3.The Study of Clinical Correlation between p53 Protein Overexpression and Ploidy in Transitional Cell Carcinoma of the Bladder.

The p53 gene product has been detected in various human tumors. To investigate the overexpression of the p53 protein in bladder tumor as a marker of tumor progression, the correlation between the overexpression of the p53 protein in bladder and the pathologic grade, stage and survival rate was studied. Among specimens of 47 patients, 19 were from patients of superficial bladder tumor, 28 were from invasive bladder tumor according to pathologic stage, while 15 were from low grade and 32 were from high grade tumors according to Ash grade p53 overexpression was determined by immunohistochemistry using DO-7 primary antibody on paraffin embedded sections and flow cytometry was performed on cell suspensions derived from the same blocks. Histologically normal bladder tissue was used as a control group Conclusions were made as follows: 1. p53 protein overexpression was observed in 31 of 47 cases(65.9%). 2. Seven of 15 specimens from well differentiated tumor(Ash grade I, II) showed positive p53 staining while 24 of 32 cases from poorly differentiated tumor (Ash grade III,IV) showed positive staining. There is a significant correlation between p53 expression and poor pathological grade(p=0.05). 3. Nine of 19 cases with superficial bladder tumor showed positive staining while 22 of28 cases of invasive tumor showed positive staining. There is a significant correlation between p53 protein overexpression and invasive tumor(p=0.027). 4. Flow cytometric examination revealed that 24 of 30 cases with aneuploid group showed positive staining while 7 of 17 cases with diploid group showed positive staining. Aneuploid group showed high correlation with p53 protein overexpression(p=0.007). 5. The survival rates of patients, exhibiting p53 protein overexpression, were slightly worse than those with p53 negative tumor(p=0.08, Wilcoxon test) but this difference was not significant enough to count. 6. p53 protein overexpression was well correlated with grade, stage and flow cytometric analysis in bladder tumor. 7. Multiple linear regression study to survival rate showed the overexpression of p53 protein had little prognostic significance in bladder tumor compared with tumor grade and stage.

5.A Case of Verrucous Cyst.

Verrucous cyst is a rare form of epidermal cyst showing verrucous changes of their stratified squamous epithelial lining, characterized by papillomatosis, acanthosis, and hypergranulosis with large, irregular keratohyaline granules. In addition, vacuolated keratinocytes, parakeratosis, and squamous eddies are often present. Herein, we describe a 45-year-old woman with a verrucous cyst, which occurred on left axilla three months ago, showing the above-mentioned characteristic histologic features. Both the immunohistochemical stain and polymerase chain reaction for the common antigen for human papillomavirus and DNA chip study for the human papillomavirus were negative.

6.The Hemolytic Uremic Syndrome Associated with Periappendiceal Abscess.

The hemolytic uremic syndrome (HUS) is clinically characterized by microangiopathic hemolytic anemia, thrombocytopenia and acute renal failure. The unique cause has not been determined but some bacteria such as E. coli was regarded as the causative agent of HUS in infant. A 30-year-old male patient was admitted due to acute abdomen. In operating field, the periappendiceal abscess was found, so the appendectomy with the drainage of abscess was performed. Initial hemoglobin level was 16.2g/dL but abruptly developed anemia (12.6g/dL) and thrombocytopenia (27000/mm3) was detected at hospital day 3. The urinary outflow was totally absent, the serum creatinine was 12.8mg/dL and the LDH was 3,650IU/L. The peripheral blood smear showed strong evidence of microangiopathic hemolysis. We performed total plasma exchange and hemodialysis under the diagnosis of HUS. The patient's renal function was markedly improved and the last creatinine was 1.9mg/dl. To our knowledge, this is the first report of HUS associated with periappendiceal abscess in Korea. We present a case and the review of literature.

The authors experienced a case of cervical periosteal chondroma presenting with symptoms of spinal cord compression in 26-year-old woman. The diagnosis was based on the characteristic features of computed tomographic scan and magnetic resonance imaging scan. Total surgical removal in two-staged operation was followed by full neurological recovery. Extensive spinal canal and extradural involvement in case of chondroma of the cervical vertebral column was observed and rarely reported in this literature.

8.Meta Analysis of Clinical Studies of Pregnancy and Delivery in Elderly Gravida.

OBJECTIVE: Our purpose was to identify the age-related inceased risks of the elderly gravida over 35 or 40 years at delivery by clarifying the effects of age and parity, their combination, and their interaction and the relationships of other complications. METHODS: We have made meta analysis about general and clinical characteristics of elderly gravida over the aged 35 or 40 years, based on the 7 domestic theses since 1996 year at the department of Obstetrics and Gynecology. RESULTS: The results were obtained as follows; In elderly gravida over the aged 35 or 40 years at delivery, maternal and neonatal outcomes were generally good, but in study group, there were a high incidence of gestational (preterm delivery, fetal presentation, pregnancy-induced hypertension, gestational diadetes, placenta previa, premature rupture of membranes) complications, uterine myoma during pregnancy and low birth weight in neonate after delivery. CONCLUSION: The increased risks of the elderly gravida over the aged 35 or 40 years at delivery may have been overshadowed by the previous focus on the elderly nullipara. In spite of high incidence of maternal morbidity, the overall maternal and neonatal outcomes were generally good. It is important to recognize what is more important in age-related pregnant-risks of the elderly gravida over the aged 35 or 40 years at delivery to appropriate counsel and manage this group of patients. This informations may be helpful for counsel in elderly gravida over the aged 35 or 40 years who are considering pregnancy.

9.A Rare Case of Pulmonary Epithelioid Hemangioendothelioma Presenting with Skin Metastasis.

Epithelioid hemangioendothelioma (EHE) is a well-differentiated and rare vascular tumor. Systemic metastases are uncommon. Herein, we present a patient with skin metastasis of pulmonary EHE (PEH) that was treated by wide excision. A 76-year-old male was evaluated due to pulmonary thromboembolism and a solitary pulmonary nodule. A biopsy was performed and pathological examination of the mass confirmed EHE. No metastasis was observed. The patient returned to care approximately two years later due to a painful nodule in the right lower leg. A skin biopsy showed metastatic EHE from the lung. We used a safety margin of 1 cm based on clinical experience, because no prior case had been reported regarding the resection margin appropriate for primary cutaneous EHE and skin metastases of PEH. At four months after surgery, the patient recovered without complications or recurrence. Skin metastasis of PEH is extremely rare, and only two cases have been reported in the literature. In this case, we report a rare case of PEH with histologically diagnosed skin metastasis that was successfully treated by curative resection. It is expected that this case report will provide a helpful contribution to the extant data regarding PEH metastases.

10.Simultaneous Laparoscopic Cholecystectomy and Endovascular Abdominal Aortic Aneurysm Repair.

Concomitant cholelithiasis and abdominal aortic aneurysm (AAA) is not uncommon. Cholecystectomy at the time of aneurysm repair was delayed in patients with asymptomatic cholelithiasis due to the possibility of the graft infection. If a patient is subjected to a high risk of aneurysmal rupture and symptomatic cholelithiasis, the combined cholecystectomy and aneurysm repair should be performed. With the advent of the minimally invasive surgery, minimal invasive approach is widely adopted in patients with cholelithiasis and AAA. We reported one patient who had symptomatic cholelithiasis and AAA, successfully treated with the laparoscopic cholecystectomy and endovascular AAA repair.
