1.The Genetic and Clinical Studies of Ichthyoses.

This study was attempted mainly to estimate the frequency of ichthyoses and its genetic characteristics among general population in Chonnam Province. One hundredand thirtyone subjects heing comprised of 121 with ichthyosis vulgaris, 6 with X-linked recessive ichthyosis and 4 with lamellar ichthyosis visited the department of Dermatology, Chonnam National University Hospital from 1971 through 1980. All except 7 patients had visited the hospital with chief complaints of a skin problem other than ichthyoses itself. The frequency of ichthyosis vulgaris seems to be somewhere between, 1/300 and 1/400. 86% were found to either be sporadic cases or come from families from which not enough information was obtained to define a mode of inheritance. The fact that 43% of the sibling of affected patients developed ichthyosis vulgaris points out that it is a highly penetrant autosomal dominant, disease. Skipped generation was observed in 2 families, and gonadal mosaicism in 1 family. Atopic diseaaes were found only in 12.0% of cnses of ichthyosis vulgaris. The disease appeared in 81% of cases before 1 year af age. The skin lesions of ichthyoses were developed on the lower limhs, back, upper limbs, abdomen and scalp in the order of frequency.

2.A Study on the Effectiveness of Clocortolone Pivalate (Purantix) Gream on Certain Common Skin Diseases.

In order to evaluate the effectiveness of clocortolone pivalate(Purantix) on corticosteroid responsive dermatoses, a symmetical paired. comparison technique was performed using 0. 1% clocortolone pivalate and 0. 025% triamcinolone acetonide creams. During the pericd of 7 months from September 1977 to March 1978, 37 patients with bilateral lesions of contact dermatitis, seborrheic dermatitis atopic dermatitis, lichen simplex chronicus and psoriasis were selected and treated with afore mentioned drugs, In the 15 psoriatic patients, clocrtolone pivalate was superior in 3, triamcinolone acetonide was superior in 2, 4oth drugs were equally effective in 10. In the evaluation of all 37 patients clocortolone pivalate was superior in 8, triamcinolone acetonide was sureior in 3, both drugs were equally effective in 26. The overall superiority of clocortolone pivalate to triamcinolone acetonide was non significant, There was no side effects with either drug. The simplicity of the vasoconstriction tcst has led to its general use in the study of the potency of new topical steroids. In the 6h occluded single-application vasocon-strictor test performed on 40 healthy students, there was no significant difference between clocortolone pivalate and triamcinolone acetonide, but they significantly d.ifferent from 1% hydrocortisone and the control; a simple ointment base(p<0.01).

3.Arthroscopy after Meniscectomy of the Knee: The Posterior Horn in 40 Cases

Diagnosis of the disorders following menlscectomy is occasionally difficult, as ther has frequently been involved many causes other than the torn menisci. The retained fragments of the menisci have been blamed to be responsible for postoperative knee disorders. Arthroscopic examination was carried out in 40 knees after meniscectomy and the frequency of retained posterior horn in relation to the postoperative trouble was observed. 1. In 11 out of total 40 knees, the retained posterior horn could be diagnosed, consisting of 27.5% of all. 2. Only flve of these 11 knees showed some kinds of postoperative disorders,while the reat had no trouble. 3. Those knees in which the excision of the posterior horn was incomplete, were often associated with Irregular healing pattern of the local tissue, which might be closely related to the occurrence of the secondary Intraarticular change.

4.Mechanism for Toxicity of Cytolysin Produced by Vibrio vulnificus.

The extracellular cytolysin produced by V. ulnificus has gained great attention as a causative factor for the pathogenesis of V. vulnificus-infected disease. In this study, an attempt was made to elucidated the mechanism for the toxity of cytolysin. 1. Cytolysin given to mouse showed lethal activity with LD of 3.7 HU and produced hemoconcentration, suggesting that its lethal activitis attributable to the increased vascular permeability. 2. The lethal activity of cytolysin for mouse was prevented by intraperitoneal administration of clemastin or verapamil one hour before intravenous injection of cytolysin. 3. Treatment of peritoneal mast cells from rats with cytolysin stimulated histamine relesse irrespective of the prescence of extracellular Ca and the stimulatory effect of cytolysin was not affected by the pretreatment of mast cells with per tussis toxin. 4. The hemolytic activity and stimulatory effect of cytolysin on histamine release from mast cell were inhibited by cholesterol. The results suggest that the lethal activity of cytolysin is attributable to the increased vascular permeability which is resulted from the stimulation of histsmine release from mast cells and/or basophils by cytolysin.

5.The Distally Based Superficial Sural Artery Flap.

