PURPOSE: This study was to evaluate the clinical usefulness of 99mTc-MDP bone scintigraphy for assessing vascular ingrowth into the ocular implants after enucleation or evisceration. MATERIALS AND METHODS: Twenty-four patients (M:F=7:17, mean age: 36 years), who buried a coralline hydroxyapatite after uncomplicated enucleation or evisceration surgery were studied. Dynamic and static scintigraphy on the orbit fossa were obtained after injection of 740 MBq 99mTc-MDP to evaluate the status of vascularization. The study was performed from the 3 to 33 weeks after surgery. According to the visual analysis, activity greater than nasal bridge was graded as 4, equal to the nasal bridge as 3, less than nasal bridge but greater than normal orbit as 2, greater than normal orbit but less than grade 2 as 1. Uptake ratio was also calculated by measuring the implants activity (H) and contralateral orbit activity (N). Grading score and uptake ratio were compared with clinical outcome of vascularization. Additionally, we also analyzed the vascularization status as time lapse between primary surgery and scintigraphic study and surgical methods. RESULTS: Twenty-one patients who had bone scintigraphy at 11 weeks after surgery showed increased uptake above grade 2 and greater H/N ratio than 1.56. Of these, 19 patients who had drilling surgery for permanent peg application showed adequate bleeding during the procedure. The activity grade and uptake ratio were inversely correlated with vascular ingrowth. Higher than grade 2 or greater than 1.56 in H/N ratio seemed to be an indicator for better prognosis. Accomplishment of vascularization was not affected by the surgical way such as enucleation or evisceration. CONCLUSION: 99mTc-MDP bone scintigraphy can be a useful method to evaluate the vascularized status of implants. Adequate time for 99mTc-MDP bone scintigraphy may be 11-20 weeks after enucleation or evisceration.

DNA flow cytometric analysis was performed on paraffin-embedded tissue from 10 cases of squamous cell eareinoma (SCC) and 10 cases of basal cell carcinoma(BCC). These results were applicable to do a better prognosis in BCC than SCC. In 10 cases of SCC, the DNA index was 1.34 and aneuploidy was identified in 9. In 10 cases of BCC, the DNA index was 1.30 and aneuploidy was identified in 6.

Gallium-67 scintigraphy is helpful in the assessment of active extrapulmonary sarcoidosis. Muscular involvement of sarcoidosis is often asymptomatic or nonspecific, and laboratory examinations do not provide convincing evidence of muscular involvement. We report a case of muscular sarcoidosis, which was detected by gallium-67 scintigraphy. In a patient who was suffering from fever and arthralgia of knee joint, gallium-67 scintigraphy showed mediastinal and hilar involvement of sarcoidosis with unexpected extensive muscular uptake. Magenetic resonance imaging revealed the detailed depiction of intramuscular infiltration of sarcoid granuloma. Gallium-67 scintigraphy is useful in detecting inflammatory muscular involvement of sarcoidosis as well as other multiorgan involvement.

A 18-year-old male patient presented with the skin colored, dome-shaped, soft, movable mass in his left retroauriculr region. The mass has slowly increased in size since birth. Histopathologic exarnination revealed sebaceous gland, eccrine gland and crosssectioned hair in the surrounding tissue, hair shaft projected into the lumen, and laminated horny material in the cyst. Transitional zone between stratified squarnous epithelium and the wall lined by histiocytes showed homogenous degenerative change. After surgical removal of the lesion, no recurrence was observed during 15 months of follow-up.

PURPOSE:The International Labor Organization (ILO) has established an international standard for chest X-ray diagnosis of pneumoconiosis since 1980. However, there is a need for improved diagnosis and staging in occupational disease. We evaluated Ga-67 citrate scintigraphy quantitatively and correlated the scintigraphic findings with pulmonary function tests and chest X-ray result. MATERIALS AND METHODS: Twenty-five patients underwent whole body scintigraphy with additional chest and abdomen images 48 hrs after intravenous injection of 185 MBq of Ga-67 citrate. Ten normal controls were also studied. Regions of interest (ROI) were drawn on the posterior image to measure counts from the liver and lungs (Lung/Liver Ratio). RESULTS: L/L ratio according to the stages of chest X-ray classification were as follows; stage 0 (normal, n=10): 0.3948+/-0.0692, stage 1 (n=10): 0.5763+/-0.1837, stage 2 (n=11): 0.6849+/-0.1459, stage 3 (n=4): 0.9913+/-0.0712. There was a significant correlation between the scintigraphic L/L ratio and the X-ray stage (r=0.618, p<0.05). However, no significant correlation between L/L ratio and pulmonary function tests were observed (p>0.05). CONCLUSION: Quantitative Ga-67 scintigraphy can be a useful method for staging of silicosis. However, it is not a method to assess pulmonary functional impairment.

