STUDY DESIGN: The authors is to report the clinical and radiological results of proximal half corpectomy with one motion segment fusion in Denis type B burst fracture. OBJECTIVES: To evaluate the efficacy of proximal half corpectomy in Denis type B burst fracture of thethoracolumbar and lumbar spine. SUMMARY OF LITERATURE REVIEW: For the operative management of burst fracture, various mothods including posterior ligamentotaxis, posterolateral decompression, anterior decompression or combined were reported. Among the methods, anterior decompression by the corpectomy of fractured vertebral body and fusion with or without instrumentation is the one of the widely accepted method of treatment. However, anterior decompression by excision of whole vertebral body has the disadvantage of high complication rate due to the instability from the large defect and long length of bone graft. Moreover, two motion segments have to be sacrified, which is very important especially in thoracolumbar and lumbar area. Material and METHODS: 43 cases operated from 1989 to 1996 and the minimum follow up period was two years and compared with that of 48 cases who were treated by total corpectomy and two motion segment fashion from 1986 to 1989. RESULTS: Solid bony union was obtained in 43 cases within 6 months and no back pain was complained in 39 cases (93%) at last follow up. There was no significant difference between two groups in correction of anterior vertebral height and kyphotic angle. Length of bone graft was 3.0cm in half corpectomy group and was 6.3cm in total corpectomy group. Hardward breakage or graft collapse was not observed in proximal half corpectomy, while there were 5 cases in total corpectomy. CONCLUSION: Proximal half corpectomy and fusion of one motion segment in Denis type B burst fracture is believed to be a successful method which can minimize the fused level, increase the stability, preserve motion segment and reduce the complication.

Objectives: The aim of this study was to investigate the prevalence of hazardous drinking (HD) and its association with sociodemographic factors, smoking, social anxiety, self-esteem, and ego-resiliency in medical students. Methods: A total of 408 medical students were included in this study. Data were collected by self-administered questionnaire, and HD was assessed by Alcohol Use Disorder Identification Test (AUDIT). The HD and non-HD group were defined using the AUDIT score 10 for male and 6 for female as a cut-off, respectively. Multiple logistic regression analyses were carried out to examine the impact of sociodemographic factors, smoking, social anxiety, self-esteem, and ego-resiliency on HD. Results: A total of 192 participants (47.1%) were identified as having HD. Frequency of smoking and the severity of social anxiety were significantly higher in HD group than non-HD group. Meanwhile, the levels of self-esteem and egoresiliency were significantly lower than HD group than non-HD group. In multiple logistic regression, smoking was associated with a higher risk of HD, whereas ego-resiliency and age were associated with a lower risk of HD. Compared to the high level of subjective socioeconomic status, both middle and low level were associated with a lower risk of HD. Conclusions HD among medical students was quite common and associated with smoking, ego-resiliency, age, and level of subjective socioeconomic status. Strong efforts to improve smoking cessation and ego-resiliency, as well as education for HD in medical schools, is needed to reduce the harmful effects of alcohol.

Retroperitoneal fibrosis sometimes causes urological problem involving the ureter, but the mechanism is uncertain. An aortic aneurysm, including an iliac artery aneurysm, is thought to be one of the mechanisms of retroperitoneal fibrosis. However, cases caused by an isolated iliac artery aneurysm are very rare, and symptoms tend to be non-specific; therefore, no definitive treatment has been established. Herein, we report our recent experience of a patient with a left common iliac artery aneurysm involving the ureter, who underwent successful surgical therapy.

We performed a retrospective study of perforating eye injuries treated at Kyung-pook National University Hospital during period from 1983 to 1987. Total number of patient with perforating eye injuries was 233(237 eyes). Of these 233 cases, 194(83.3%) were male and 39(16.7%) were female. Mostly, they were under 30 years of age and the peak age group was the most frequent(126 patient 54%) and combined corneoscleral perforation was next. The complication was mostly lens opacity, 95(40%) out of 237 eyes. Uveal prolapse were seen in 70(29.5%) eyes, traumatic hyphama was seen in 49(20.7%) eyes, IOFB(Intraocular-Foreign body) was seen in 22(9.3%)eyes, and vitreous hemorrhage was seen in 20(8.4%) eyes. Other less-frequent complications included retinal lesion(4.2%), iridopialysis(2.1%), and optic nerve injury(0.4%). The most frequent cause of perforating eye injuries was industrial accidents followed by home accidents and traffic accidents. The causes of decreased visual acuity was mostly corneal opacity, 80 out of 237 eyes(33%) followed by vitreous opacity, 18(7.9%)eyes. Final visual acuity better than 0.5 was noted in 79(33.3%).

