3.Allergen Prick Test Reactivity in the Patients with Uriticaria and Atopic Dermatitis.

This study was undertaken to investigate the skin sensitivity to 54 different allergens in 248 patients of 81 acute urticaria, 141 chronic urticaria and 26 atopic dermatitis, who visited in Department of Dermatology, College of Medicine, Chung Ang University Hospital from February 1986 to January 1987. The results were as follows . 1. The sex ratio of male to female of 248 patients with allergic state was 1.1: 1 (130: 118) and third and fourth decades occupied 56.5%. In regard to the duration of chronic urticaria, less than 1 year were 31.2% and between 1 to 3 years were 28.3%. 2. In acute urticaria, the prick test positive reactivity to allergens were in descending order to mite, household insect mixture, Trichophyton mixture, house dust, mold mixture C and silk, whereas in chronic urticaria ', mite, household insect mixture, house dust, cockroaeh mixture, Trichophyton mixture and silk and in atopic dermatitis ; mite, house dust, Trichophyton mixture, household insect mixture, mold mixture A and C. 3. The positive reactivities of allergens in patients with atopic dermatitis, chronic urticaria and acute urticaria were 69.2%, 52.5% and 35.8%, respectively. 4. The positive reactivity to compound allergens were higher than to single and the most frequent combinations were 2 to 5 allergens (85.4%). 5. Out of 54 allergens used in this study, 49 allergens were reactive and the most frequent group of allergens were house dusts (41.5%) and commonly involved allergens were mite (27.8%), household insect mixture (15.7%), house dust (13.7%), Trichophyton mixture (11.7%) and cockroach mixture(7.7%). 6. Common other allergic diseases combined with atopic dermatitis were rhinitis and bronchial asthma, occuring in 23.1%.

4.A Case of Paraffinoma.

We experienced a 57-year-old male patient who have had multiple coin sized, erythematous, psinful, indurated ulcerations with purulent discharge on the right calf area since 10 years before. Previously he received injections of paraffin in the lesion site for correcting the thinning of right calf due to complication of pyoderma. Histopathologic examination showed "Swiss cheese" appearance of parsffinoma. Good result was obtained after treatment with surgical excision and split thickness skin graft.

5.The SPM Study on the Change of Titanium Surface Roughness following Air-powder Abrasive and Application Time of Citric Acid.

The Peri-implantitis causes inflammation of periodontal tissue and bone loss. It contaminates surface of implants. Therefore, guided bone regeneration has been used for the treatment of this disease. For the reosseointegration of the exposed surface, various mechanical and chemical methods have been used for cleaning and detoxication of implant surface. Among these methods, air-powder abrasive and oversaturated citrate are known to be most effective. However, these treatments may deform implant surface. In this research, changes of surface roughness they were examined. 10 experimental machined titanium cylinder models were fabricated to be used for control groups. Each of them was air-powder abraded for 1 minute and they were named group 1. And then, group 1 were burnished with cotton pellets soaked with citrate for 30 seconds(Group 2), 1 minute(Group 3), 3 minutes(Group 4), and 5 minutes(Group 5) burnishing were applied for grouping respectively. Each group were examined with SPM, and their surface roughness were measured and analyzed. 1. Surface roughness of titanium decreased when it was air-powder abraded for 1 minute. It was statistically significant. 2. When Air-powder abraded titanium were treated with citrate for 3 minutes, Their surface roughness was the lowest. Titanium treated for 1 minute was the second lowest and 30 seconds was the third and titanium burnished for 5 minutes was the highest. 3. Surface roughness of titanium which was treated with citrate was decreased till 3 minutes, which was statistically significant. There was no statistical significance from 30 seconds to 1 minute and from 1 minute to 3 minutes, and there was statistical significance from 30 seconds to 3 minutes. 4. Oxide layer was formed when titanium is exposed to air, and it was removed when air-powder abraded. It was made when treated with citrate. It is thought that citrate treatment is necessary after the air-powder abrasion, and 1 minute is clinically and qualitatively adequate for burnishing time of citrate.

6.The Effect of Platelet Rich Plasma Combined with Bovine Bone on the Treatment of Grade II Furcation Defects in Beagle Dogs.

Current acceptable methods of promoting periodontal regeneration are basis of removal of diseased soft tissue, root treatment, guided tissue regeneration, graft materials, biological mediators. Platelet Rich Plasma have been reported as a biological mediator which regulate activities of wound healing progress including cell proliferation, migration, and metabolism. The purpose of this study is to evaluate the possibility of using the Platelet Rich Plasma as a regeneration promoting agent for furcation involvement defect. Five adult beagle dogs were used in this experiment. With intrasulcular and crestal incision, mucoperiosteal flap was elevated. Following decortication with 1/2 high speed round bur, degree II furcation defect was made on mandibular third(P3), forth(P4) and fifth(P5) premolar. 2 month later experimental group were PRP plus bovine bone and bovine bone only. After 4, 8 weeks, the animals were sacrificed by perfusion technique. Tissue block was excised including the tooth and prepared for light microscope with Gomori's trichrome staining. At 4 weeks after surgery, there were rapid osteogenesis phenomenon on the defected area of the Platelet Rich Plasma plus bovine bone group and early trabeculation pattern was made with new osteoid tissue produced by activated osteoblast. Bone formation was almost completed to the fornix of furcation by 4 weeks after surgery. In conclusion, Platelet Rich Plasma can promote rapid osteogenesis during early stage of periodontal tissue regeneration.

