2.The relationship between depressive tendency, somatic symptoms and drug use in housewives.

BACKGROUND: In women, the prevalence of depression is known to be more than twice as high as in men. Because many depressive women complain of their somatic symptoms instead of neurovegetative ones, many cases of depression have been misdiagnosed. Women also tend to use medicine more frequently than men do to manage depression. So we investigated the relationship between somatic symptoms and drug use in housewives for better management. METHODS: From 1st. March to 31st. March in 1996, the prepared questionnaires were presented to housewives living in the area of Kyung-ju city, who responded the questionnaires under direct interview. The questionnaire was composed of two parts. One part included demographic data, somatic complaints, drug use, places of drug purchase, current diseases that had been diagnosed by doctors. The other included Korean standard Beck Depression Inventory-questionnaire. The subjects were divided into two groups by BDI score(21 point), which was the cut-off point for depression in Korea. The relationships of somatic symptoms and drug use between the two groups were analyzed. RESULTS: There were 110 housewives who had depressive trends among 569 respondents. In general characteristics, there was significant difference between the two groups except the item of religion. In the items of somatic symptoms, there were more somatic symptoms in the group of high BDI score(>_21) except fatigue(P<0.01). More drug users were found in the group of high BDI score(>_21) (P<0.05). In the group of high BDI score(>_21), there was definite correlation between the items of somatic symptoms and drug use. There was no difference between the two groups in the item of drug purchase site. The most common site of drug purchase was the drug store in both groups. CONCLUSIONS: Depressive housewives complained of more somatic symptoms and more often used drugs as well.

Kasabach-Merritt syndrome is the association of thrombocytopenia, disorder of coagulation, spontaneous bleeding and enlargement of a hemangima or extensive hemangiomatosis, which can be often life threatening. We experienced a three years-old girl with suddenly enlarging hemangioma of right cheek, complicated by severe thrombocytopenia, consumptive coagulopathy and hemorrhagic cystitis. Generalized petechia and ecchymosis were noted on the overlying skin of hemangioma, trunk and extremities. But the study of MRI and angiography, hypevascular was located within the right masseter muscle extending to temporalis muscle and anterior to parotid gland. So selective embolization with PVA (polyvinyl alcohol) was performed through the distal branch of internal maxillary artery and facial artery. She was medicated with oral corticosterioid for 3 weeks. After embolizationn, blood flow to hemangioma and facial swelling were markedly decreased. Patient was followed up for 7 years and she showed normal contour of face and no bleeding diathesis.

6.Microleakage and water stability of resin cements.

STATEMENT OF PROBLEM: Recently, resin cements have become more widely used and have been accepted as prominent luting cements. Current resin cements exhibit less microleakage than conventional luting cements. However, the constant contact with water and exposure to occlusal forces increase microleakage even in resin cements inevitably. Most bonding resins have been modified to contain a hydrophilic resin such as 2-hydroxyethylmethacrylate (HEMA) to overcome some of the problems associated with the hydrophobic nature of bonding resins. By virtue of these modifications, bonding resins absorb a significant amount of water, and there may also be significant stresses at bonding interfaces, which may adversely affect the longevity of restorations. Therefore the reinforcement of water stability of resin cement is indispensable in future study. PURPOSE: This study was conducted to examine the influence of water retention on microleakage of two resin cements over the period of 6 months. MATERIALS AND METHODS: 32 ex acted human teeth were used to test the microleakage of a single full veneer crown. Two resin cements with different components and adhesive properties - Panavia F (Kuraray Co., Osaka, Japan) and Super-Bond C&B (Sun Medical Co., Kyoto, Japan)- were investigated. The storage medium was the physiological saline solution changed every week for 1 month, 3 months, and 6 months. One group was tested after storage for 1 day. At the end of the each storage period, all specimens were exposed to thermocycling from 5.C to 55.C of 500 cycles and chewing simulation of 50,000 cycles, and then stained with 50% silver nitrate solution. The linear penetration of microleakage was measured using a stereoscopic microscope at x 40 magnification and a digital traveling micrometer with an accuracy of +/-3 um. Values were analyzed using two-way ANOVA test, Duncan's multiple range tests (DMRT). RESULTS: Statistically significant difference of microleakage was shown in the 3-month group compared with the 1-day or 1-month group in bot systems (p<0.05) and there were statistically significant differences in microleakage between the 3-month group and the 6-month group in both systems (p<0.05). The two systems showed different tendency in the course of increased microleakage during 3 months. In Panavia F, microleakage increased slowly throughout the periods. In Super-Bond C&B, there was no significant increase of microleakage for 1 month, but there was statistically significant increase of microleakage for the next 2 months. For the mean microleakage for each period, in the 3-month group, microleakage of Super-Bond C&B was significantly greater than that of Panavia F. On the other hand, in the 6-month group, microleakage of Panavia F was significantly greater than that of Super-Bond C&B (p<0.05). CONCLUSION: Within the limitation of this study, water retention of two different bonding systems influence microleakage of resin cements. Further studies with the longer observation periods in viro are required in order to investigate water stabilty and the bonding durability of the resin cement.

7.Bile duct changes in rats reinfected with Clonorchis sinensis.

