2.Pathology of Cardiac Anomalies and Systemic Edema in the Murine Fetus with Trisomy 16.

BACKGROUND: Trisomy 16 in mice is considered to be an animal model for Down's syndrome in human. We studied the morphologic characteristics of the heart and the edema, and their significance to the teratogenesis in this animal model. METHODS: A total of 30 dams were sacrificed to bear 125 (61.9%) normal fetuses 35 (17.3%) abnormal fetuses and 42 (20.8%) resorptions. Cytogenetic study and morphological examination were performed using microdissection agar-mount sectioning histologic examination and scanning electron microscope. RESULTS: The crown-rump length was significantly shorter in abnormal (trisomic fetuses) than the normal (eusomic) fetuses. Trisomic fetuses showed massive edema at the back from the vertex to the lumbar area. Four-chamber view section of the agar-mount and histologic section showed a common atrioventricular valve bridging the left and the right atrioventricular junction. Scanning electron microscopic examination on atrioventricular valves showed three types of atrioventricular valves:five cases with common atrioventricular orifice, three cases with partitioned atrioventricular orifice and two cases with atresia of the right atrioventricular orifice. CONCLUSION: This study confirmed the presence of massive edema and cardiac anomalies in the mouse with trisomy 16. But there was morphologic diversity of cardiac anomaly in this model.

4.A Case of Netherton's Syndrome.

Nethertons syndrome is characterized by a triad of ichthyosiform dermatosis, multiple hair-shaft defects(including trichorrhexis invaginata), and an atopic diathesis. Intermittent amminoaciduria, mental retardation, or recurrent infection have been observed in some cases. We have seen an 8-year-old girl presenting with polycyclic, erythematous patches bordered by distintive double-edged scales as a skin manifestation and ball-and-socket deformity as a hair defect.

5.In Vivo Delayed Hypersensitivity to the Recall Antigens in Different Dermatoses.

Authors have performed patch test to evaluate the differences between the allergic contact dermatitis to topical medicaments and to the other substances including cosmetics. One hundred eighteen patients were evaluated according to age, sex, and causative agents of the disease. Furthermore, these patients were analysed as to the causative allergic contact factors by clinical inspection and history. The most common causative allergic contact factors by clinical, history and patch test were topical medicarnents(45.8%), cosmetics(24. 6%), rubbery(10.2%), and metals(8.59%.). The common etiogloic medicaments of contact dermatitis were Madecassol, Betadin, Tiger balm, Canesten, Mercurochrom, and Tego solution. The causative agents were confirmed by the patch test with Hollister-Stiers standard allergens in 57 patients from Jan. 1983 to Dec. 1984, 1 hirty eight among 57 patients were positive to patch test, Back ground possible explanation of the patch test results were discussed and a comparison was made between this report and others.

Twin reversed arterial perfusion (TRAP) sequence is a rare specific anomaly of twin gestation with fused placenta and umbilical anastomosis. This syndrome occurs once in about 34,600 births and reported first by Beneditti in 1533. We report on prenatal diagnosis of a case of TRAP sequence with color Doppler sonography, this case is presented with a brief review of the literature.

7.Effects of Health Disorder Signals on Irritable Bowel Syndrome and Urination Disorders: Focusing on Door-to-Door Salespeople and Women Who have Experienced Symptoms of Irritable Bowel Syndrome

Purpose:
This study investigated the effects of Health Disorder Signals on irritable bowel syndrome (IBS) and urination disorders, and the relationship between IBS and urination disorders.
Methods:
A questionnaire survey was conducted on those who experienced symptoms among door-to-door salesmen and women who visited S Hospital in Busan, and irritable bowel syndrome and gastrointestinal symptoms were surveyed based on the Roman Standard II.
Results:
The study results showed that the IBS Health Disorder Signal had a significant positive (+) effect on the IBS. It was also found that the Health Disorder Signal of urination disorders showed a certain pattern of urination disorders on the back of the hand. IBS was also found to have a significant positive (+) effect on urination disorders (Health Disorder Signals of urination disorders, urination disorders).
Conclusion
First, this study is Korea's first attempt at understanding Health Disorder Signals, and it is of great help in conducting a wide range of studies on such Health Disorder Signals in the future. Second, it is a surprising result confirming that bowel disease can have a direct effect on urination disorders.
8.Persistent high serum bilirubin level after percutaneous transhepatic biliary drainage: analysis of 32 cases

