2.The Clinical Study on Pin and Plaster Method about open Tibial Shaft Fractures

Recently, the rates of various traffic accidents and industrial injuries accordlng to industrial progression were increased very much. In those injuries, open tibial shaft fractures were one of the most common ones of all long bones of the body. The authors have reviewed and analyzed 30 cases of the open tibial shaft fractures which were treated by pin and plaster method at our hospital from April 1976 to August 1979. The results were as follows: 1. Most of the fractures occurred in 20 and 30 age levels, average age was 34.4. Male 5 > female 1 in accident frequency. 2. The causes were traffic accident (67%), direct blow (17%), falling down from a height (13%), and mechanical trauma (3%) in order. 3. In location of fractures, Lt. 16 > Rt. 11 > both. 3; lower (47%), middle (33%), upper (20%), one thirds and comminuted (60%) > transvers (20%) > oblique (13%) > spiral (4%) > segmental fractures in frequency. 4. The associated injuries were fractures of fibulas (67%) > multiple skin damages (40%) > knee joint patella injuries... etc. Total of those injuries were 68.; One patient gained 2.26 kinds of those injuries simultaneously. 5. By classification of Ellis, minor severity (33%) > moderate one (40%) > major one fractures (27%). And according to results of pin and plaster treatment, bony union took roughly 11, 15, above 23 weeks respectively. 6. The summed contaminatlon and infection rate was below 20%, and cultured bacterias were E. Coli (50%) > staphylocci (33%) > pseudomonas (17%) and susceptabilities to penicillin were 60, 90, 25% respectively. 7. By classification of time interval from injury to apply pin and plaster method, early applied group (17 cases) showed bony union rate (7.6.4%), delayed one (11.7%), nonunion (5.8%), late group showed 50, 12.5, 6.3%, far late group showed 40, 40, 20% respectively. 8. Duration of the cast immobilization was 4–7 weeks and then it was changed by P. T. B. cast until about 4 monthes from injury, after it, vigorous P. T. was carried out. Good results were come in 80% (24 cases). 9. Pin and plaster method will be indicated in minor & moderate severity-fractures, but in major ones, it will be used as a preliminary stage operation.

3.Three Cases of Aneurysmal Bone Cysts

Aneurysmal bone cyst is an uncommon tumor-like lesion of the bone containing distended vascular channels or cvities filled with blood. These cases, twenty-nine old female, eighteen years old male and female patients complained of pain and swelling in Rt. Medial aspect of proximal foot, Lt. proximal tibial portion and Rt. entire talar aspect of foot, respectively. Roentgenograms showed ballooned-out distensions and eccentric buldging of those lesions and soapbubble like multiple radiolucent cystic changes within interiors of the bones at talar neck, Proximal tibia and entire talus. Those cases were treated with radical curettage and autogenous iliac bone grafts and snugly fitted long leg casts in functional positions, respectively. On 2nd postoperative day, Cast windows were made and daily sterile dressings were carried out until complete wound healing. Pathological Examinations grossly and Microscopically revealed typical Aneruysmal bone cysts in all 3 cases. All-Patients had good therapeutic recovery within about seven weeks post-operatively.

4.Percutaneous biliary drainage in acute suppurative cholangitis with biliary sepsis.

Acute suppurative cholangitis is a severe inflammatory process of the bile duct occurred as a result of partial or complete obstruction of the bile duct, and may manifest clinically severe from of disease, rapidly deterioration to life-threatening condition. We analyzed emergency percutaneous transhepatic biliary drainage in 20 patients of acute suppurative cholangitis with biliary sepsis to evaluate the therapeutic effect and complication of the procedure. The underlying causes were 12 benign diseases(stones) and eight malignant tumors and among eight malignant tumors, bile duct stones(n=4) and clonorchiasis (n=1) were combined. Percutaneous transhepatic biliary drainage was performed successfully in 17 of 20 patients improvement of general condition and failed in three patients. The procedures were preterminated due to the patient's condition in two and biliary-porto fistula was developed in one. After biliary decompression by percutaneous transhepatic biliary drainage, effective and successful elective surgery was performed in nine cases, which were seven biliary stones and two biliary cancer with stones. Our experience suggests that emergency percutaneous transhepatic biliary drainage is an initial and effective treatment of choice for acute suppurative cholangitis with sepsis and a safe alternative for nonsurgical treatment.

