic resonance imaging was feasible, and a reference dataset of cerebrospinal fluid flow peak velocities was obtained through the cervical subarachnoid space and cerebral aqueduct in healthy dogs.