Propyl gallste and other gallic acid esters are used as antioxidants in lipsticks, lip balms and salves, cosmetic creams and lotions, bakery products, edible fats and other pharmaceutical and industrial products. Propyl gallate is used widely but allergic contact dermatits from propyl gallate is rare. A 44-year-old female patient had pruritic multiple tiny erythematous papules and vesicles on the margin of her lip for a week. We found that the causative material of the allergic contact cheilitis was propyl gallate. We proved it with a patch test, provocation use test and quantitative and qualitative analysis of the lipstick. To our knowledge, this is the first case report of lipstick allergic contact cheilitis from propyl gallate in Korean literature.

The incidence of thyroid cancer is rapidly increasing worldwide. Recently, attention to quality of life (QOL) issues has been increasingly addressed in the management of cancer. The goal of this review is provide a systematic overview for clinical factors associated with QOL in patients with thyroid cancer. Age is often cited as a QOL predictor with older patients more vulnerable than younger patients. High levels of fatigue and psychological distress such as anxiety and depression might be associated with decreased QOL. Although surgery for thyroid cancer leads to worse QOL shortly, there is a trend towards recovery with time. Levothyroxine treatment in thyroid cancer can result in similar or slightly impaired QOL. Thyroid hormone withdrawal causes significant reductions of QOL of thyroid cancer patients. The use of recombinant human thyrotropin (rhTSH) instead of thyroid hormone withdrawal can prevent QOL deterioration by thyroid hormone withdrawal. Generally, thyroid cancer survivors have a similar or slightly worse QOL compared with the normative population. In conclusion, thyroid cancer has a considerable impact on QOL of patients, and therefore multidisciplinary approach with special concern for QOL is recommended.

Paraquat is the most important bipyridyl herbicide, acting as a norselective herbicide with somewhat unique properties. The major acute local cutaneous side effects reslt from the caustic properties of paraquat, which produce local ulceration of the epithelial surface. Blistering of the skin and cracking and even loss of the fingernails have been reported. Its systemic taxic ties also include pulmonary fibrosis, cardiac failure, renal failure, and hepatotoxicity. We report a case of erytlema multiforme with transient heat-toxicity due to percutanious absorption of paraquat. A 31-year-old farmer was admitted to our de artment because of target-like bullous erythema on both thighs and buttocks following accidenta contact with paraquat. The histopathologic findings show d spongiesis, exocytosis, intercellular dema in the epidermis, degeneration of dermo-epidermal junction and inflammatory cell infiltrations in the dermis especially papillary and perivascular area.

In 1984, Gustilo subgrouped type III open fracture into III a, III b and III c according to the severity, soft tissue damage and vascular injury. Type III c open fracture presents serious problems especially possible amputation due to vascular insufficiency, so emergency vascular surgery with bone fixation by using plate, IM nail fixator is mandatory. We reviewed 10 cases of type III c open fracture of the forearm and evaluated the bone healing process according to fixation method, intramedullary nailing and plating treated at Orthopaedic Department of Sung-Ae General Hospital from Feb. 1986 to Aug. 1988. The folliwing results were obtained: l. All cases were industrial accidents, 9 cases in right, and the mean age was 24.1 ranging from 17 to 34. 2. There were 4 cases of the radial & ulnar artery rupture and 6 cases of the radial artery rupture, 7 cases were treated end to end anastomosis and 3 cases with vein graft. 3. The fracture level of ulna & radius was same in all cases, 7 cases in distal one-third, 2 cases in middle one-third and 1 case in proximal one-third. 4. The average time of union was 14.1 weeks in the radius and 14.5 weeks in the ulna. 5. The fixation methods were plating in 7 cases and IM nailing in 3 cases. 6. The complications were non-union in 1 case, osteomyelitis in 2 cases and refracture in 1 case. 7. The functional result was excellent in 1 case, satisfactory in 3 cases, unsatisfactory in 5 cases, and failure in 1 case according to the rating system of Anderson.