5.An unusual Cellular Blue Nevus involving Phalangeal Bone.

A 34-year-old female had an unusual case of cellular blue nevus which had behaved in a locally aggressive fashion. The lesion infiltrsted the adjacent soft tissue of left nail bed and extended into deep phalangeal bone. The involved left thumb was partially amputated because complete excision was impossible and we were quite concerned about malignant transformation. Histologically biphasic pattern without cellular atypism, a characteristic features of cellular blue nevus, was shown. About 2 years later, the lesion had not recurred.

6.CT findings of exophytic hepatocellular carcinoma.

We retrospectively evaluated the characteristic computed tomographic (CT) findings in nine patients with exohepatic hepatocellular carcinoma(HCC) pathologically prove by surgery(n=2) or percutaneous needle biopsy(n=7). The CT findings of exohepatic HCC were correlated with clinical findings and compared with those of usual HCC Lesions were in the left lobe lobe(n=7) and right lobe(n=2) of the liver. All lesions showed a well-marginated hypodense mass with capsular enhancement on enhanced CT scan. The patterns of capsular enhancement were complete in five and partial in four cases. The portal vein thrombosis was seen only in one case. There was no difference between exohepatic HCC and usual HCC in clinical findings such as increased α-fetoprotein(α-FP), positive Hepatitis B surface antigen(HBsAg), and underlying liver cirrhosis. In conclusion, the CT findings of exohepatic HCC were a well-defined hypodense mass with complete or partial capsular enhancement and these findings may be useful in differentiation from the tumors of adjacent organs.

8.An Investigation Study on the Role and Performance Ability of Physical Therapists in the Community Care System for Elderly

Purpose:
A survey was conducted to investigate the role, priority, and awareness of physical therapists in the community care system for the elderly.
Methods:
Nine hundred ninety-three physical therapists were invited to an online survey from 1st to 30th June 2020.
Results:
While the awareness of physical therapists for the community care system for the elderly was found to be low, the role priority and performance ability were found to be high. Fall training scored the highest for the job duties of physical therapists, and functional training scored the highest for the performance ability of physical therapists. For the role priority by the education status regarding the community care system, the participants who received the education program showed higher scores in the performance ability than those who did not. For the role priority and performance ability according to the education levels, higher education levels resulted in higher scores in both outcomes. Regarding the clinical experience, low scores were found in the participants with less than five years of experience.
Conclusion
Physical therapy establishes itself as an essential area of expertise in the community care system for the elderly. Therefore, these results can be utilized in developing a model for Korean community care for the elderly.

10.Treatment of Fractures of the Femur Neck in Children

We have treated total 37 cases of femoral neck fracture in children who were under the 16 years old in age at the Catholic University Medical College since 1985, followed them for average 21 months to analyse their types of fracture and treatment, complications and prevention of complications, and obtained the following results. 1. According to the Delbet's classification, type I was 1 case, II were 18(49%), most common, III were 10 and IV were 8 cases. Their ages were average 8.2 years and boys were predominant in 27 cases. 2. Thirty-six cases were treated with open reduction and internal fixation within 1-13 days after fracture, but only one type IV was treated with closed reduction, followed by hip spica cast for 6 to 9 weeks in all. 3. The fixation devises were removed at average 8.7 months after operatio. 4. fifteen complications were observed in 11 cases(37%), that were AVN(3 cases), early physeal closure(8 cases), coxa vara(3 cases), coxa valga and limb shortening(each 1 case), and their causes were severe fracture displacement(2 cases), penetration of fixation devices to physis(8 cases), incomplete reduction and loss of initial reductio (1 case) etc, but were not correlated with their method of fracture treatment. Conclusively, the femur neck fracture in children could favorably be treated with open reduction and internal fixation to reduce their complications. Physeal early closures due to penetration of screws and pin were most common cause of complications, and their long-term follow-up should be necessary.