Using the 8 strains of Vibrio vulnificus isolated from the patients with primary sepsis, the viability of Vibrio vulnificus were studied in the various range of hydrogen ion concentration. The opitimal pH range of growth was 7-9, and survived between pH 3. 6 and 12. 5 when incubated at 37C for one hour.

We report a 44-year-old female patient with Eosinophilic cellulitis who had developed pruritic erythematous plaque on the right lower abdomen since two months ago. The blood eosinophil count was 2,066/mm at first visit. This lesion responded well to cortieosteroid therapy. However new erythematous swelling reappeared on the left lower abdomen and left inner thigh 3 months after the first treatment. A biopsy specimen taken from an erythematous plaque showed many flame figures and diffuse infiltration of eosinophils and histiocytes in the lower dermis and subcutaneous fat tissue.

One hundred patients of osmidrosis were treated by surgical procedure at the Department of Dermatology, Wonkwang University Hospital. The methods of operative treatment were elliptical excision for 37(74), bipedicle incision with apocrine glancl undercutting for 14(28), and single incision wit.h apocrine gland undercutt,ing for 49 (98). The results were summarized as follows : 1. Of the 74 axillae treated by elliptical excision, one occurred recurrence, and 1.7 showed complication, hematoma in 6, infection and delayed union in 4, each necrosis and severe scar in one each, 2. Of the 28 axillae treated by bipedicle incision with apocrine gland undercutting, 6 showed complication; hematoma in 4, infection and delayed union in one each 3. Of t..he 98 axillae treated by single incision with apocrine gland undercutting, 5 showed complication, hematoma in 3, infection and delayed union in one each In conclusion, single incision with apocrine gland undercutting was found to be more excellent compared with the other surgical methods.

Percutaneous transhepatic cholangiography(PTC) is of value in differential diagnosis of cholestatic diseases. Authors had performed PTC in 120 patients with flexible needle of 23 gauge at the Department of Radiology, KangNam St, Mary's Hospital and Myung Dong St. Mary's Hospital during the period from Jan. 1976 to April 1980. PTC was accomplished successfully in 112 of 120 patients. Diagnoses included cholangiocarcinomas (27 cases), arcinomas ofpancreas head (21 cases), ampullary carcinomas (4 cases), metastatic carcinomas(4 cases), metastatic carcinomas (5cases), bile duct stones(27 cases), sclerosing cholangitis(6 cases), hepatitis(6 cases), liver cirrhosis(6 cases), post operative adhesions(5 cases), chronic pancreatitis(3 cases), stomach carcinomas(3 cases), clonorchiasis(2cases), blood clot (1 case), and remaining normal 4 cases. Some characteristic PTC findings were (1) segmentalannular narrowing and abrupt complete obstruction and polypoid filling defects of the bile duct incholangiocarcinoma, (2) typical nipple or rat-tail appearance of the distal commmon bile duct in pancreatic headcarcinoma, (3) single or multiple sharply and smoothly outlined filling defects whthin bile duct in all cases ofstones, (4) complete obstruction with shallow concavity in ampullary carcinoma, (5) diffuse or segmental narrowingof the intrahepatic bile duct and common bile duct in sclerosing cholangitis, (6) multiple tiny polypoid fillingdefects of gallbladder, common hepatic duct, and common bile duct in clonorchiasis, (7) normal appearance inhepatitis. The overall diagnostic accuracy of PTC in 66 operated cases was 89.4%, and complications following the examination were signficantly reduced by using a fine flexible needle(23 gauge). From the present study it is concluded as follows; 1. To evaluate obstructed or stenosed site accurately, aspiration of bile juice must bepreceded by a 23 gauge needle connected to either 10mo or 5ml syringe. 2. To diagose carcinoma of hte ampullaryportion, serial spot filmings were indispensible ot demonstrated modified appearance of obstructive lesions due tocontraction of Oddi sphincter. 3. In contrast to other reports, the most common site of cholangiocarcinomas wasthe common hepatic duct (12 of 27 cases) in our series. Rat-tail appearance was seen in both pancreatic headcarcinoma and carcinoma involving the distal end of the common bile duct. 4. PTC was a more valuable method for diagnosis of clonorchiasis.