The superficial sural artery flap is supplied by the superficial sural artery that accompanies the sural nerve. The superficial sural artery connects distally with a peroneal artery septocutaneous perforator via a suprafascial network of vessels that permits the flap to be raised on its distal pedicle. We treated 10 patients who showed soft tissue defect of the lower third of the leg, around the ankle and the hindfoot with this flap. The causes of the soft tissue defect were trauma in 6 cases, pressure sore in 2 cases, squamous cell carcinoma in 1 case and osteomyelitis in 1 case. The sites of the soft tissue defect were the lower third of the leg in 5 cases, around the ankle in 2 cases and the hindfoot in 3 cases. The size of the soft tissue defect was from 5x6 cm to 8xl2 cm. All 10 flaps survived. Two flaps showed slight venous congestion which cleared in a few days. The time for flap dissection was 15 minutes in average. The results of our cases show that the distally based superficial sural artery flap is useful for the soft tissue coverage of the lower third of the leg, around the ankle and the hindfoot. The advantages of the flap are that the blood supply is reliable, elevation is easy and quick, the major arteries are not sacrificed and the donor site morbidity is negligible.

6.Accessory Axillary Breast Tissue.

We have experienced a case of the more unusual form of supernumerary breast tissue which is characterized by the presence of aberrant gland tissue alone. The case was a 30-year-old female patient in puerperium with bilateral axillary masses of seven years duration. Gentle pressure over the right-sided one caused milk to exude from a skin pore. Histopathologic findings of the tumors revealed the lobules of the breast forming islands of glandular tissue.

7.A Case of Kyrle's Disease in a Patient with Chronic Renal Failure.

We present a case of Kyrle's disease. The pstient was a 48-year-old male who had been undergoing hemodialysis for 6 years due to chronic renal failure caused by polycystic kidney. The lesions were hyperkeratotic papules on the lower legs and abdomen witb 3 months' duration. Histopathologically, the epidermis showed central invsgination filled with heavy keratotic, partly parakeratotic plug, basophilic debris and neutrophils. There were a few premature keratinized cells below the parakeratotic plugs and complete epidermal perforation was present.

8.Percutaneous Drainage of Lung Abscess and Infected Bulla.

BACKGROUND: Antibiotic therapy has proven an effective method of treatment on the majority of patients with pyogenic lung abscess and infected bulla. When medical therapy has failed, pulmonary resection is the current generally recommended therapy. But nowdays complications of percutaneous tube drainage has decreased with the use of small catheter. So we evaluated the effect of percutaneous tube drainage as an alternative therapy to the pyogenic lung abscess and infected bulls refractory to medical therapy in preference of the pulmonary resection. METHOD: Nine cases of the lung abscess and three cases of infected bulls which has large cavity size over 6cm, and has underlying diseases such as lung cancer, diabetes mellitus, refractory to over 1 week of antibiotics, were performed percutaneous tube drainage with All Purpose Drainage catheter(Medi-tech, Watertown, USA) under fluoroscopy. RESULTS: All the cases except one case which complicated empyema was improved clinically. Fever was down within 4days of percutaneous tube drainage(mean : 1.9days). Mean duration of tube drainage was 9.9days. CONCLUSION: Percutaneous tube drainage is an effective and relatively safe procedure in the management of lung abscesses that do not response to medical therapy We speculate this procedure should be considered as an alternative therapy for the lung abscess refractory to medical therapy in preference to the surgery. The safety and effectiveness of this procedure in infected bulla should be evaluated with an additional study.

9.Multiple Trichoepithelioma Associated with Milia.

We present a case of multiple trichoepithelioma associated with milia. A 12-year-old girl developed symmetrically distributed, numerous, skin-colored papules on her face, some of which were studded with milia-like whitish materials. Histopathological findings of these lesions revealed the typical findings of trichoepitheliomas with overlying milia in the upper dermis.

10.Effects of pH Differences on the Skin Moisturizing Capacity of Natural Moisturzing Factors.

The flexibility of surface layer of skin is largely dependent upon the layer's water content. This, in turn, depends primarily on the quantity of water soluble hygroscopic and surface active msterials within the straturn corneum, often known as the natural moisturizing fsctors(NMF). It consist of amino acid, lactic acid, pyrrolidone carboxylic acid and inorganic salts etc.. Some of these components are greatly influenced by hydrogen ion content and ionic form of them were found to be highly hygroscopic. In this experiment, sunburn exfoliated human stratum corneum and pig epidermis separated by 2N-NaBr were used as in vitro test. These tissues were so treated to hsve each pH value (3, 5, 7, 9) and their water holding capacity and hygroscopicity were measured by gravimetric method. The results are summarized as follows : 1, Water holding capactiy of sunburn exfoliated human skin showed significant increase at pH 5 (p<0.1), 7 (p<0.01), 9 (p<0.01) compared to pH 3, and increase in pH 7 (p<0.1), 9 (p<0.05) compared to pH 5 in lactic acid. In pig epidermis, there was significant increase at pH 5 (p<0.05), 7 (p<0.01), 9 (p<0.05) cornpared to pH 3 in lactic acid. 2. Hygroscopicity of pig epidermis showed significant increase at pH 5 (p<0.05), 7 (p<0.1), 9 (p<0.05) in lactic acid and at pH 5
