1.A Case of Generalized Normolipemic Plane Xanthoma.

We report a case of generalized plane xanthoma which showed no-rmolipoproteinemia and was not associated with unerlying disorders or other type of xanthomas. A 48-year-old female has noticed well defined, slightly elevated, yellowish plaques which occurred on the periorbital areas, sides of the neck, scapular areas, anterior chest, buttocks and lower extremities for 3 years. Biopsy specimen taken from the neck showed perivascular accumulations of foam cells in the upper and mid dermis. Paper electrophoresis didn't, show elevation of any lipoprotein.

2.Immediate Hypersinsitivity Reaction in Chronic Urticaria.

In a large proportion of patients with chronic urticaria, at present, the definite tiology can't be found. There have been occasional attempts to identify the aIIergic causes uaing allergic skin tests, only ta suggest the Candicla albicans and. some food yeasts ae probabIe causes. Thia atady was designed to investgate how often patients with chronic urticaria from Chonnam Province react positively to skin tests with extracts of 42 common. allergene in the standard group Al-13 produced by Bencard Ltd. in England. The authora alao surveyed the clinical characteristics of 63 patients with chronic urtimria of more than 3 months duration, who bad visited the skin clinic of Chosun Univeresity HcepitaI from May. 1977 throug 3. Apr. 1979. The results were summarized as follows; 1. Among 63 patients, the maIe to female ratio was 1: l.2, and the peak incidence was in the 20s and 30s. 2, In regard to the duration of chroic urticaria, the highst frequency waa below one year duration. 3. Among 60s of the pa,tients who experienced seasonal changes, 32.4% had suffered aggravation during the winter. 4. The most common diseases combined with chronic urticaria were allergic rhinitis and atopic dermatitis, occuring in 9.5% of all subjects. 5. Of thase examined, 69.8% showed positive reactions to the prick test with the peak incidence in the 20s and 30s. 6. Among 42 allergens used far the prick test, 28 allergens turned out to be possibly casal agents, the rnost frequent group of allergens involved were grain du sts (27.1%), animal hairs(21.8%) and moulds(16.5%) in that order of frequency and occupying over 65% of the total positive reactions. And the most frequently involved specific allergens were hay dust, mixed grain dust straw dust and tree in that order of frequency. Moreover, 90 of the involved allergens were inhalants. 7. In teen-agers, the reactions to grain dusts, animal ha.irs, rnoulds and pollens were the rnost frequent while reactions to food allergens were most frequent in the 20s, and cotton and house dust are most frequently involved in tbe 40s. 8. Multiple sensitivity was not:d in a majority of cases and the most frequent combination was 2 to 5 allergens. 9. The rnost common abnormal hematologic finding in chronic urticaria was eosinophilia.

3.Pineal Anlage Tumor: A case report.

The term "pineal anlage tumor" has been recently proposed and few cases have been reported. We report the first Korean case of pineal anlage tumor in a 6-year-old girl who complained of headache and vomiting for 2 months. Brain MRI revealed a well defined, lobulated, calcifying mass in the pineal region. Tumor was totally removed. Pathological examination revealed a primitive pineal parenchymal tumor with melanotic epithelial component that was similar to histologic findings of melanotic neuroectodermal tumor of infancy, so-called retinal anlage tumor and of the developing pineal gland. The tumor was composed mostly of small, undifferentiated cells, Flexner-Wintersteiner rosettes, and ganglionic differentiation. The tumor also contained the cartilage and skeletal muscle cells.

4.Comparative Study on the Teeth Mobility between Modified Widman Flap and Undisplaced Flap.

Teeth mobility is an important part of a periodontal examination, because it represents a function of the persisting height of the alveolar bone and the width of the periodontal ligament. The purpose of this study was to evaluate clinical difference in teeth mobility after treatment with the modified Widman flap and the undisplaced flap in humans. Twenty males with moderate periodontal disease were selected. The severity of periodontal disease was evaluated with sulcular bleeding index, pocket depth and attachment level and tooth mobility was measured with Periotest(Siemens Co., Germany) at the initial examination, 1, 2, 4, 6, 8 and 12 weeks following the modified Widman flap and the undisplaced flap operation using the split-mouth technique. The relation of mobility to clinical parameters was statistically analyzed by multiple regression and the change of teeth mobility according to healing process by independent t-test using SPSS program. The results were as follows: 1. There was a strong relationship between the Periotest value(PTV) and attachment level. 2. The change of teeth mobility in both flap procedures was increased significantly at 1 week post-op. and was decreased to preoperative level at 4 weeks post-op. in modified Widman flap and at 6 weeks post-op. in undisplaced flap. 3. The change of teeth mobility in premolar teeth group in undisplaced flap compared to modified Widman flap was generally increased but these changes were not statistically significant. The changes of teeth mobility in molar teeth group in undisplaced flap was increased significantly at 2 weeks post-op.. 4. The change of teeth mobility following undisplaced flap was increased significantly compared to that of modified Widman flap at 2 weeks post-op.

5.The Value of Tc-99m DTPA Diuretic Renography for Assessment of Dilated Upper Urinary Tract in Children.

PURPOSE: The purpose of this study was to evaluate the accuracy of Tc-99m DTPA diuretic renal scans in children upper urinary tract. MATERIALS AND METHODS: We reviewed diuretic renal scans of 14 pediatric patients (age range: 3 days to 4 years) with unilateral hydronephrosis diagnosed by ultrasonography. Diuretic renal scan was done using Tc-99m DTPA and standardized protocol. In 3 neonates, diuretic renal scans were performed within 1 week and 3-7 months after birth. RESULTS: Six patients required pyeloplasty and eight were managed conservatively. All 6 patients requiring pyeloplasty were diagnosed as having ureteropelvic junction obstruction in the diuretic renal scan. In these 6 patients, post-operative renal scans at 3-12 months after surgery were converted to nonobstructive pattern in 5 and a nonobstructive patterns in 1. In 3 patients who underwent diuretic renal scan within 1 week after birth., nonobstructive patterns of initial scan were coverted to obstructive patterns in the follow-up scan. However, all patients with nonobstructive diuretic renal scans performed after the neonatal period did well on serial ultrasonography and showed favorable clinical outcome without progression to obstruction. CONCLUSION: Tc-99m DTPA diuretic renal scan with standarized protocol is useful in assessing suspected ureteropelvic junction obstruction in children as an initial diagnostic or post-operative follow-up modality. Nonobstructive or indeterminate scan RESULTS in the neonatal period requires follow-up scan to monitor development of the obstructive pattern.

Extrapelvic endometriosis is a rare disease. The majority of extrapelvic endometriosis cases involve scar tissue following obstetric and gynecologic procedures. We have treated two cases of extrapelvic incisional endometriosis. A 39 year old female patient with cyclic vaginal spotting after laparoscopic assisted vaginal hysterectomy due to uterine myoma and a 35 year old female patient with a painful palpable abdominal mass after cesarean section. Both underwent complete excision and were proven to have endometriosis by pathology. Here we report on both cases and review the medical literatures.
