2.Predictive Findings of Visual Outcome in Spectral Domain Optical Coherence Tomography after Ranibizumab Treatment in Age-related Macular Degeneration.

PURPOSE: To investigate which spectral domain optical coherence tomography (SD-OCT) findings predict visual outcome after anti-vascular endothelial growth factor (VEGF) treatment in neovascular age-related macular degeneration (NV-AMD). METHODS: We reviewed the medical records of patients with treatment-naive NV-AMD who underwent three or more consecutive anti-VEGF injections. The patients were divided into three groups according to their changes of visual acuity (VA); improved (group I), static (group S), or worsened (group W). We assessed the incidences and values of all available SD-OCT findings of these groups, compared these findings between the three groups and compared the initial values with the post-treatment values. RESULTS: Better initial VA and longer external limiting membrane (ELM) length were associated with less change in VA after anti-VEGF treatment. The initial VA was mildly correlated with initial photoreceptor inner and outer segment junction (IS/OS) length and initial ELM length. The final VA was also mildly correlated with the final IS/OS length and the final ELM length. VA was significantly changed after anti-VEGF treatment in groups W and I. With regard to incidence, disruption of the IS/OS (IS/OS-D), disruption of the ELM (ELM-D) and ELM length differed significantly between the three groups, particularly ELM-D. The incidences of IS/OS-D and ELM-D in group I were significantly lower than those in groups S and W, and those in group S were also lower than those in group W. The ELM length in group I was significantly longer than it was in groups S and W, and the ELM length in group S was longer than that for group W. However, these three findings did not change after the anti-VEGF treatment. CONCLUSIONS: Initial IS/OS-D, ELM length and particularly ELM-D can be useful predictors of the visual outcome after anti-VEGF treatment in NV-AMD patients.

3.A Web-Based Performance Assessment Model for OSCE (Objective Structured Clinical Examination) .

PURPOSE: OSCE, a performance assessment method using simulator and multimedia standardized patients can evaluate the student's clinical skills more effectively. It, however, lacks specific assessment criteria and methods for each station which OSCE consists of. Thus, in this research, we propose a web-based PBL (Problem Based Learning) performance assessment model which consists of the assessment criteria (Rubric) and the process for each OSCE station. METHODS: In our model, setting the assessment criteria (Rubric) makes the goal of problem- based learning clearer and the assessment procedure more objective by giving the concrete assessment measure. We decompose the PBL-based OSCE into five procedures: problem understanding, hypothesis clarification, the clarification of interaction role, the decision of performance assessment types, performance assessment testing. RESULTS: The web-based model was developed consisting of five corresponding areas: problem setting, assessment criteria (Rubric), community, performance assessment types (automatic production, template production), the performance assessment testing. CONCLUSIONS: A proposed web-based PBL model will provide the concrete guidelines for clinical skill and attitude learning by giving more objective and clarified assessment criteria and by increasing the quality of assesment through the continuous development of assessment criteria.

4.A Case of Progressive Zosteriform Macular Pigmented Lesion.

Progressive zosteriform macular pigmented lesion(PZMPL) is a chronic pigmentary dermatosis similar to progressive cribriform and zosteriform hyperpigmentation(PCZH). This dermatosis described by Simoes in 1980 is characterized by a uniformly tanned macular pigmented lesion in a zosteriform distribution preceded by multiple pruritic macular pigmentation in a part of the dermatome for a period. PZMPL is not a fully understood disease entity but it is thought to be a variant of PCZH. It is differentiated from PCZH by accompanying pruritus as a prodromal symptom, a characteristic clinical course, and histological findings such as pigmentary incontinence. We report herein a case of PZMPL in a 17 year-old girl with the pigmentary skin lesion extending from the left forearm to the left chest along the Blaschkos line. The histological findings revealed increased melanin pigments in the basal layer and focal pigmentary incontinence in the upper dermis. To our knowledge, this case is the first report of PZMPL in korea thought to be the same case reported by Simoes.

5.The Effects of Paracetamol, Ketorolac, and Paracetamol Plus Morphine on Pain Control after Thyroidectomy.

