2.Comparative Studies on the Effects of Experimental Cryopexy and Diathermy Coagulation on Rabbits ERG.

Twelve albino rabbits were treated with cryoretinopexy on the left eye and surface diathermy coagulation on the right eye at the equatorial region in two parallel rows concentric with the limbus. Cryopexy was applied for 10 seconds with Krwawicz's probe (the diameter of the tip was 1.5 mm) after cooling with dry ice and alcohol mixture. Diathermy coagulation was performed with 40 mA currents for 2 seconds with needle electrode without perforating the sclera. After the treatment, ERG was recorded with varying length of intervals up to 30 days. The light stimulus was given by 100 watts incadenscent lamp and the illumination at the eye was 800 lux, its duration being 20 msec. Only the sizes of the b-potentials after 30 minutes of dark adaptation were considered. The patterns of the b-potential changes after cryopexy were roughly divided into 3 types. The first type was seen in 5 eyes, and it consisted of abrupt reductions, followed by slight augmentations, never reaching the pre-treatment level of b-wave even after 30 days. The second type (seen in 3 eyes) showed initial slight decreases followed by marked increases and remained supernormal at the end of 30 days. The third type(4 eyes) showed marked increases from the early post-operative days and returned to the original level after 30 days. These changes were demonstrated graphically in figures 3, 4 and 5. By diathermy coagulation, the same kinds of patterns were observed. The same animal, however, did not show the same pattern as with the cryopexy. The final b-potential after 30 days were 105.4% of the pre-operative value by cryopexy and 82.7% by diathermy coagulation. Thus the cryopexy produced less severe changes in ocular tissues than the diathermy coagulation. The histological observations performed also supported this conclusion.

4.Studiedies on the Pollution Bacteria in the River Water of Baek Ma.

The purpose of this study was to survey bacteria which was known as a fecal bacteria living in the human and animal gastro and intestinal tract. The colony density of the fecal coliform and fecal streptococci was 280 and 130 in 100ml sample amounts respectively. Staphylococcus aureus and Pseudomonas aeruginosa which is usually exist in swimming pool and was known as indicator of the pool water contamination was detected.

5.Operative Methods of Chronic Anteromedial Rotatory Instability of Knee: Advancement and Bony Reinsertion Operation of Tibial Collateral Ligament

Chronic anteromedial rotatory instability has not been demonstrated to have a satisfactory solution and we have experienced recurrence of the instability after operations. So the authors devised a new operative technique for the TCL (Tibial collateral ligament), that is, burying of the torn end of the ligament beneath the “ㄷ” shaped bony flap. It would better be called advancement and bony reinsertion operation. And we performed modifed MacIntosh tenodesis for the torn anterior cruciate ligament. The results were as follows: l. Of six knees, four (66.6%) rated “good”, two (33.3%) “fair”, and none (O%) “poor” (follow-up period; at least 6 months). 2. The technical problem of the advancement and bony reinsertion operation was discussed.

6.A Case of Cheolitis Granulomatosa.

Cheilitis granulomatosa is one of the rare dermatoses which is characterized by a sudden onset and a progressive course termination in chronic enlargement of the lips Usually, enlargement of lip is the only symtom without ulceration, fissurign or scaling. It may be considered a part of the Melkersson-Rosenthal syndrome and the etiology is still unknown. A 35 year old female with the clinlcal symptoms of cheilitis granulomatosa is presented; the diagnosis was confirmed by histopathologic study.

7.Effect of Cryotherapy with Liquid Nitrogen on Alopecia Areata.

