5.Correlation of Nitric Oxide Concentrations in Follicular Fluid with the Ovarian Response as well as the Age in Women Undergoing Controlled Ovarian Hyperstimulation.

OBJECTIVE: To elucidate the correlation of nitric oxide (NO) concentrations in follicular fluid (FF) with the ovarian response as well as the age in women undergoing controlled ovarian hyperstimulation (COH). METHODS: This study was performed in 72 cycles of whom underwent COH for in vitro fertilization at infertility clinic of Changwon Hanmaeum Hospital and Pusan National University Hospital from Aug., 2003 to Dec., 2003. Each FF was collected at the oocyte retrieval and NO concentrations in FF were assessed. We analyzed the correlation of NO concentrations in FF with the ovarian response as well as the age in women undergoing COH. RESULTS: Among 72 cycles, the clinical pregnancy rate was 37.5% (27cycles). There were no significant differences in patient's age, ampules of gonadotropins used, estradiol level on the day of hCG administration, the number of oocytes retrieved, and fertilization rate between pregnant and nonpregnant groups. However, negative correlation was found between the follicular size and NO concentrations in FF. No significant correlation was found between age and NO concentrations in FF. Comparing NO concentrations in FF between pregnant and nonpregnant women by age, NO concentrations in FF were higher at nonpregnant women, significantly those of them more than 40 years old. CONCLUSION: These results suggest that NO concentrations in FF cannot be a marker for the ovarian response, and have no correlation with all parameters of COH including patients' age with exception for the follicular size.

6.Sacral Perineural Cyst with Lumbar Disc Herniation: A Case Report.

STUDY DESIGN: A case report is presented of a symptomatic sacral perineural cyst with lumbar disc herniation that has reported a few case in the literature. We report the case of symptomatic perineural cyst with lumbar disc herniation. SUMMARY OF LITERATURE REVIEW: Symptomatic sacral perineural cyst is rare and some cases of sacral perineural cyst associated with disc herniation were reported. But it is not well known that the correlation between clinical symptoms caused by mass itself and concomittant lumbar disc herniation. METHODS: A 34-year-old male presented with low back pain, both leg dysesthesia and bilateral radiating pain. Clincal sign and symptom, physical examination, diagnostic imaging studies including MRI, myelography and pathologic findings of surgical specimens were evaluated. RESULTS: After the decompressive partial laminectomy and discectomy, the patient who had diapnosed with herniated nucleus pulposus of L4-5 and L5-S1 complained residual both leg paesthesia and hypoesthesia. Patient's clinical symptom was correlated with sacral perineural cyst. So we removed the cyst. Postoperatively neurologic symptoms and signs were completely relieved. Pathologically, Sacral perineural cyst composed of meningoepithelial cell and neural tissue within cystic wall were confirmed. CONCLUSION: We could find that sacral perineural cyst sufficiently evoked a clinical symptom. So, careful observation was needed in that case of sacral perineural cyst associated with disc herniation.

7.Evaluation of Macular Circulation in Patient with Diabetic Maculopathy using Scanning Laser Ophthalmoscope(SLO).

In the condition of diabetic retinopathy, the vascular changes are localized primarily in the retinal capillaries and are presumed to promote angiogenesis. To investigate the change of retinal blood flow velocities and morphological parameters in diabetic retinopathy, we measured perifoveal capillary blood velocities(v) and the size of foveal avascular zones(FAZ). Thirteen patients with diabetic maculopathy and nine healthy volunteers were included in this study. The scanning laser technique in conjunction with an image analysing system were used to assess the morphological and hemodynamic changes in diabetic retinopathy. Diabetic maculopathy group showed a slower capillary blood velocity than normal group(2.44+/-0.39mm/sec vs2.75+/-0.61 mm/sec, p>0.18). The foveal avascular zone was significantly larger in diabetic maculopathy group than in normal group(313.5+/-64.6micrometervs. 238.9+/-93.8micrometer, p<0.05). This results indicate that the retinal microcirculation is altered in diabetic patients compared with healthy subjects. These alterations may be due to the change of the capillary wall and blood viscosity in diabetic patients. The determination of these parameters can be utilized in monitoring the progress of diabetic maculopathy.

8.A Case of Squamous Cell Carcinoma Arising in the Mature Cystic Teratoma with Direct Invasion to Transverse Colon and Jejunum.

Malignant degeneration of mature cystic teratoma has been reported in 1~3% of cases, usually between the age of 30 and 70 years with a peak incidence of 40~60 years. The most common malignancy developing in such tumors is squamous cell carcinoma arising in a mature cystic teratoma. Hirakawa reported two patients with benign teratomas diagnosed 25 and 32 years prior to surgery for malignancy and Dorothea reported a patient who was diagnosed as benign teratoma 50 years prior to operate for carcinoma. But there has been no report of squamous cell carcinoma arising in the mature cystic teratoma with direct invasion to gastrointestinal tract. We report a case of squamous cell carcinoma with direct invasion to transverse colon and jejunum in 62-year-old female who was diagnosed as mature cystic teratoma 20 years ago.

9.A Case of Traumatic Globe Luxation.

PURPOSE: The globe luxation is a rare phenomenon in which there is a forward displacement of the eye ball so that the eye lids spasmodically close behind it. In Korea, no case has been reported. The authors experienced a case of globe luxation by laceration of the conjunctiva with trauma. METHODS: The authors experienced a case of globe luxation in 64-year-old male who had crushed with infantile marry-go-round handle and complained proptosis and ocular pain. Ocular examination revealed best corrected visual acuity of right eye 0.1, IOP 44mmHg, conjunctival hyperemia and laceration, extraocular muscle limitation in all direction. Exophthalmometer shows right eye 23mm, left eye 19mm. Orbital CT yielded proptosis and globe luxation without avulsion of internal and external rectus muscle and optic nerve. RESULTS: After the lateral canthotomy, we successfully treated with the corticosteroid therapy without visual impairment or eyeball movement disorder. we report a case of globe luxation with the review of literature.

10.A Case of Acral Persistent Papular Mucinosis.

Acral persistent papular mucinosis is a subtype of the localized papular mucinosis. Clinically, ivory to flesh-colored papules develop exclusively on the back of the hands, the extensor surface of the wrists, and occasionally the distal forearms. Histologically, mucin accumulates in the upper reticular dermis, typically sparing a subepidermal zone and fibroblasts are not increased in number. On experiencing a case of numerous papules on both hands and forearms of a fifty year-old woman, we present it as acral persistent papular mucinosis.
