1.Acetabular Erosion after Peplacement of Femoral Head

Acetabular erosion, intrusion of the prosthesis into the pelvis, is the most serious late complication of hemiarthroplasty. For the detailed observation on this lesion, the authors reviewed 55 patients who underwent femoral head replacement and were followed up to at least 1 year after operation. The acetabular erosion were present in 18 patients(32.7%) out of the total 55 patients. Based on the results of this study, the authors presented a new classification of acetabular eorsion: Stage I(erosion of the cartilage only) was seen in 11 patients, stage II (evidence of the initial bony erosion) in 4 patients,and stage III(intraosseous intrusion of the metallic head) in 3 patients. Revision arthroplasty was obligatory in two of the three stage III patients. The severity of symptoms did not always correspond with the radiological changes. The acetabular erosion was more common in younger patients, in male patients and in patients with avascular necrosis of femoral head than in older patients, in female patients, and in patients with femoral neck fracture. The tendency of acetabular erosion occurring mainly in younger and male patients is probably attributed to their better survival and greater activity.

3.A case of orbital Polyarteritis Nodosa Complicated with Retinal Vasculitis and Exophthalmos.

PURPOSE: Polyarteritis nodosa is a necrotizing vasculitis affecting medium and small sized arteries throughout the vascular system, including ocular tissues. We report a case of orbital polyarteritis nodosa complicated with retinal vasculitis and exophthalmos. METHODS: A 51-year-old woman complained of swelling and hard mass in periorbital region for 3 months. The left eye was anophthalmos after enucleation secondary to corneal infection. The patient has treated marginal ulcer of the cornea and keratomalacia. Posterior synechia and motility disturbance of lateral rectus muscle were also revealed. Fundus examination showed focal retinal hemorrhage, macular hard exudate, and soft exudate, which may mean retinal vasculitis. Both eyes gradually developed exophthalmos. Orbital CT scan revealed homogenous infiltration in the orbit, ethmoid bone, and nasal cavity and perforation of the nasal septum. The pathologic examination for the biopsy specimen from the orbital tissue demonstrated a nongranulomatous vasculitis, diagnosing the polyarteritis nodosa. RESULTS: The authors present ocular manifestations, including marginal corneal ulcer, keratomalacia, dysfunction of the extraocular muscles, posterior synechia, retinal hemorrhage, soft exudate, hard exdate in the macula, and exophthalmos, in this patient with the polyarteritis nodosa.

4.The Use of Calcium Sulfate as a Bone Substitute.

Bone defect occurs due to various causes, such as neoplasms, cysts, trauma, infection, congenital disease and surgical intervention. Autograft is generally considered to be the gold standard in bone graft surgery. But, allograft, xenograft and bone substitutes have been used because of complication and limited quantity of autograft. Calcium sulfate is one of the bone substitutes. We reviewed and analyzed 18 cases with bone defects that were treated with calcium sulfate from January 1997 to April 1997. We used the Osteoset' pellets that contained calcium sulfate 98%, produced by Wright Medical Technology Inc. in U.S.A., to fill defects in all cases. Except in 2 cases, autogenous cancellous bone from patients own iliac crest was mixed with calcium sulfate from 30% to 50%. Follow-up time averaged 13.7 months(range, from 12 to 15 months). The degree of absorption of calcium sulfate and new bone growth at conventional roentgenography was represented by percentage at each follow-up. The results were as follows. First, 8 weeks after operation, 93% of calcium sulfate was absorbed. Second, 6 months after operation, nearly 90% of new bone growth was observed. Third, until last follow-up, there was no noticeable complication, such as infection or foreign body reaction. We concluded that calcium sulfate might be on effective material for bone defects because of rapid absorption rate and easy recognition of new bone growth.

5.Classification of the Calcaneal Fracture Based on Computed Tomography

The os calcis is most frequently fractured than any other tarsal bone and the displaced intraarticular fractures account for 60-75% of them. Because of complex contour of calcaneous, it is difficult to evaluate the pattern of fracture exactly by conventional roentgenograms. But recently, more detailed understanding of fracture pattern can be possible with CT scan, which is thought to be one of the most important factor in deciding method of treatment and predicting prognosis. From Jan. 1992 to Dec. 1992, intraarticular calcaneal fractures of 31 cases treated in Kyungpook university hospital were analysed preoperatively with CT scan and classified by Sanders classification system. The results were obtained as follows: 16 cases of 31 cases were classified as type II and this type were sudivided as IIA in 7 cases, IIb in 4 cases, IIC in 5 cases. Type III fractures were found in 9 cases and subdivided as III AB in 3 cases, III BC in 4 cases IIIAC in 2 cases. Type IV fractures were found in 6 cases.