BACKGROUND: Alopecia areata is replete with data from studies using variuos therapeutic approaches, none of which is clearly superior to another. OBJECTIVE: This study was designed to evaluate the therapeutic effect of cryotherapy in alopecia areata. METHODS: Thirty three patents with AA(M:13, F:20) well conduted in this study, precluding 3 patients with the moderate and extensive lesions. Cryotherapy using a cotton swab dipped with liquid nitrogen was applied to the involved areas, repeated with 2 times of freeze thaw cycle of two seconds. In 13 cases of AA multiplex, the lesions were divided into two groups, each of which was undertaken cryotherapy and intralesional injection of triamcinolone(3mg/ml) for evaluation of their respeetive effects. The treatment was done weekly method of 1 to 11 times, Th therapeutic response was determined as the regrowth of terminal hairs on the treated area(s) at 12 Weeks. RESULTS: Of the 33 patients with AA, the overall responders were 22(66.7%). There seemed good response rates of 70.0 % in females, 71.4 % in AA multiplex and 72.2 % in third decade as well as 72.4 % in those with lower than 1 year of duration. The remarkable resionse had 3-4 times in treatment numbers. There were no untoward side effects in patients except mild erythema. In the 13 patients treated with both modalities, cryotherapy had a slightly higher esaonse than triamcinolone therapy (69.2% vs 61.5%, p>0.05 by x-test). CONCLUSION: The effect of cryotherapy therapy in AA patients is not less than the intralesional therapy of triamcinolone. In addition, cryotherapy may be recommendec as the new, safe therapeutic modality.

8.The Distally Based Superficial Sural Artery Flap.

The superficial sural artery flap is supplied by the superficial sural artery that accompanies the sural nerve. The superficial sural artery connects distally with a peroneal artery septocutaneous perforator via a suprafascial network of vessels that permits the flap to be raised on its distal pedicle. We treated 10 patients who showed soft tissue defect of the lower third of the leg, around the ankle and the hindfoot with this flap. The causes of the soft tissue defect were trauma in 6 cases, pressure sore in 2 cases, squamous cell carcinoma in 1 case and osteomyelitis in 1 case. The sites of the soft tissue defect were the lower third of the leg in 5 cases, around the ankle in 2 cases and the hindfoot in 3 cases. The size of the soft tissue defect was from 5x6 cm to 8xl2 cm. All 10 flaps survived. Two flaps showed slight venous congestion which cleared in a few days. The time for flap dissection was 15 minutes in average. The results of our cases show that the distally based superficial sural artery flap is useful for the soft tissue coverage of the lower third of the leg, around the ankle and the hindfoot. The advantages of the flap are that the blood supply is reliable, elevation is easy and quick, the major arteries are not sacrificed and the donor site morbidity is negligible.

9.A Case of Erythema Multiforme - like Eruptions due to Contact with Lacquer Tree.

Irritant or allergic contact dermatitis presents usually eczematous process or occasionally noneczematous lesions like in this case. We report a case of erythema rnultiforme-like eruptions in a 40-year-old female who presented multiple iris or annular shaped maculopapular eruptions on both hands and forearms after contact of lacquer tree. Being performed with standard patch test screening series, lacquer tree extracts and urushiol, she showed significant reactions to the lacquer tree and urushiol. Histopathologic findings showed both features of erythema multiforme and suh acute contact dermatitis.

10.T Cell Epitope Analysis of Structural Protein of Adenovirus.

Thelper (Th) cells play a pivotal role in the regulation of immune responses. Since Th epitopes in adenovirus have not yet been defined, in this study, it was attempted to search for Th epitopes of adenovirus antigens that are restricted by MHC class II (H-2E). Among candidate viral proteins to be screened for Th epitopes, structural protein was selected, since they induced strong IL-2 release from adenovirus immune lyrnph node (LN) cells and the presence of E1 protein, which contains immunodominant cytotoxic T lymphocyte epitopes, did not potentiate the T cell responses. To confirm the presence of Th epitopes in the structural protein, virions were trypsinized and the resulting polypeptides whose molecular weights were lower than 5,000 were fractionated by HPLC. Some of the HPLC fraction turned out to induce LN cell proliferation. Ten synthetic peptides were designed as candidate Th epitopes from the primary amino acid sequences of adenovirus hexon and penton protein which are major constituents of the virion. The selected sequences share the common features of other known H-2E' binding ligands. Among these ten synthetic peptides, peptide of hexon protein amino acid residue 709-721 induced noticeable proliferation of LN cells from preimmune mice, and also able to induce IL-2 secretion from adenovirus-specific T hybridomas, suggesting that the peptide was the most immunodominant Th epitope. Hexon protein 221-233 and hexon protein 676-688 are considered as epitopes also. This study revealed three epitope sequences from adenovirus structural protein that are presented by class II MHC, H-2E.
