1.Mineral Water Investigation on 10 Area in Seoul.

We are calling the erupting ground water as drug water or mineral water in Korea and thinking those water and food making water. Authors tried to investigate the evidence of contamination by user on the 10 erupting ground water and gained following results: 1. All of the mineral waters on 10 area in Seoul were unfittable to drinking water standard. 2. In the view of the bacteriology 80% were contaminated by coliform group. 3. The highest value of the free carbon dioxide contents were 652.96 ppm at Sam Sun Mineral Water.

3.Evaluation of Meniscal Tears of Knee by Post

Tentative diagnosis of meniscal pathology is very helpful for the successful operative management including arthroscopy. Conventional arthrography has long been used to meet this purpose, but it has some limitations due to complex views and some difficulties in imaging the whole anatomical structures. Recent diagnostic advancement produced post-arthrographic HRCT and MRI for the compensation of these problems of conventional arthrography. Arthrography and post-arthrographic HRCT has been carried out on 49 knees in 43 patients. 38 knees among them could be confirmed by arthroscopy and following results were obtained. 1. Diagnostic accuracies of post-arthrographic HRCT for medial and lateral meniscal injuries of knee were 94.7% and 92.1% repectively. 2. Shape, contour of torn meniscus and the relationships between the torn fragments were better demonstrated with HRCT. 3. Horizontal tear could also be suspected by HRCT. 4. Peripheral detachment could be differentiated with synovial recess on the HRCT, especially in the reformat view, but was more clearly visible in arthrography. 5. Arthrography and HRCT can be used as a good complementary study for the evaluation of meniscal injuries.

4.A Clinical Analysis of Fatigue Fractures among the Navy Recruits: Report of 18 Cases

Among two groups of navy recruits undergoing training, one consisting of 500 enlisted men and another 100 newly enrolled midshipmen, 18 reported to Chinhae Naval Hospital with fatigue fractures during the period from February though September 1969. Following observations were made on these cases: 1. The incidence of fatigue facture was 1.4 percent among the enlisted men and 11 per cent among the midshipmen. 2. There were 6 cases of complete fraeture and 2 cases of incomplete fracture. In the remaining 10 cases, only periosteal reaction was evdent roentgenographically with no visible fracture line initially: fracture lines, however, became apparent on subsequent examinations in all the cases. 3. The fractures involved the femur in 4 cases and the tibia in 14 cases, a ratio of 1:3. 5 in latter s favor. 4. The site of predirection in the femur was the distal one third of the shaft, occurring in 3 of the cases. In the tibia the junction of the upper and middle thirds was most frequently affected, occurring in 10 of the cases. 5. All the cases in our series healed without any complications.

5.Clincal Observation on Infants of Diabetic Mother.

Chlinnical observation was conducted on 10 cases of infants of diabetic mother at II Sin Woman's Hopital and the Busan National University Hospital during 19 months from Jan. 1977 to Jul. 1978. The results were summarized as follows : 1. Frequency of diabetic mothers was 0.094% and most of them were in 3rd decade of age. 5 mothers treated, two of them treated irregularly, and five not treated during pregnacy had the mean duration of 3 years from onset of diabetic symptoms to delivery. 2. Diabetic mother whose mean gravidity was 3.6 had histories of toxemia(50%), natural abortions or stillbirths(19%), and I case of polyhydramniosis and pyelonephritis respectively. 3. Among 10 cases of infants of diabetic mothers (5 full terms and 5 preterms), four large for gestational age, three appropriate for gestational age, and three small for gestational age, seven of them were delivered with vaginal and three with C-S deliveries. 4. Among 10 cases of infants of diabetic mothers, two of three infants with hypoglycemia had Apgar score below 6 at 1 minute and died soon after birth, and I case showed hypoglycemic symptoms such as tremor, sweating and so on. Hyperbilirubinemia ranging from 10 to 18 mg/dl in 3 cases, RDS 2 cases, and congenital anomaly 3 cases were also observed.

7.Osteoid Osteoma: Preoperative CT Guided Nidus Marking.

PURPOSE: To evaluate the utility of preoperative nidus localization guided by computed tomography(CT). MATERIALS AND METHODS: In the light of clinical, radiographic, and imaging findings, osteoid osteoma was suspected in all ten patients studied. An osteoid osteoma of the tibia was present in seven cases, and of the femur in three. During the 24 hours preceeding surgery, a small hole was drilled directly over the nidus with a 15gauge Ostycut bone biopsy needle, using CT for control and guidance. The hole and needle tract were stained with Gentian violet and GE 9800 and Hi Speed Advantage CT (GE) were used to obtain both 2D and 3D images. RESULTS: Each osteoid osteoma was in the cortex of the bone, nidi ranged in size from 10x6x6 to 40x7x9mm and the sclerotic area around each varied in size between 150x18x14 and 50x15x12mm. Marking was successful on the first attempt in eight cases, on the second attempt in one case and on the third attempt in one case. The average time per marking attempt was 15 minutes. In the operating field, stained marking on the cortex was easily found inall cases. With the marking it was possible to excise only as much of the bone as necessary and none of the ten patients suffered complications either during or after the procedure. The diagnosis of osteoid osteoma was histopathologically confirmed in all patients. CONCLUSION: CT guided nidus marking of an osteoid osteoma assured precise localization during surgery ; bone resection was therefore minimal and the time required for surgery was reduced. The procedure is a simple, safe, effective method for precise and easy nidus excision.

9.Assessment of Genotoxic Hazard in Petrochemical Workers.

In order to evaluate the genotoxic hazard among workers potentially exposed to low level petrochemical substances, the analyses of micronuclei (MN) and sister chromatid exchanges (SCEs) in lymphocytes were performed in 46 male workers (as exposed group) and 46 nonexposed subjects (as control group). Mean frequencies of MN and SCEs (respectively, 12.9/1000 cells and 6.5/cell) in exposed group were very significantly higher than those (10.2/1000 cells and 5.4/cell) in control group. And there were also significant differences in mean frequencies of MN and SCEs adjusted for age, employment duration, smoking, and drinking between two groups. Median frequencies of MN and SCEs in exposed group were very significantly higher than those in control group. Frequencies of SCEs were higher in smokers than in non-smoker. Frequencies of MN in smokers, however, were similiar to those of non-smoker. Interaction between exposure and smoking on MN and SCEs induction was not observed. The results suggest that there is genotoxic hazard in high risk group like workers handling carcinogens in petrochemical plants and the analyses of MN and SCEs are useful biomarkers for the exposure to hazard substances even at the level below the exposure limit.