7.Study of Normative Gingival Proportion in Anterior Maxilla.

Tooth is the most important element in esthetic consideration on facial area. Tooth alignment which is in harmony with gingiva, lips, and face is also key element. The purpose of this study was to give a clinical discipline for restoration of gingival contour, which contains a ratio of maxillary 6 anterior teeth, research for gingival contour etc., in case of rehabilitation of maxillary anterior teeth. 300 Dankook university dental school students ,who is their twenties and free from periodontitis, participate in this study. The result was from each 70 males and females who had normal occlusion and tooth alignment. Length, width and length/width ratio of maxillary 6 anterior teeth were 0.74-0.81 in male, 0.81-0.84 in female. There was significant difference between male and female. The most deepest position of gingiva in maxillary central incisor and canine was located in distal part of teeth and maxillary lateral incisor was middle portion. In maxillary central incisors, gingival line of the most deepest point appeared significant difference between male and female (p<0.01) whereas there was no difference in lateral incisors and canines. Distance between interdental papilla apex and the most deepest portion appeared significant difference beween male and female. Distance of the deepest positon of gingiva is statistically significant except maxillary central incisors and lateral incisors. Standard deviation and mean of the width of labio-lingual were measured in cervical area and there was a significant difference both male and female.(p<0.01) From this result, we could get the mean of maxillary anterior gingival line and these results have great value in clinical guidance in studying maxillary anterior teeth.

9.Clinical Effect of MS Coat and Elmex gel on the Dentin Hypersensitivity following Periodontal Surgery.

Dentinal hypersensitivity is one of the complicated symptom rather than a disease. It has special reaction or pain over uncovered root by heat, mechanical or chemical stimulations, which are normal with healthy teeth. The purpose of this study is to discover rather MS Coat (oxalate-containing pre-polymerized resin suspension) or Elmex gel(amine fluoride+sodium fluoride) is effective on hypersensitivity after periodontal treatment using NRS(Numerical Rating Scales), which it is useful for evaluating pain level This study has been prepared in Dankook Dental Hospital with generally healthy adult who had been suffered from dentinal hypersensitivity after periodontal treatment. Divided in three groups with saline(control group), MS Coat(test 1 group) and Elmex gel(test 2 group). And then, following evaluations were made at the end of 1 minute, 1 week, 1 month and 3 months. 1. The sequence of higher frequency & severeness of hypersensitivity is water within 7degrees C, air stream & explorer. 2. As time goes on, 1 minute, 1 week, 1 month & 3 months, severeness of hypersensitivity scored lower with water, air stream and explorer. 3. With explorer, the differences among three groups as time had to seen. 4. With air stream, the sensitivity scored lower after 1min with MS Coat, Elmex, and saline sequence. As time goes on the sensitivity was lower with MS Coat and Elmex than saline, but there was no difference between MS Coat and Elmex. 5. With water in 7degrees C, the sensitivity was much decreasing with MS Coat and Elmex than saline, but there was no difference between MS Coat and Elmex. As the result, MS Coat and Elmex are effective on hypersensitivity caused by periodontal treatment

10.Hepatic Resection in Patients with Liver Metastasis from Gastric Cancer.

The clinical significance of hepatic resection for gastric metastases is controversial, even though hepatic resection has been widely accepted as a modality for colorectal metastases. Very few patients with gastric hepatic metastases are good candidates for hepatic resection because of multiple bilateral metastases, extrahepatic disease, or advanced cancer progression, such as peritoneal dissemination or extensive lymph node metastases. Therefore, several authors have reported the clinical significance of hepatic resection for gastric metastases in a small number of patients. Considering the present results with previous reports. The number and distribution of tumors in hepatic metastases from gastric cancer was considered based on the present and previous reports. Several authors have reported significantly better survival in patients with metachronous metastasis than in those with synchronous disease. However, metachronous hepatic resection necessitates the dissection of adhesions between the pancreas, liver, and residual stomach to prepare for Pringle's maneuver. Patients with unilobar liver metastasis, and/or metastatic tumors <4 cm in diameter may be good candidates for hepatic resection. Synchronous metastasis is not a contraindication for hepatic resection. Most of the long-term survivors underwent anatomic hepatic resection with a sufficient resection margin. After hepatic resection, the most frequent site of recurrence was the remaining liver, which was associated with a high frequency of mortality within 2 years. A reasonable strategy for improvement in survival would be to prevent recurrence by means of adjuvant chemotherapy and careful follow-up studies.
