3.A Study on the Effectiveness of Clocortolone Pivalate (Purantix) Gream on Certain Common Skin Diseases.

In order to evaluate the effectiveness of clocortolone pivalate(Purantix) on corticosteroid responsive dermatoses, a symmetical paired. comparison technique was performed using 0. 1% clocortolone pivalate and 0. 025% triamcinolone acetonide creams. During the pericd of 7 months from September 1977 to March 1978, 37 patients with bilateral lesions of contact dermatitis, seborrheic dermatitis atopic dermatitis, lichen simplex chronicus and psoriasis were selected and treated with afore mentioned drugs, In the 15 psoriatic patients, clocrtolone pivalate was superior in 3, triamcinolone acetonide was superior in 2, 4oth drugs were equally effective in 10. In the evaluation of all 37 patients clocortolone pivalate was superior in 8, triamcinolone acetonide was sureior in 3, both drugs were equally effective in 26. The overall superiority of clocortolone pivalate to triamcinolone acetonide was non significant, There was no side effects with either drug. The simplicity of the vasoconstriction tcst has led to its general use in the study of the potency of new topical steroids. In the 6h occluded single-application vasocon-strictor test performed on 40 healthy students, there was no significant difference between clocortolone pivalate and triamcinolone acetonide, but they significantly d.ifferent from 1% hydrocortisone and the control; a simple ointment base(p<0.01).

4.A Case of Muscular Sarcoidosis diagnosed by Gallium-67 Scintigraphy and Magnetic Resonance Imaging.

Gallium-67 scintigraphy is helpful in the assessment of active extrapulmonary sarcoidosis. Muscular involvement of sarcoidosis is often asymptomatic or nonspecific, and laboratory examinations do not provide convincing evidence of muscular involvement. We report a case of muscular sarcoidosis, which was detected by gallium-67 scintigraphy. In a patient who was suffering from fever and arthralgia of knee joint, gallium-67 scintigraphy showed mediastinal and hilar involvement of sarcoidosis with unexpected extensive muscular uptake. Magenetic resonance imaging revealed the detailed depiction of intramuscular infiltration of sarcoid granuloma. Gallium-67 scintigraphy is useful in detecting inflammatory muscular involvement of sarcoidosis as well as other multiorgan involvement.

5.Treatment and Clinical Application Of a Triangular Splint Method for Fracture of the Humerus

Fracture of the shaft of the humerus had been treated by many different way, such as hanging cast, shoulder spica cast, abduction splint, and Velpeau bandage, etc, The authors have employed the new trianglar splint designed by ourselves for the 54 cases of fracture of the humeral shaft from Jan., 1965 to Oct., 1976 in the department of Orthopedic surgery of Han-il Hospital. In our experience, the triangular splint method is useful for not only fracture between the head of the humerus and supracondylar region, but also shoulder joint injuries and diseases which need firm immobilization. The triangular splint method has merits, such as spontaneous reduction by the fractured arm weight with gravity, the stability during care, relatively comfort during procedure. Satisfactory results had been obtained by the triangular splint method.

6.The Role of Arthroscopy in Diagnosis and Treatment of the Knee Disorders

Arthroscopy has now become a valuable method for the diagnosis and treatment of the internal derangement of the knee. Based on the experiences gained during the past three years, in which 93 knees with posttraumatic disorders were observed arthroscopically, several uncommon and interesting findings were encountered. Such illustrative findings have been summarized here. I. Injury at the parameniscal portion is readily overlooked and especially the middle and posterior one-thirds are inaccessible. A wavy undulation of the concave margin of the meniscus, when abnormally exaggera;ed by tibial rotation, possibly suggests the presence of this type of injury. 2. The discoid cartilage and the bucket-handle tear of meniscus occasionally show similar pattern arthroscopically. 3. The plica of the knee can sometimes be a cause of knee disorders. Any internal derangement, which tends to be localized around the patella, warrants close scrutiny for its presence. 4. A large foreign body within a knee joint could easily be removed by virtue of direct arthroscopic visualization.

7.Allergic Contact Dermatitis due to Plaster of Paris Cast: A Case Report

A 42-year old man was admitted for treatment of a fracture of the medial condyle of the left femur. After 10 days of cast immbilization of the above mentioned extremity, erythematous papulo-vesicular eruption was noted under the external splintage. We concluded that this was an unusual allergic contact dermatitis which is caused by CaSO4.1/2H20, and proved it by patch test in our clinic.

8.A Case of Subcutaneous Dermoid Cyst.

A 18-year-old male patient presented with the skin colored, dome-shaped, soft, movable mass in his left retroauriculr region. The mass has slowly increased in size since birth. Histopathologic exarnination revealed sebaceous gland, eccrine gland and crosssectioned hair in the surrounding tissue, hair shaft projected into the lumen, and laminated horny material in the cyst. Transitional zone between stratified squarnous epithelium and the wall lined by histiocytes showed homogenous degenerative change. After surgical removal of the lesion, no recurrence was observed during 15 months of follow-up.

9.Immediate Hypersinsitivity Reaction in Chronic Urticaria.

In a large proportion of patients with chronic urticaria, at present, the definite tiology can't be found. There have been occasional attempts to identify the aIIergic causes uaing allergic skin tests, only ta suggest the Candicla albicans and. some food yeasts ae probabIe causes. Thia atady was designed to investgate how often patients with chronic urticaria from Chonnam Province react positively to skin tests with extracts of 42 common. allergene in the standard group Al-13 produced by Bencard Ltd. in England. The authora alao surveyed the clinical characteristics of 63 patients with chronic urtimria of more than 3 months duration, who bad visited the skin clinic of Chosun Univeresity HcepitaI from May. 1977 throug 3. Apr. 1979. The results were summarized as follows; 1. Among 63 patients, the maIe to female ratio was 1: l.2, and the peak incidence was in the 20s and 30s. 2, In regard to the duration of chroic urticaria, the highst frequency waa below one year duration. 3. Among 60s of the pa,tients who experienced seasonal changes, 32.4% had suffered aggravation during the winter. 4. The most common diseases combined with chronic urticaria were allergic rhinitis and atopic dermatitis, occuring in 9.5% of all subjects. 5. Of thase examined, 69.8% showed positive reactions to the prick test with the peak incidence in the 20s and 30s. 6. Among 42 allergens used far the prick test, 28 allergens turned out to be possibly casal agents, the rnost frequent group of allergens involved were grain du sts (27.1%), animal hairs(21.8%) and moulds(16.5%) in that order of frequency and occupying over 65% of the total positive reactions. And the most frequently involved specific allergens were hay dust, mixed grain dust straw dust and tree in that order of frequency. Moreover, 90 of the involved allergens were inhalants. 7. In teen-agers, the reactions to grain dusts, animal ha.irs, rnoulds and pollens were the rnost frequent while reactions to food allergens were most frequent in the 20s, and cotton and house dust are most frequently involved in tbe 40s. 8. Multiple sensitivity was not:d in a majority of cases and the most frequent combination was 2 to 5 allergens. 9. The rnost common abnormal hematologic finding in chronic urticaria was eosinophilia.