This study describes an evaluation of the sonographic, cholangiographic, pathological, and immunological findings, and the protective effect shown by rats reinfected with Clonorchis sinensis. Eight experimental rat groups were, namely, a normal control, a primary infection control, a reinfection I (reinfection 7 week after treatment following 3-week infection), a reinfection II (reinfection 2 week after treatment following 8-week infection), a reinfection III (exploration of the intrahepatic bile ducts 1 week after reinfection 4 week after treatment following 4-week infection), a superinfection, a secondary infection control, and an infection following immunization group. Sonographic and cholangiographic findings showed moderate or marked dilatation of the bile duct confluence in the primary infection control, reinfection II, and secondary infection control groups. Juvenile worms survived in the intrahepatic bile ducts 1 week after reinfection following treatment in the reinfection III group. It was concluded that reinfecting juvenile worms found during the first week following reinfection failed to survive or grow further. Anatomical, pathophysiological, or immunological changes may induce protection from reinfection in rats.

8.Therapeutic Effects of Norfloxacin(Neutam-400.

Norfloxacin is a new broad spectrum of antibacterial activity against both Gram-positive and Gram. negative bacteria. After oral administration of Norfloxacin, it produces high concentrations in the urine, so it has the excellent effect for the treatment of urinary tract infection. Norfloxacin was studied with respect to clinical effect in 115 patients with the urinary tract infection and the following results were obtained. 1. Excellent efficacy was 53% and efficacy rate was 90% in upper urinary tract infection(UTI). Efficacy rate for 20 patients with upper UTI without the underlying disease was 100%, and in 29 patients with underlying disease efficacy rate was 83%. 2. In upper UTI resistant for other antibiotics, efficacy rate was 78%. 3. In lower UTI excellent efficacy rate was 36% and efficacy rate was 71%. In gonococcal urethritis excellent efficacy rate was 76% and efficacy rate was 88%. 4. Eliminated rate of bacteria on culture findings were E. coli 93%, pseudononas 89%, proteus 100%, klebsiella 80%, gonococcus 88% and staphylococcus 80%. 5. Mild side effects such as epigastric pain, nausea, dizziness and itching sensation were seen in 5 cases. We consider Neutam-400 is one of the most effective agents in urinary tract infection.

9.Sexual Function in Patients with Spinal Cord Injury.

One important aspect for patients with spinal cord injury is adjustment to altered sexual activity. We studied for the evaluation of the sexual function in patients with spinal cord injury. The following results were obtained. l. 46 spinal cord injury patients were interviewed concerning erections, coitus, ejaculation and orgasm. 32 (69.6%) of t6 spinal cord injury patients were returned erections: 14 (30.4%) patients attempt coitus were successful, 7 (15.2%) patients could ejaculate, and 3 (6.5%) patients had orgasm. Psychogenic erection occur more with LMN lesions. Reflexogenic erection was shown at a higher incidence in patients with LMN lesion. In incomplete UMN lesions, both psychogenic and reflexogenic erection occur with the reflexogenic erection being more predominat. 2. In 8 impotent patients due to spinal cord injury, PBI, BCRLT and Papaverine test were measured. 1) In all patients PBI were greater than 0.75. This result was thought to be not related with vasculogenic factor in the impotence due to spinal cord injury. 2) In BCRLT LMN lesion and mixed type showed no response and delay, UMN lesion was within normal limit. 3) In 5 impotent patients due to spinal cord injury, 30mg papaverine was injected into the corpus cavernosum. One patient showed partial erection, and in 4 patient the penile rigidity was lasted upto 60-90 minutes. There was no complication. 3. The Jonas Silicone silver penile prosthesis was implanted in 3 spinal cord injury patients associated with the impotence and neurogenic bladder. There was no complication, no change of previous voiding pattern and no increased residual urine after implantation of penile prosthesis.

10.Lipid Profile and Its Association with Coronary Aneurysm in Kawasaki Disease.

PURPOSE: The value of serum lipid in children after recovery of Kawasaki disease may be important because of the predilection of this disease for the coronary artery. METHODS: We measured serum high density lipoprotein (HDL)-cholesterol, total cholesterol, triglycerides in 22 patients (mean age 38months, range 6 to 93 months) with Kawasaki disease during 10 days or less after onset and 2 months later after recovery. RESULTS: The level of serum HDL-cholesterol was depressed in the acute stage compaired with the level after recovery (33.6+/-15.2mg/dl vs. 54.8+/-8.3mg/dl, mean+/-S.D., p<0.05). Total cholesterol, triglycerides were normal during the acute stage (125.3+/-46.1mg/dl, 142.1+/-32.2mg/dl, respectively) and after recovery (147.4+/-35.5mg/dl, 130.6+/-74.9mg/dl, respectively). Echocardiography confirmed coronary aneurysm in 7 patients (31.8%) and otherwise, normal coronary figures (n=15). In the patients with coronary aneurysm, the level of serum total cholesterol, HDL-cholesterol were 134.0+/-34.1mg/dl, 34.5+/-14.9mg/dl, respectively and in the patients without coronary aneurysm 147.7+/-36.7mg/dl, 29.5+/-12.8mg/dl, respectively. They showed no significant differences between two groups (p=0.5, p=0.43, respectively). CONCLUSIONS: Significantly reduced HDL-cholesterol level, which is considered a risk factor for coronary atherosclerosis, is transient finding in the acute stage of Kawasaki disease and not associated with the incidence of coronary aneurysm. Further basic research is needed to elucidate the pathophysiology by which this abnormality is produced.