The aim of percutaneous transhepatic biliary drainage(PTBD) is to decrease serum bilirubin level and promoteliver function in patient with biliary tract disease, especially obstruction by malignant disese. But somepatients showed persistent high serum bilirubin level or higher than pre-PTBD level. Percutaneous transhepaticbiliary drainage was performed in 341 patients of obstructive jaundice for 5 years from July, 1981 to July, 1986at departement of radiology, Seoul Natinal University Hospital. Follow up check of the serum bilibrubin level waspossible in 188 patients. Among them the authors analysed 32 patients who showed persistent high serum bilirubinlevel after PTBD. The results were as follows: 1. The male to female ratio was 3.4:1 and the age ranged from 33 to 75. 2. The causes of obstructive jaundice included 30 malignant diseases and 2 benign diseases. Malignant diseasewere 16 cases of bile duct carcinoma, 7 cases of pancreatic cancer and 7 cases of matastasis from stomach, colonand uterine cervix. Benign disease were 1 cases of common hepatic duct stone and 1 case of intrahepatic ductstones. 3. The msot common level of obstruction was trifurcation in 17 cases. 4. The most common indication ofPTBD was palliative drainage of obstruciton secondary to malignant tumor in 28 cases. 5. Change of serujmbilirubin level ratio(post-PTBD level) was 1.28, 1.22, 1.38, 1.51 in serial period of 1–3 days, 4–6 days, 1–2 week 2–3 week after PTBD. 6. Causes of persistent high serum bilirubin level after PTBD were 12 cases of partialdrainage of intrahepatic bile, 13 cases of hepatic dysfunction including 9 cases of metastatic nodule, 2 cases ofbiliary cirrhosis, 2 cases of multiple liver abscess, and 7 cases of poor function of cather including 4 cases ofhemobilia, 1 case of multiple intrahepatic stones, pyobilia and intraheptic Clonorchis sinensis.

9.Attenuation of endothelial relaxation in umbilical arteries from preeclampsia patients.

OBJECTIVES AND METHODS: To directly examine the function of the endothelial cell(EC) and smooth muscle cell in umbilical arteries acquired from preeclampsia patients between June 1998 to November 1999, using a conventional tension measurement and bioassay experiment. RESULTS: Relaxation responses to EC-dependent relaxing agents including bradykinin and A23187 in human umbilical artery rings were significantly decreased in preeclampsia(p<0.01). Relaxation responses to EC-independent agents(SNP and SNAP) were also inhibited in umbilical artery rings acquired from preeclampsia patients(p<0.01). To test the change of endothelial cell function in preeclampsia without involvement of smooth muscle dysfunction, we used human umbilical artery and rabbit femoral artery as a donor and detector, respectively, in bioassay experiment. Relaxation responses to EC-dependent agents(A23187 and bradykinin) showed similar results to conventional tension measurement (p<0.01). Relaxation responses to 8-bromo-cGMP in human umbilical artery rings were also significantly decreased in preeclampsia(p<0.01). CONCLUSIONS: It can be concluded that increased vascular resistance in preeclampsia is not only due to the disturbance of endothelial function, but also to smooth muscle dysfunction.

10.A Serial MR Imaging of Myocardial Infarction with Non-Surgical Animal Model.

Objectives: The purpose of study is to evaluate serial MR imaging of myocardial infarction using non-surgical model of myocardial infarction after percutaneous transcatheter coronary arterial embolization in dogs. MATERIALS AND METHODS: We evaluated serial pre- and post-contrast MR images with Gd-DTPA (gadolinium-diethylenetriamine-pentaacetic acid) of heart of the eleven mongrel dogs (immediate group (n=3), one week group (n=3), three weeks group (n=5)) after making non-surgically induced myocardial infarction. We confirmed the infarct with TTC staining and microscopically. The location and extents of the myocardial infarction were correlated. RESULTS: A total of 24 MR images were archived; 11 images of the immediate post-embolic period, 8 images of one-week follow-up, and five of 3-week follow-up images. Comparing with the signal intensity of normal myocardium, immediate post-embolic MR images showed low or iso signal intensities (SI) of the infarct area on T1-weighted images (T1WI) and high SI on T2-weighted images (T2WI). No contrast enhancement with Gd-DTPA was made in all cases of the immediate post-embolic MRI. One-week and 3-week follow-up MR images showed low or iso SI on T1WI and slight high or iso SI on T2WI. Contrast enhancement images in both one-week and 3-week follow-up MRI showed denser enhancement of infarct area in one-week follow-up. The myocardial wall thinning was seen in 5 of eight dogs after one week and in 3 of five after 3 weeks. CONCLUSION: In non-surgical animal models of myocardial infarction, MR images showed low or iso SI on T1WI, and high SI on T2WI in various stages, and contrast enhancement was maximum after one week and gradual decrease to 3 weeks. The myocardial wall thinning was seen in one-to 3-week follow-up MR images.
