1.Acquired , Bilateral Nevus of Ota - like Macules : A Clinical and Histopathological Observation.

We observed the clinical and histopathologic characteristics of acquired, bilateral nevus of Ota-like macules in each 80 and 23 patients. The results were as follows: 1. All patients were female and the peak age of onset was third decade with an a,verage age of 27.4 years. 2. The colors of lesions were slate, brown, dark brown, and blue black in order of freguency. 3. The lesions were most comrnonly observed in the malar area and alae nasi. 4. Three patients had family history of acquired, bilateral nevus of Ota-like :macules. 5. Histopathologically pigment-bearing cells, bipolar or irregular in shape, were found mostly in the upper- and mid-portion of the dermis. The melanocytes showed weakly positive dopa reaction. 6. In electron microscopic observation, these pigrnent-bearing cells contained many singly dispersed melanosomes in stage 3 and 4 of melanization and wece surrounded by an extracellular sheath.

4.Two Cases of Eccrine Spiradenoma.

She describe herein two cases of eccrine spiradenoma. One is an indolent and the other is painful or tender multiple small nodules distributed in a zosteriform pattern. Histopathologically, the encapsulated lobules of the tumor are composed of two types of cells, intensely staining cells and pale staining cells, usually arranged in whorls, cords, and pseudoglands. In the second case, enzyme histochemical staining and electron microscopic examination have been done and are compared with other cases in the literature.
5.Clinical usefulness of rapid antigen test to detect respiratory syncytial virus infection.

PURPOSE: Respiratory syncytial virus (RSV) is the most frequent cause of lower respiratory infections in infants and young children. Early detection allows quarantining of infected inpatients to prevent nosocomial transmission and to choose a treatment. To achieve rapid reporting, to facilitate prompt antiviral therapy, and to avoid unnecessary use of antibiotics, an easy, rapid diagnostic method for RSV is needed. We evaluated a lateral flow immunochromatography (RSV Respi-Strip test) and EIA (Enzyme immuno assay) compared to RT-PCR. METHODS: From April 2007 to March 2008, 112 consecutive respiratory specimens (nasopharyngeal aspirates, throat swabs, tracheal aspirates, sputum) from patients who were suffering from the clinical signs and symptoms of respiratory tract infection were enrolled in Busan. A total of 112 patients were tested with RSV Respi-Strip (Corio-BioConcept, Belgium), EIA, and RT-PCR at the same time. RESULTS: Of the 112 specimens tested, the number of children who showed positive results at RT-PCR and Respi-Strip were 45 and 42, respectively. The Respi-Strip rapid antigen test had a sensitivity of 88% and a specificity of 94%. The positive and negative predictive values were 90% and 92%, respectively. The agreement was 83%. CONCLUSION: In our study, the rapid antigen test had as much sensitivity as any method for detection of RSV. The test has many advantages such as easy performance, simple interpretation, and rapid results. If the rapid antigen test is widely applied in the clinical setting, the may be useful for diagnostic and epidemiological studies of RSV infection.

6.Assessment of Genotoxic Hazard in Petrochemical Workers.

In order to evaluate the genotoxic hazard among workers potentially exposed to low level petrochemical substances, the analyses of micronuclei (MN) and sister chromatid exchanges (SCEs) in lymphocytes were performed in 46 male workers (as exposed group) and 46 nonexposed subjects (as control group). Mean frequencies of MN and SCEs (respectively, 12.9/1000 cells and 6.5/cell) in exposed group were very significantly higher than those (10.2/1000 cells and 5.4/cell) in control group. And there were also significant differences in mean frequencies of MN and SCEs adjusted for age, employment duration, smoking, and drinking between two groups. Median frequencies of MN and SCEs in exposed group were very significantly higher than those in control group. Frequencies of SCEs were higher in smokers than in non-smoker. Frequencies of MN in smokers, however, were similiar to those of non-smoker. Interaction between exposure and smoking on MN and SCEs induction was not observed. The results suggest that there is genotoxic hazard in high risk group like workers handling carcinogens in petrochemical plants and the analyses of MN and SCEs are useful biomarkers for the exposure to hazard substances even at the level below the exposure limit.

7.Significance of Nocturnal Penile Tumescence in the Differential Diagnosis of Sexual Impotence.

Nocturnal penile tumescence is very importent in differential diagnosis between psychogenic and organic impotence. NPT was assessed in 37 impotent patients and 8 normal potent patients. There was no discrepancy between psychogenic and control group, but some 4 case had slight reduced NPT. In all organic cases, the figures of frequency, degree and duration of NPT was significantly reduced. This method is not definite to differentiate impotent: 27% of psychogenic importent patients have reduced NPT. We use some complementary diagnostic tool to diagnose organic impotence: routine lab, snap gauge test, PDI, erection penogram, BCRL and SEP, hormonal test and MMPI etc. This method is useful to differentiate impotence, when used with other complementary method. It is promising, but further study is needed.

8.A case of Hydrofluoric Acid Burn.

Hydrofluoric acid(HF) burn is characterized by progressive tissue necrosis and severe pain. A 31 year old employee in the semiconductor industry came in contact with the 50% aqueous HF through a pin hole in the thumb of his protective gloves. About 2 hours after exposure, h noted deep, severe burning, excruciating pain and svtelling of right thumb. Thereafter, clark bluish eclematous necrotic patches were visible through the nail plate and on the distal end of right thumb. Pain was not effectively relieved by digital nerve block.

9.Clinical Experience of the Erectiometer.

An erectiometer for the measurement of sleep erections is described. Clinical data using the erectiometer indicates that it processes a significant potential for valid measurement of the circumfernetial increase that may occur during sleep. Because of its reliability, simplicity, reproducible data and low cost the erectiometer is proposed as a screening procedure or as a diagnostic alternative when sleep laboratory facilities are not feasible practically.

10.Ten years of experience with various penile prosthesis in Korean.

Currently there are more than 10 types of penile prosthesis available, ranging from the very simple to the very sophisticated. We review our experiences with various penile prosthesis, with particular regard to the complication rate. From Dec. 1983 to Jul. 1993, we implanted 295 penile prosthesis of eight different types. The average age of patients was 44 years. Every patient was evaluated with various multidisplinary diagnostic approaches. The etiologies of impotence were vasculogenic 29%, diabetogenic 22%, spinal cord injury 16%, pelvic bone injury 11%, etc. The types of implanted prosthesis were AMS malleable 143, Jonas 42, Dynaflex 36, Hydroflex 8, Uni-Flate 1000 2, AMS 700 CXM 58, Ultrex 3, Mentor alpha-1 3 and the mean follow-up period was 34 months. The diameters of implanted prosthesis were from 9.5 mm to 13 mm, mostly 9.5 mm (52.9%). The length of implanted prosthesis were from 10 cm to 20 cm, mostly 16 approximately 18 cm (68.8%). Cases with uneven diameters or lengths were 20 (6.8%). The int aoperative complications were 1 corporeal rupture and 1 bladder rupture, and the postoperative complications were 2 prosthesis infections, 2 mechanical failures, and 1 prosthesis infection with mechanical failure. In those 4 patients reimplantations were successful. More than 99% (290/291) patients still have functioning prosthesis. Every prosthesis has their advantages and disadvantages. Factors to be analysed in the selection of proper prosthesis should include patients economic status, education, personality, social activity, hand dexterity, and penile size.
