1.Roles of Outer Membrane Vesicles (OMVs) in Bacterial Virulence.

Outer membrane vesicles (OMVs) are ubiquitous membranous structures in all Gram-negative bacteria, including pathogens and non-pathogens. Gram-positive bacteria also release membrane-derived vesicles (MV). Originating from the cell envelope, OMVs are enriched with bacterial antigen molecules that conduct multiple functions as decoys to manipulate the host immune system. Besides, OMVs and their components play diverse roles in nutrient acquisition, biofilm formation, and resistance to antibiotics. Despite the diverse benefits ascribed to OMVs, many questions remain unanswered with regard to OMV biogenesis and cargo selectivity. In this report, we review the advantages of vesiculation in the context of all bacteria and then focus on additional benefits acquired by OMVs in pathogenic bacteria.

2.A Study on the Stress and the Dietary Habits of Elementary School Children by Mother's Employment Status.

The purpose of this study is to determine the stress and the dietary habits of elementary school children and assess the associations with mother's employment status. The subjects were 423 students of 5th and 6th grade students (210 boys and 213 girls) in Seoul and Gyeonggi-Do. Two hundred fifty-two mothers had some kinds of job and 171 mothers were housewives. The students had stress the most from their studies/schoolwork (M = 2.37), while the least from "teacher/school" (M=1.51). The average stress score of the subjects was 1.76. The average score of dietary habits was 3.43. More specifically, the statement "not skipping breakfast" had the highest score (M = 4.04) and the statement "not substituting cereals, breads or drink milk for any meal" had the lowest score. Students with employed mothers had higher stress level than students with unemployed mothers. Students with employed mothers who have blue color job showed the lower dietary habits score than those with employed mothers who had other types of job. Students of employed mothers who go to school at dawn showed the lowest dietary habits score. In both employed and unemployed mothers, the stress score of students negatively correlated with the score of dietary habits. It was important for the mother to be at home when the students go to school and return. Based on this study, mothers should be aware of the stress level of their children get depending on their employment status. They should also provide their children to have good dietary habits and nutritional education regularly.

4.Sensory Polyneuropathy and Cobalt Intoxication after Metal-on-Metal Total Hip Arthroplasty.

We describe a patient with sensory polyneuropathy and cobalt intoxication. The cause of cobalt intoxication was metallosis of a metal-on-metal hip joint composed of cobalt-chrome alloy. A nerve conduction study revealed axonal sensory polyneuropathy, which improved slightly after chelation therapy and revision surgery. This case implies that a history of arthroplasty should not be neglected in the context of sensory polyneuropathy.

5.Nutrient Intake and Food Restriction in Children with Atopic Dermatitis.

This study was performed to investigate the status of food restriction and the list of restricted foods in children with moderate to severe atopic dermatitis (AD), and to find out the effect of food restriction on the changes in nutrient intake and the severity of the disease. Sixty two patient children aged 12 months to 13 years presenting AD with a SCORing of Atopic Dermatitis (SCORAD) index between 20 and 50 were enrolled. The presence of food limitation, and list of restricted foods were surveyed through the caretakers and the patients were divided into 3 groups by the number of restricted food: non-restricted group, one to three restricted group, and more than three restricted group. Dietary intake was assessed for 3 months using a food frequency questionnaire (FFQ). Half of the subjects restricted foods. The restriction was higher in the order of soda, food additives, walnut, peanut, and other nuts as a single food item; and shellfish and crustacean group, processed foods, nuts, milk & dairy products, and meats as a food group. More than three restricted group ingested more fruits and less fish and meats, resulting in high consumption of vitamin C (p = 0.027). No significant difference in the ratio of nutrient intake by the number of restricted foods was observed in other nutrients. Significant improvement of AD symptom was observed in non-restricted group (p = 0.036) and one to three restricted group (p = 0.003). It is necessary to provide proper nutrition information and systematic and continuous nutrition management for balanced nutrient intake and disease improvement in children with AD.

6.Small Fiber Neuropathy and Postural Orthostatic Tachycardia Syndrome after Human Papillomavirus Vaccination

We describe a 44-year-old woman with paresthesia, fatigue, and palpitation, 10 days after human papillomavirus (HPV) vaccination. The quantitative sensory test showed abnormal detection threshold in her foot. Tilt test result indicated postural orthostatic tachycardia syndrome. Symptoms were improved after immunomodulating therapy, pain control drug, and oral beta blocker medication. This is first case report for small fiber neuropathy and autonomic dysfunction after HPV vaccination in Korea.

8.Performance Evaluation of the CRE2 Reagent from Siemens for Serum Creatinine Measurement.

BACKGROUND: For creatinine measurement, the enzymatic method is known to be more accurate than the Jaffe method; however, the latter is still widely used. We evaluated the performance of the CRE2 reagent (Siemens Healthcare Diagnostics Inc., USA), which uses a modified Jaffe method. METHODS: Three quality control standards were used for precision evaluations of CRE2 on Dimension VISTA 500 instrument (Siemens). Moreover, the linearity and carryover characteristics were assessed. Sixty-eight creatinine results obtained using the CRE2 and ECREA (enzymatic) reagents (Siemens) were compared with those obtained using the L-CRE (enzymatic) reagent (Shinyang Diagnostics, Korea). The accuracy of CRE2, ECREA, and L-CRE was evaluated using a standard reference material. RESULTS: The CV of within-run (0.7–2.4%), between-run (0.4–1.7%), between-day precision (0.7–0.9%) for three standards, and total CV for medium (1.6%) and high levels (1.3%) satisfied the analytical goal. The linearity for CRE2 was excellent (R2=0.999). Comparisons of CRE2 and ECREA to L-CRE were well correlated (r=0.996 and 0.997, respectively). In comparison with L-CRE, 5 CRE2 results and 15 ECREA results exceeded minimum bias goal (5.1%) in samples with creatinine levels of >1 mg/dL. The carryover rate was −0.04%. In terms of accuracy, the percent bias values of CRE2, ECREA, and L-CRE were 7.4, −6.4, and −3.4, respectively, for low level; and 3.9, −1.5, and 0.7, respectively, for high level. CONCLUSIONS: For creatinine measurements, the CRE2 reagent showed good performance. It can be used in the diagnosis, treatment monitoring, and risk assessment of kidney diseases.

9.Effects of Systematic Discharge Education Program on Anxiety and Parenting Confidence in Mothers of Premature Infants

Purpose:
This quasi-experimental single group study aimed to confirm the effects of discharge education using a systematic discharge education program on anxiety and parenting confidence in mothers of premature babies.
Methods:
This study conducted discharge education for 3 to 5 days prior to the discharge of 29 mothers of premature babies born in the neonatal intensive care unit. Data were collected between April 1, 2021, to June 30, 2021, and were examined. The hypotheses were analyzed using the Wilcoxon signed-rank test.
Results:
Discharge education using a systematic discharge education program was effective in increasing the parenting confidence of mothers with premature babies (z=-3.839, p<0.001). However, it was not effective in reducing anxiety (z=-1.712, p=0.087).
Conclusion
The effects of the systematic discharge education program development and discharge education systematized discharge nursing education, reduced mothers’ anxiety in raising premature babies at home after discharge, and contributed to improving parenting confidence.