6.Acetabular Anateversion of Normal Korean Adult Population

The aim of investigation is to calculate the mean value of the acetabular anteversion of normal Korean adult after closure of growth according to different gender and sex age group, to find out any significant difference of the value of acetabular anteversion to some other variables such as age, height, weight and radiographic inclination(abduction angle) of acetabulum. Transverse pelvic computed tomography(CT) scans of 360 right hips with no known pathology were examined. The study population consisted of 12 groups by gender (male and female) and age(from third to eighth decade). The number of each group was thirty. The scanning slice nearest the middle region of the acetabulum (nearest through the center of the femoral head) on the scout film was chosen. A measuring point was assigned at the anterior edge of the acetabulum and second at the posterior edge. The line connecting these points was drawn. The angle formed by this line and the plane sagittal to the pelvis was determined as acetabular anteversion. The data analysis was run on a statistical analysis system(SAS). The mean value of acetabular anteversion 19.8±5.4 degrees(17.1±5.6 in males, 22.2±5.4 in females), it was greater significantly in females than in males (p=0.0001). The mean value of acetabular anteversion in third decade male group than the third, fourth and fifth decade group significantly(p < 0.05). The mean value of acetabular anteversion of Koreans was similar to Caucasians value. The value of acetabular anteversion was relatively releated to age. In conclusion, the acetabular anteversion of Korean females was larger than the Korean males. It was increased with aging. When we perform the total hip arthroplasty and pelvic osteotomy or reorientation procedure, we must consider the acetabular anteversion by different gender and age group.

7.Opponensplasty with Palmaris Longus in Carpal Tunnel Release.

Thumb opposition is essential for all precision handling, in daily activity. Thenar muscle wasting is common in long-standing severe carpal tunnel compression. Unfortunately, muscle wasting rarely recovers after decompression of the nerve. A single-stage procedure, combining carpal tunnel release and opponensplasty, is therefore a logical proposal. Between July of 1993 and May of 1996, 15 patients of carpal tunnel syndrome with thenar muscle atrophy were operated upon Camitz-Littler opponensplasty at Kyungpook University Hospital; combination of palmaris longus transfer and median nerve decompression as a simultaneous procedure. Postoperative thumb opposition was assessed by means of three separate indices-maximal palmar abduction, spatial rotation, thumb-to-finger apposition. Results for an average post-operative follow-up of 12.0 months were achieved 'good' by 86.7%. We concluded that palmaris longus transfer as an opponensplasty carried out in combination with carpal tunnel release is a simple and effective procedure for restoration of opposition in case of severe carpal tunnel syndrome.

8.Postoperative FP ( 5-Fluorouracil , Cisplatin ) Chemotherapy for Patients with High - Risk Gastric Cancer.

PURPOSE: Although adjuvant chemotherapy after resection of gastric cancer is a popular practice in Korea, there are still controversies about the effectiveness of the treatment. The fact that the relatively less effective drugs have been used and the rarity of large-scaled controlled studies may be partially responsible for the controversies. FP(5-FU, Cisplatin) combination is one of the most active regimen against advanced gastric cancer, consistently showing a response rate of 50~60%. We tried the FP chemotherapy as an adjuvant treatment for high-risk patients after curative resection of gastric cancer. MATERIALS AND METHODS: Between February 1992 and June 1996, 35 patients with completely resected high-risk gastric cancer(postoperative stage III or IV except thase with M1) received six courses of FP chemotherapy. Endpoints were toxicities of treatment, relapse free survival, and overall survival. RESULTS: With a median follow-up time of 17.1 months, Kaplan-Meier estimates of 2-year overall survival was 63.3% and relapse free survival estimates was 49%. There were no differences between stage III and IV patients in terms of overall survival or relapse free survival. Hematologic and non-hematologic toxicities were tolerable for most of the patients. CONCLUSION: Postoperative FP combination chemotherapy was tolerable for patients with high-risk(stage III and IV) gastric cancer. It is too early to determine the long term survival rates for this patients, but 2-year overall and relapse free survival were comparable to that of historical non-cisplatin containing regimens. Randomized phase III studies are warranted.