BACKGROUND: The aim of this study was to compare the efficacy of ketorolac, paracetamol, and paracetamol plus morphine on pain relief after thyroidectomy. METHODS: Eighty patients were randomly allocated to one of the 4 groups: normal saline (group C), ketorolac 30 mg (group K), paracetamol 1 g (group P), and paracetamol 700 mg plus morphine 3 mg (group PM). Each regimen was administered intravenously (IV) 30 min. before the end of surgery. If pain was not relieved, patients received an IV bolus of pethidine hydrochloride 25 mg. Pain intensity using a visual analogue scale (VAS) was recorded at 0.5, 1, 2, 4, and 6 hr after the end of surgery. RESULTS: VAS at 0.5 and 1 hr after the end of surgery were significantly lower in group K, group P, and group PM than in group C (P < 0.05). The number of patients receiving pethidine hydrochloride at 0.5 and 1 hr after the end of surgery was significantly lower in group K, group P, and group PM than in group C (P < 0.05). There was no significant difference among the groups in the incidences of adverse events associated with study medications and patient satisfaction (P > 0.05). CONCLUSIONS: Paracetamol 1 g IV possesses a similar analgesic efficacy to ketorolac 30 mg IV after thyroidectomy. Paracetamol may represent an alternative to ketorolac for pain prevention after mildly to moderately painful surgery in situations where the use of NSAIDs is unsuitable.

6.EEG Analysis of the Left and Right brain activities from cartoon learning versus text learning.

OBJECTIVE: Cartoons have been known to motivate learners and make learning process easier by combining verbal and visual effects. But they are mostly applied to motivate the less able learners, and have limits in delivering comprehensive information. Thus, more careful and scientific validation for the pros and cons of using cartoons for everyday use in various subjects is in need. METHODS: In this research, we used Electroencephalography(EEG) to compare cartoon learning and text learning by measuring four characteristic brainwaves including theta, alpha, sensory motor rhythms(SMR), and beta, from the left and right brain. The EEG signals acquired from 24 subjects are analyzed using the mean difference of the left and right brain and canonical correlation analysis. RESULTS: The theta brainwave of the left brain and right brain shows significant differences (p<0.05) from cartoon learning versus text learning in the theta brainwave while the other brain waves show similar patterns. CONCLUSION: Cartoon learning produced significantly stronger theta brainwaves than text learning implicating that cartoon learning reduces more focused attention, SMR brainwaves and beta brainwaves from the left brain explained cartoon learning and text learning process while alpha brainwaves explained those processes in the right brain.

7.Cure of severe hypothermia due to loss of consciousness after drug intoxication of inhaled hydrocarbons, Case 1.

A 19-year-old man had a good neurologic recovery from a severe hypothermia(19 degrees C) and a prolonged coma following active infernal rewarming. From inhaling hydrocarbons, he was left unconscious on the cold floor for 24 hours. As soon as the patient was brought into the emergency medical center, he was early evaluated arid treated aggressively. ECG showed Osborn(J) wave on all leads. The temperature o( patient was increased by 2-3 degrees C per hour through active external rewarming (by heating blankets and warm bag) and active infernal rewarming (by airway rewarming, warmed IV fluids, gastrointestinal tract irrigation, and bladder irrigation). The temperature reached 36 degrees C after 6 hours. Active infernal rewarming provides rapid core rewarming with the additional benefit of circulatory support during the period of cardiac instability.

8.Correlation of Nitric Oxide Concentrations in Follicular Fluid with the Ovarian Response as well as the Age in Women Undergoing Controlled Ovarian Hyperstimulation.

OBJECTIVE: To elucidate the correlation of nitric oxide (NO) concentrations in follicular fluid (FF) with the ovarian response as well as the age in women undergoing controlled ovarian hyperstimulation (COH). METHODS: This study was performed in 72 cycles of whom underwent COH for in vitro fertilization at infertility clinic of Changwon Hanmaeum Hospital and Pusan National University Hospital from Aug., 2003 to Dec., 2003. Each FF was collected at the oocyte retrieval and NO concentrations in FF were assessed. We analyzed the correlation of NO concentrations in FF with the ovarian response as well as the age in women undergoing COH. RESULTS: Among 72 cycles, the clinical pregnancy rate was 37.5% (27cycles). There were no significant differences in patient's age, ampules of gonadotropins used, estradiol level on the day of hCG administration, the number of oocytes retrieved, and fertilization rate between pregnant and nonpregnant groups. However, negative correlation was found between the follicular size and NO concentrations in FF. No significant correlation was found between age and NO concentrations in FF. Comparing NO concentrations in FF between pregnant and nonpregnant women by age, NO concentrations in FF were higher at nonpregnant women, significantly those of them more than 40 years old. CONCLUSION: These results suggest that NO concentrations in FF cannot be a marker for the ovarian response, and have no correlation with all parameters of COH including patients' age with exception for the follicular size.
